A towing vessel on a rivers route must carry 3 day and 3 night visual distress signals. Day and night distress signals can be red hand-held flares approved under approval series 160.021 or 160.121 and are acceptable as both day and night signals. Vessel did not have any visual distress signals onboard. Place appropriate amount and type of visual distress signals onboard in accordance with 46 CFR Table 141.370.

The Master of a towing vesel must ensure that each crewmember participates in firefighting drills and receives instruction atleast once each month. The instruction may coincide with the with the drills. All crewmembers must be familiar with their firefighting duties. There was no record of firefighting training or drill conducted on the new vessel. Conduct, log, and document as appropriatte friefighting drill/training.

Each towing vessel must be fitted with a general alarm that has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads "Attention General Alarm-When Alarm Sounds or Flashes Go To Your Station". The Audible alarm is not loud enough to overcome the generators and there is no flashing red light on the lower deck of the Machinery Space. Install a flashing red light in the lower machinery space that is noticeable from the generator room.

The cable clamp connector for the hot water heater had wires exposed in the clamp instead of wire jacket. There was a receptacle on the exterior of the vessel without an outlet cover installed. Properly route Cable/secure water heater clamp and install outlet cover for the exterior receptacle.
