The owner or managing operator must maintain valid proof of registration. The EPIRB registration has a different vessel call sign listed when compared to the FCC station license. 46 CFR 141.380(d)

Rails or equivalent protection must be installed near the periphery of all decks accessible to the crew. Railing IVO aft steering was damaged and missing. 46 CFR 144.800(a)

(a) Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. (b) If a storage room is provided, it may be any room or compartment that is free of ignition sources. (c) If a dedicated storage cabinet is provided it must be secured to the vessel so that it does not move and must be either: (1) A flammable liquid storage cabinet that satisfies UL 1275 (incorporated by reference, see § 136.112 of this subchapter); or (2) A flammable liquid storage cabinet that satisfies FM Approvals Standard 6050 (incorporated by reference, see § 136.112 of this subchapter); or (3) Another suitable steel container that provides an equivalent level of protection. Vessel had (2) non-approved fiber-glass containers on deck containing paint and gasoline cans.

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Both life rings are cracking in multiple area exposing foam core. 46CFR141.230

Each towing vessel must carry at least one appropriately-sized lifejacket approved under approval series 160.002, 160.005, 160.055, 160.155, or 160.176, for each person onboard. Vessel's jackets don't meet any of these approval series. Ensure there are 08 jackets onboard which includes the 02 required for those on watch.

Stowage positions for lifejackets stowed in a berthing space or stateroom must be marked in block capital letters and the number of minimum quantity. - There are lifejacket labels marked on a cabinet near the galley which does not contain lifejackets. Remove this marking. - The cabinet which holds the lifejackets is not marked. Mark all stowage locations with block letters (including the watch stowage location), also include the quantity stored there.

Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. Handrails on starboard side of upper deck had been removed prior to annual inspection due to damage. Repair rails and provide photographs to marine inspector prior to engaging in commercial service. 46CFR144.800

Each towing vessel must be equipped with at least one fire axe that is readily accessible for use from the exterior of the vessel. Fire axe onboard installed in engine room fidley space bulkhead. Affix fire axe to exterior of the vessel and provide photograph of axe in location to the marine inspector. 46CFR142.227

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Vessel was not equipped with required navigation lights in accordance with Rule 30. Vessel restricted to daylight operations until required navigation lights are installed and provide photographs to marine inspector. 46CFR140.720

The vessel operator of each vessel required by § 155.720 to have transfer procedures shall maintain them current and shall require vessel personnel to use the transfer procedures for each transfer operation. Vessel not in compliance with transfer procedures due to no red light onboard for nighttime transfer operations. Restricted to daylight transfer operations until red light is provided onboard in accordance with the vessel's transfer procedures. 33CFR155.730

Vessel experienced pin hole leak on the starboard engine cooling hose. Replace leaking hose on starboard engine to be in compliance with 46CFR58.01-35.
