(a) Except as provided in paragraphs (b) through (g) of this section, each tank vessel must undergo dry-dock, internal structural, and cargo tank internal examinations as follows: (2) Vessels that operate in fresh water at least six months in every 12 month period since the last dry-dock examination must be examined in accordance with the intervals set forth in table 31.10-21(b). Double hull barge with external framing shall conduct internal structural exams every five years. The vessel is in fleeting awaiting orders to discharge cargo. Receiving ship isn't ready for the barges cargo to be delivered. Vessel is authorized a one time cargo discharge and must complete the internal structural exam in this Captain of the Port Zone before any additional cargo operations. At the #1 port void there are 3 rust pinholes within a 6''x6'' square. There were rust pinholes on the stern port and three on the stern starboard. Each were 1-2 inches wide. Upon discharging cargo the barge must not load cargo until proper repairs have been made to the satisfaction of the attending marine inspector.

The cargo pump prime mover would not secure the engine due to the air dampener not shutting all the way. Also during testing of the emergency stop the engine would not restart and required a starter change out.

Prior to departure from MM 707.2 on the LMR, provide for review a written damage survey report form a recognized marine surveyor to include a detailed description of the damage and any damage control measures necessary in order for the vessel to safely complete its voyage.

INITIATE AND/OR PROVIDE DOCUMENTATION OF A BENZENE MONITORING PROGRAM AND MARK REGULATED AREAS ON THE BARGE AS NECESSARY. THIS REQUIREMENT SHALL BE COMPLETED TO THE SATISFACTION OF THE COGNIZANT [name withheld] 05JUL92.

COMPLY WITH UNRESOLVED ISSUEDS ADDRESSED IN G-MSC PLAN REVIEW LETTER DATED 06JUN91, PARAGRAPHS 1, 4-7, AND 9-11. THE VAPOR RECOVERY SYSTEM IS NOT TO BE USED UNTIL THE ABOVE ITEM ARE COMPLETED.

COMPLETE INSTALLATION OF HIGH LEVEL SENSORS AND SHUTDOWN, PRIOR TO ANY CARGO OPERATIONS.

PLACE ALL REQUIRED HULL, CARGO TANK, AND OTHER MARKINGS ON VESSEL, PRIOR TOANY CARGO OPERATIONS.

INSTALL ALL PV VALVES FOR CARGO TANKS AND VAPOR RECOVERY SYSTEM, PRIOR TO ANY CARGO OPERATIONS.

PLACE COPY OF OIL TRANSFER PROCEDURES ON BOARD VESSEL PRIOR TO ANY CARGO OPERATIONS.

INSTALL ALL REQUIRED WARNING SIGNS AND NOTICES, PRIOR TO ANY CARGO OPERATIONS.

CONDUCT TEST OF CARGO PUMP ENGINE EMERGENCY SHUTDOWN, PRIOR TO ANY CARGO OPERATIONS.
