Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure: all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Excessive amount of MDE oil found in bilge and along sides of engine. Clean engine room bilge to satisfaction of attending USCG Marine Inspector.

Propulsion machinery, including shafting and bearings must: Be maintained to ensure proper operation. Port and Starboard side stuffing boxes are leaking continuously. Shaft seals/stuffing boxes should not leak at all when shaft is not spinning, and only periodically while spinning. Repair/replace stuffing boxes for re inspection from attending USCG Marine Inspector.

Verify that all observed unsafe practices, fire hazards, and other situations are corrected, and that all required guards and protective devices are in satisfactory condition. Sight glasses in engine room need guards re-mounted. Observed practice found during last annual survey completed 30OCT22; still not corrected. Re-mount guards on all sight glasses in space to the satisfaction of the attending USCG Marine Inspector.

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. Wheelhouse breakers not completely labeled. Label all breakers in panel. Observed practice found during last annual survey completed 30OCT22; still not corrected.

All items referenced on CG835V for towing vessel CULLEN LANDOLT, dated 09APR2020 from Sector New Orleans, shall be rectified and addressed by vessel TPO. Once all items are rectified to the satisfaction of the TPO, a detailed written statement and corrective action reports from the TPO must be provided to Sector Mobile showing deficiencies have been satisfactorily cleared.

Inspect exhaust system and assess fire detection sensor that was activated to ensure sensor was not damaged and test sensor in precense of TPO. Provide U.S. Coast Guard Sector New Orleans Marine Inspector last fire detection servicing record.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: High bilge levels; Low hydraulic steering fluid levels, if applicable; and Low fuel level, if fitted with a day tank. These three alarms do not have audio & visual alarms at the operating station. Provide a plan to our office of the temporary safety measures that will be enforced until installation occurs. All 7 alarms in 46CFR143.230(a) must also be tested twice in a five year period per 46CFR143.245(b) table, incorporate this testing interval into your TSMS to ensure it is completed at proper intervals. Plan to be submitted/ approved by our office prior to departing from New Orleans, LA. 46 CFR 143.230

Fire drills must include testing of all alarm& detection systems by operation of the test switch or by activation of one or more devices. The vessel needs to obtain the fire detection system manual to know how to test the system monthly during drills, and obtain the proper heat or smoke equipment if need be. All crew must know how to test the system, conduct training. All By 13APR2020. 46 CFR 142.245(c)(3)

Emergency lighting must be provided for all internal crew working and living areas. The forward space of the engine room had inoperable emergency lighting. Repair and test under emergency power, by 13APR2020. 46 CFR 143.410(b)

The functional requirements of a TSMS include Procedures for verification of vessel compliance with this subchapter. The master of a towing vessel must ensure that each crewmember participates in fire-fighting drills and receives instruction at least once each month. It was observed from vessel logs that fire drills were not conducted in Aug 2019 and Feb 2020, 1) ensure management properly trains all crewmembers of the monthly fire drill records, and 2) enforces or develops a mechanism that has management verify these drills are being done at proper intervals, to include required contents of a drill listed in 46 CFR 142.245(c). Provide what this TSMS mechanism will be to our office, by 2 weeks. 46 CFR 142.245(a)

The safety of the towing vessel is the responsibility of the master and includes Compliance with the applicable provisions of this subchapter and with the TSMS. The master of the vessel did not properly report a brief fire per USCG Marine Casualty Reporting requirements, nor complied with the company's TSMS Section E.1, Incident Reporting. Conduct training with the master and other crew on board in regards to these reporting requirements. Furthermore, include in Section E.1 the dispatcher phone number, and an alternate point of contact other than the dispatcher if they are busy/not able to be reached. Recommend also placing all the USCG Sector Command Center names and phone numbers that the vessel routinely operates in for marine casualty reporting. Prior to departing from New Orleans, LA. 46 CFR 140.210(a)(2)

The safety of the towing vessel is the responsibility of the master and includes Compliance with the applicable provisions of this subchapter and with the TSMS. The master of the vessel did not conduct/log navigation assessments every change of watch as required, since departure from Mobile, AL, to New Orleans, LA. Train the master and mate on proper assessments and logging requirements per the TSMS and 46CFR140.635. Prior to departing from New Orleans, LA. 46 CFR 140.635, 46 CFR 140.210(a)(2)
