Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: High bilge levels; Low hydraulic steering fluid levels, if applicable; and Low fuel level, if fitted with a day tank. These three alarms do not have audio & visual alarms at the operating station. Provide a plan to our office of the temporary safety measures that will be enforced until installation occurs. All 7 alarms in 46CFR143.230(a) must also be tested twice in a five year period per 46CFR143.245(b) table, incorporate this testing interval into your TSMS to ensure it is completed at proper intervals. Plan to be submitted/ approved by our office prior to departing from New Orleans, LA. 46 CFR 143.230

The master of a towing vessel must ensure that each crewmember participates in fire-fighting drills and receives instruction at least once each month. It was observed from vessel logs that fire drills were not conducted in Aug 2019 and Feb 2020, 1) ensure management properly trains all crewmembers of the monthly fire drill records, and 2) enforces or develops a mechanism that has management verify these drills are being done at proper intervals, to include required contents of a drill listed in 46 CFR 142.245(c). Provide what this TSMS mechanism will be to our office, by 2 weeks. 46 CFR 142.245(a)

The safety of the towing vessel is the responsibility of the master and includes Compliance with the applicable provisions of this subchapter and with the TSMS. The master of the vessel did not properly report a brief fire per USCG Marine Casualty Reporting requirements, nor complied with the company's TSMS Section E.1, Incident Reporting. Conduct training with the master and other crew on board in regards to these reporting requirements. Furthermore, include in Section E.1 the dispatcher phone number, and an alternate point of contact other than the dispatcher if they are busy/not able to be reached. Recommend also placing all the USCG Sector Command Center names and phone numbers that the vessel routinely operates in for marine casualty reporting.
