A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Diesel fuel was found in the forward center void as well as what appears to be waste oil in the forward port void. Determine the cause of diesel fuel and waste oil entering the void space to include an examination of the fuel tank and void common bulkhead and make repairs as necessary, to the satisfaction of an attending marine inspector.

Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails, OR in areas where space limitations make deck rails impractical hand grabs may be substituted. The outboard rails of the vessel were broken off due to facing wire damage and not replaced. On the port side, there is approx 10 foot gap that has no outboard rails or inboard grab rails. Ensure proper fall overboard prevention measures are in place on port and starboard side of main deck to the satisfaction of attending CG marine inspector.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Both port and starboard main diesel engines have oil leaking from turbine side of all turbo chargers. Repair all 4 turbo charger oil leaks and clean all residual oil residue from exhaust collection manifold and exhaust pipe prior to movement.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Entire engine room bilge has approximately 6-7 inches of oily bilge waste throughout even though it was pumped clean 7 days ago. Remove oily waste and repair leaks to prevent further water intrusion from shaft seal leaks to the satisfaction of attending CG marine inspector.

Policies must be in place that cover the owner or managing operator's approach for ensuring vessel compliance including, but not limited to polices on maintenance and survey, safety, the environment, security and emergency procedures. On 18JUL25, a discharge of diesel fuel into a navigable waterway occurred due to overfilling a fuel tank. As a result CG marine inspectors attended the vessel on 24JUL25 and again as a follow up on 29JUL25 and discovered a sufficient number of safety and environmental discrepancies and lack of TSMS record keeping to warrant an external vessel audit be completed by the TPO. This external audit must take place after all identified deficiencies have been corrected and prior to the vessel returning to commercial service. Audit results must be submitted to CG MSU Lake Charles upon completion.
