Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station, conspicuously marked, and located at the midpoint of such vessel, or 100 feet from the engine, whichever is the more practical. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine (such as by cutting off the intake air). STBD prime mover failed to secure during operational testing of the shutdown. Provide shutdown IAW regulation above prior to the carriage of cargo. 46CFR 32.50-35

All tank vessels are subject to the regulations contained in parts 50 to 63, inclusive, of subchapter F. The hydrostatic test pressure must be maintained for a minimum total time of 10 minutes and for such additional time as may be necessary to conduct the examination for leakage required by 56.97-30(d). Piping system failed to achieve or maintain 1.5 times the MAWP (Test Pressure 225psi) when tested by the attending Marine Inspector. MI observed water instripping line bowls in cargo tanks, all but #4 port, indicating possible leak by of the remaining seven 1" stripping valves. Repair/Replace valves and re-conduct hydrostatic test of the cargo system in the presence of an attending Marine Inspector prior to cargo operations.

If the periodic inspection reveals deficiencies in your vessels maintenance, you must make any or all repairs or improvements within the time period specified by the [name withheld] port cargo engine observed leaking fuel from the fuel filter. Identify and repair leak to the satisfaction of the attending Marine Inspector prior to use of the engine.

Complete periodic gauging of midbody in accordance with 46 CFR 31.10-21a. Coordinate mtg w/CG & PE to concur on gauging grid. Provide gauging survey results and an engineering analysis signed by a registered Professional Engineer.

Provide P/V valves that are IAW the PRIS or service existing P/V valves and re-submit calculations to the Marine Safety Center for review.

The vapor collection system must have a mechanism to eliminate liquid condensation, such as draining and collecting liquid from each low point in the line. Modify VCS piping to meet the above regulation to the satisfaction of the attending USCG Marine Inspector prior to next credit drydock.

The last 1.0 meter (3.3 feet) of vapor piping before the vessel vapor connection must be painted red/yellow/red with the words "VAPOR" in black letters 2" high. Vapor lines were not marked.

The vessel owner or operator must ensure restricted areas are designated on board the vessel and clearly mark all restricted areas and indicate that access to the area is restricted and that unauthorized presence within the area constitutes a breach of security. Restricted area signs were missing/painted over.

The vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel is beyond 3 months after the anniversary date. Conduct annual inspection prior to cargo operations.

The last 1.0 meter (3.3 feet) of vapor piping before the vessel vapor connection must be: (1) Painted red/yellow/red with: (i) The red bands 0.1 meter (0.33 feet) wide, and (ii) The middle yellow band 0.8 meter (2.64 feet) wide; and (2) Label "VAPOR" in black letters at least 50 millimeters (2 inches) high. The barges vapor line is not properly marked with some areas unpainted and "VAPOR" stenciling not visible.

Vessel's starboard cargo pump would not start, therefore emergency shutdown could not be tested. Vessel restricted to only using port side cargo pump.

Cargo hatches are to be marked "Danger Keep Out"

Install containment plug retention lanyards.

Mark the Fuel shut off valves.

Renew barge name or areas that are deteriorated IAW 46 CFR 32.05-15.

Clean and service all P/V valves. Prepare them for inspection with bluing compound.

Vessel's annivesary date is 04 February. Vessel failed to have its second annual or periodic inspection prior 05 May in accordance with 46 CFR 31.10-17.

Replace red warning light prior to conducting night transfers.

Replace temp gauge on port prime mover.

Place currently serviced fire extinguishers onboard prior to carrying cargo.

Service anchor release and retrieval system.

Barge is restricted to carriage of cargo with a closed cup flashpoint of 140 F or greater. Address/Remove all non-explosive proof electrical installations in the hazardous locations zone to the satisfaction of the attending USCG marine inspector by 31 OCT 2009. Advise/obtain approval from this office prior to any modifications or installations.

Remove all electrical components that fall with in hazardous location zones to include navigation lights, switches, junction boxes and wire in conduit or replace existing with electrical components suitable for Class 1/Division 1 locations to the satisfaction of cognizant [name withheld] this office prior to any modifications or work being completed.

Both rudder hatches found with non functional hatch dogs, wasted gaskets and voids under them full of water. Vessel must repair both hatches to watertight / gastight condtion prior to commercial operations.

Vessel found in non-compliance with it's vessel response plan in that no drills or exercises had been executed nor logged in the past three years in contrary to the plans requirements to conduct drills and log same once each quarter. Prior to commercial operations conduct a major drill to sat of USCG.

Provide a written proposal to [name withheld] that outlines how company will come into compliance with it's VRP with regard to drills and exercises and logging of same.

Flame screen of vacuum breaker found fouled with grease and oil and ineffective. Clean to satisfaction of USCG prior to commercial operations.
