In applying the provisions of this subchapter, the OCMI may give special consideration to authorizing departures from specific requirements when unusual circumstances or arrangements warrant such departures and an equivalent level of safety is provided. Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected spaces including fuel tanks, and may require vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel reported under water allusion resulting to damage to compressed air cylinders. Following damage survey submit a repair proposal to be reviewed and approved by the attending USCG Marine Inspector. Once repair plan is approved by USCG Marine Inspector complete repairs to the satisfaction of the attending USCG Marine Inspector. 46 CFR 176.600 (d)

Vessels operations manual requires the constant monitoring for the build up of hydrogen gases. The forward hydrogen gas sensor is inoperable. Prove proper operation of the vessels hydrogen gas monitoring system. 46 CFR 175.550 Hawaii Atlantis Operations Manual - Jan 2021

Each vessel must have adequate emergency lighting fitted along the line of escape. The port aft emergency light failed to activate. Provide a functional emergency light. 46 CFR 183.432

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Stern light was not operational.

The owner or managing operator shall make a vessel available for drydock examinations, internal structural examinations, and underwater surveys (UWILD) required by this section. Vessel owner/representative must conduct UWILD with USCG attendance prior to vessel going back in service or vessel will be restricted from carrying passengers.

Several passenger smoke hoods were found to have the seal broken and were unservicable. Vessel restricted to as many passengers as servicible smoke hoods.

Provide saftisfactory class report for hull and machinery prior to carrying passengers.

Provide analysis report and company statement that all submarines in SECTOR HONOLULU ZONE have made proper repairs.

Conduct U/W operational tests to the satisfaction of CG Marine Inspector prior to carrying passengers.

Effect permanent repairs to the damaged area of the #6 port air ballast tank located at the upper support bracket.

Replace all outdated air sampling test tubes.

REPLACE THE UNDERSIZED VERTICAL THRUSTER MOTOR WITH THE PROPER SIZED VERTICAL THRUSTER MOTOR.

EXPEDITE SUBMISSION OF ALL REMAINING MTH AND MSC PLAN REVIEW COMMENTS.

SUBMIT STABILITY CALCULATIONS FOR THE SUBMERSIBLE WITHOUT THE DROP WEIGHTS SHOWING IT HAS POSITIVE BG BELOW THE SURFACE AND POSITIVE GM ABOVE AND THATONE OR BOTH OF THE HATCHES ARE CAPABLE OF BEING OPENED FOR OFF LOADING PASSENGERS WHEN SURFACED WITH OUT THE DROP WEIGHTS.
