Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. If a dedicated storage cabinet is provided it must be secured to the vessel so that it does not move and must be either a flammable liquid storage cabinet that satisfies UL1275 or a flammable liquid storage cabinet that satisfies FM Approvals standard 6050, or another suitable steel container that provides an equivalent level of protection. Designated storage cabinet is severely corroded and needs replacement. 46CFR142.255

All hand-portable fire extinguishers, semi-portable fire-extinguishing systems, and fixed fire-extinguishing systems required by this part must be approved by the Commandant (CG–ENG). Without manufacturer prescribed bracket for marine use, the additional 40-B fire extinguisher for designated storage cabinet does not constitute approved equipment. 46CFR142.215(a)

Each escape hatch and emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 2 inches high "EMERGENCY EXIT, KEEP CLEAR". Emergency exit in E/R was not marked properly. 46CFR144.160(f)

Exercises must be conducted at least once each calendar year, with no more than 18 months between exercises. Each exercise must test communication and notification procedures, and elements of coordination, resource availability, and response. Exercises are a full test of the security program and must include the substantial and active participation of relevant company and vessel security personnel, and may include facility security personnel and government authorities depending on the scope and the nature of the exercises. Vessel does not have records of a full scale security exercise from within the past 18 months. Complete a security exercise within 30 days, document it in accordance with 33 CFR 104.235(b)(2), and submit documentation to USCG for review. 33CFR104.230(c)

Cable and wire on a towing vessel must be installed to meet the following requirements: multi outlet adapters may not be connected to other adapters or otherwise used in a manner that could overload the capacity of the receptacle. Remove all multi outlet adapters.

Each towing vessel operating in Oceans and Coastwise service must have a line throwing appliance with an axillary line that is at least 1500 feet long, has breaking strength of at least 9000 pounds force, and is, if synthetic, of dark color or certified by manufacturer to be resistant to deterioration from ultraviolet light. Provide axillary line meeting regulatory requirements or alter service.

Any towing vessel with alternating watches or overnight accommodations must identify the duties and duty stations of each persons onboard during an emergency. Vessels watch quarter station bill did not accurately reflect duties of each individual. Alter document to encompass duties of qualified personnel.

Machinery and fuel tank spaces must be separated from accommodation spaces by a bulkhead. Doors may be installed provided they are self-closing type. Install self-closing type device.

Individuals who are not crewmembers onboard towing vessels must receive a safety orientation prior to getting underway or as soon as practical. Vessel did not have prescribed safety orientation. Provide orientation for non-crewmembers.

Prior to getting on her way the master of the navigational watch must examine and test steering gear, signaling whistle, propulsion, towing gear navigation lights, navigation equipment and communications. The vessel had no record of pre-underway checks being conducted.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists when essential system developed problems that require attention. The Axillary generators must be equipped with a RPM gauge.

Every towing vessel equipped with an inflatable survival craft must have, in conspicuous places near each inflatable survival craft, approved placards or other posted instructions for launching and inflating inflatable survival craft. Provide launching instructions on the bulkhead.

Survival craft must be stowed in accordance with the requirements of 46CFR199.130 of this chapter, as far as practicable on existing towing vessels. Vessel's liferaft was not able to be practicably launched or moved side to side. Company to provide adequate lifesaving arrangement/stowage to the satisfaction of the Marine Inspector.

The owner or managing operator of a towing vessel must establish procedures to address fall overboard prevention and recovery of persons in the water. Marine Inspectors observed vessel crew could not demonstrate recovery of a person in the water.

The frame of any size III, IV, or V semi-portable extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. Marine Inspectors discovered the B-V extinguisher was not properly secured (tied to handrail with rope).

A flammable liquid storage cabinet must satisfy UL 1275, FM Standard 6050, or be an equivalent level or protection. Flammable liquid storage cabinet does not meet the requirement nor does it meet the requirements in order to be granted special consideration.
