The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Vessel's Certificate of Documentation was expired. 46 CFR 67.313(a)

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Forward and midships life rings had faded vessel name. 46 CFR 185.604(b)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Forward and midships life rings had degraded retroreflective material. 46 CFR 185.604(i)

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel did not have record of monthly portable fire extinguisher inspections being conducted. 46 CFR 176.810(b)(ii)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical outlet in aft salon bar area was daisy-chained creating potential electrical hazard. 46 CFR 183.200(b)

An adult life jacket must be provided for each person carried on board a vessel. 12 Adult type 1 PFD's did not have approval information attached. Vessel is limited to carriage of 122 adults. 46 CFR 180.71

A power-driven fire pump shall be installed on each mechanically propelled vessel which is permitted to carry more than 49 passengers. Fire pump electric controller box had broken faceplate. 46 CFR 181.10-1

Spanner wrenches must be provided where a 40 millimeter (1.5 inch) diameter firehose is required by § 181.320(b). Existing vessels as of July 22, 2016 have 180 days to comply with this requirement. No spanner wrench was located at hydrant in accommodation space. 46 CFR 181.310(d)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date Charts of large enough scale to make safe navigation possible. Vessel did not have up-to-date charts. 46 CFR 184.420(a)(1)

Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Head pump installed with temporary extension cord as main power source. 46 CFR 183.200(b)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. The fire pump switch mount was broken. The switch was dangling free at the entrance to the the engine room. 46 CFR 183.220(a)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. AFT exit was unmarked and not readily apparent to a person in those spaces. 46 CFR 185.606

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel is missing placards for different manufacturers of life jackets. 46 CFR 185.516(a)

A hinged watertight bulkhead door must be fitted with a quick action closing devise operable from both sides of the door. FWD watertight door striker plates and dogs were painted, unserviced, difficult and slow to close. 46 CFR 179.330(b)

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck. Reach rod was not adequately secured and located behind ladder potentially interfering with personnel. 46 CFR 182.510(d)

Each vessel must - have permanent draft marks at each end of the vessel; or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. Vessel is missing these hull marks.

All equipment, including switches, fuses, lamp holders, etc., must be suitable for the voltage and current utilized. Found deck lights are spliced into the navigation light conductor & circuit. Provide MSC approved plans for the installation of the deck lights. 46CFR183.220(a) 46CFR183.115(c)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Found the following sails - Flying Jib, Fore Top, and the Main Top with numerous repairs and they cannot be repaired any further. The vessel is restricted from using said sails without OCMI authorization. 46CFR176.840

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Found sta lok turnbuckles not properly secured. 46CFR176.25-1

All machinery to be installed and maintained in such a manner as to afford adequate protection against machinery failure. Found packing gland leaking for the fire pump supply valve. Found union leaking in the fire pump supply piping. 46CFR182.01-5

All vessels shall be provided with a hand operated portable fire pump having a capacity of at least 5 gallons per minute. This fire pump shall be equipped with suction and discharge hose suitable for use in fire fighting. The hand portable fire pump may also serve as a bilge pump. Vessel does not have a portable fire/bilge pump on board. 46CFR181.10-5

In systems and applications where flexible hoses are permitted to be clamped: double hose clamping is required where practicable. Vessel has an overboard discharge hose hanging & not connected to anything in compartment below galley. 46CFR182.410 (d)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: charts of large enough scale to make safe navigation possible. Charts, Light Light, Colregs, and Coast Pilot are outdated. Light list, Colregs, and Coast Pilot were corrected on the spot, Charts remain outdated.46CFR185.20-35

For each watertight door, an indicator must be installed in the pilot house and at each vessels operating station from which the door is not visible. The indicator must show whether the door is open or closed. Vessel's number 2 Class 1 WTD indicating switch was inoperable. 46CFR170.270 (e)

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Vessel did not have a guard in place for the top engine. 46CFR177.35-10

An adult life jacket must be provided for each person carried don board a vessel. In addition, a number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets provided to meet this section. Vessel had 4 adult and 1 child PFD unserviceable. 46CFR180.71 (a)(b)

Junction boxes, connection boxes, and outlet boxes, for use shall be watertight. Connection box needed cover in compartment under galley. 46CFR183.10-20(k)

A vessel on an Oceans, Coastwise, Limited Coastwise, and Great Lakes routes must carry: Six hand red flare distress signals approved in accordance with 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including but not limited to, approval series, 160.121 and six hand orange smoke distress signals approved in subchapter Q in this chapter, or other standard specified by the Commandant. Vessel was missing six hand orange smoke distress signals. 46CFR180.68(a)

Conduct underway rigging examination/survey in the presence of Coast Guard Marine Inspector.

At each initial and subsequent inspection, the owner shall be prepared to have the vessel ready for inspections of hull structure and appurtenances, including superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Update vessel's current PMP to meet all requirments of NVIC 02-16. (A) Provide a Rigging Preventative Maintenance Plan that provides a level of detail commensurate with Appendix (2), Enclosure (1) of NVIC 02-16(Rigging Inventory/Preventative Maintenance Inspection Template (Sample)) or propose a mutually acceptable third party to survey the rigging and provide a report with a level of detail commensurate with either Appendix (3) or (4) to Enclosure (1) of NVIC 02-16 (Third Party Rigging Survey Samples A & B). Requirements to be satisfied not later than 01/31/2021.

Vessel is not in compliance with NVIC 02-16. (B) Step masts and make available for inspection. Take down rig and make available for inspection. Requirements to be satisfied not later than 12/31/2021.

A vessel exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock and internal structural examination at least once every two years. (46 CFR 176.600(c)(1))

The vessel is currently broadcasting (1) Length Difference.

Found 3 of the required adult life jackets were found to be non-serviceable. Witnessed vessel owner destroy in the presence of [name withheld] total persons allowed to 131.

Bilge suction strainers were missing/rotted in the forepeak and galley spaces.

Vessel needs to develop a written maintenance plan for their sail rigging equipment and components in accordance with NVIC 02-16.
