Vessel has an electronic engine control system, a qualitative failure analysis (QFA) and Design Verification Tests (DVT) shall be provided and approved by Marine Safety Center (MSC). A test of vessel's loss of power to propulsion was tested but control system shall be reviewed by MSC. Vessel will have until 27DEC2024 for deficiency to be cleared.

Vessel is to place electrical configuration to it's original approved plan and provide single line drawing. If no plans are available, provide plans to MSC for review.

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of piping, controls, and valves, and the inspection and testing of alarms and ventilation shutdowns, for each fixed gas fire extinguishing system and detecting system to determine that the system is in operating condition. Marine inspector found the starboard side fire damper was not fully closing. Fix damper to ensure proper function and notify local OCMI once repairs are complete.

Found a bulkhead penetration in the A/C closet in the main passenger space that was not sealed and would allow the fixed firefighting to escape into another compartment. Seal bulkheaded penetration with approved material and notify local OCMI when repairs are completed.

At each initial and subsequent inspection for certification of a vessel, the vessel must be equipped with an adult size lifejacket for each person authorized. Marin inspector found one lifejacket with a hole in it. Replace life jacket to required number as per COI. Vessel passenger count is restricted to number of life jackets onboard. Notify local OCMI once life jacket is replaced.

The regulations in this part provide a means to minimize the use of intoxicants by merchant marine personnel and to promote a drug free and safe work environment. Provide local OCMI of current enrollment in a DAPI program.

Starter wires located in engine room bilge. Remove wires from engine room bilge.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Piping in forward storage overhead not connected to fitting/hanging free. Make repairs to piping.

Vessel rep has contract to replace upper deck roofing. If replacing in-kind, provide specifications of old and new steel being used. If increasing the metal thickness, submit new stability calculations to MSC.

Each inlet/discharge pipe that penetrates the hull below the waterline must have a means to prevent water from entering the vessel if the pipe fails. Raw water feed valve supplying vessel HVAC system was stuck in the open position. Must repair to satisfaction of the USCG Marine Inspector.

Each watertight bulkhead must be of sufficient strength and to remain watertight. Discovered pinholes IVO fuel line penetrations in watertight bulkhead separating engine space and fuel tank space. Must make temp repair to the satisfaction of the USCG Marine Inspector until permanent repairs can be affected at next dry-dock.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons and the vessel from electrical hazards including fire and electrical shock. Discovered the following electrical hazards that need to be addressed to the satisfaction of the USCG Marine Inspector: 1)dead end wires under throttle in wheel house,2)daisy chained extension chord for speakers in upper accom space,3)loose battery under wheel in wheel house.

Each window and its means of attachment to the hull must be capable of withstanding the maximum load from wave and wind conditions. Discovered cracked window in forward section on first deck adjacent to the bar. Must make repair by next dry-dock.

Discovered soft patches on upper accom deck located in the lower and upper aft section. Must make permanent repairs by next dry-dock.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes.Identified several fuel leaks in the E/R resulting in accumulated fuel in the bilge. Leaks were identified on the closest valve to the generator and a fitting located above the engine.

All escape hatches and other emergency exits used as a means of escape must be marked on both sides in clear legible letters at least 2" high "EMERGENCY EXIT, KEEP CLEAR". Exit from the engine room and A/C room do not have signs posted on the exits.

Electrical equipment on a vessel must be installed an maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. There are multiple dead end wires under the main console and engine room, outboard of the main engine.

Solenoid lead for engine cut-off & alarm for the pre-engineered fixed fire extinguishing system is missing.

Starboard side engine room fire damper failed to close properly due to two broken springs.

Fire main discharge pressure gauge indicated 34 PSI with a single fire hose.

Fuel supply line to stbd main is leaking slightly.

Type II reto reflective tape must be replaced on the fwd lifebuoy.

Only fire drill logged on Oct 24th and last MOB drill logged was April 3rd. Vessel operator counseled and documented.

The life line on the fwd life buoy suffeted sun damage and was breaking away when touched.

Sewage cap on the aft sewage tank was broken exposing the vent to the atmosphere.

Flame bowl was found to be missing on generator racor filter.

Stbd junction box in steering gear room leading to steering pump needs to be permanantly mounted.

Exposed wires wre found underneath wheelhouse steering consul. Wires must be removed or properly terminated.

A vessel to which this sub chapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Since its issued COI date of 08/25/2021; subject vessel operator has missed its Annual inspection date of 11/25/2022 and in doing so the vessel’s COI will no longer be valid and is suspended under the authority of 46 U.S.C. 3313(b)(4) until the required annual inspection is completed on the 28th of November 2022.

Vessel experienced water ingress through two (02) 1"x 1" holes in the bottom centerline hull in the number two compartment. Vessel operator must conduct the following: 1) Haul vessel out of water at dry-dock facility and conduct complete spot gauging of all sections below the waterline and provide report to USCG 2) Provide repair proposal based on gauging report and must be approved by USCG 3) Conduct crop/renew repairs based on approved repair proposal and non destructive testing to the satisfaction of the USCG. All three (03) items must be completed prior to carrying passengers.

Filling and sounding pipes shall be so arranged that overflow of liquid or vapor cannot escape to the inside of the vessel. An active fuel leak was discovered in the union elbow flange of the fuel pipe connecting the bunkering station to the fuel tank while the vessel was bunkering. Vessel operator must make necessary repairs to stop leaking and conduct leak test to ensure no further leaks to the satisfaction of the Coast Guard prior to operating.

Vessel must maintain electrical safety standards to prevent fires and other safety issues. Dead end wire splices and broken junction boxes were discovered in the galley space, the lazerette space, the engine space and the fuel tank space. Must repair/replace junction box covers and remove dead end wiring or place in junction boxes to satisfaction of the Coast Guard.

Vessel crew must conduct fire and man overboard drills to demonstrate their familiarity with emergency procedures onboard the vessel to complete COI exam in the presence of Coast Guard Marine Inspectors prior to carrying passengers.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. It was discovered that the port Main Diesel Engine was missing a guard over the serpentine belt. Must install guard to satisfaction of the Coast Guard.

Batteries shall be located as high above the bilge as practicable and secured against shifting with motion of boat. It was discovered that all the battery boxes in the engine space were not properly secured. Must secure boxes to the satisfaction of the Coast Guard.

All hazmat in engine space must be properly secured for sea. It was discovered that buckets of hazmat were not secured in engine space. Must rectify to the satisfaction of the Coast Guard.

Vessels must have appropriate documentation for their routes. It was discovered that the vessel's Certificate of Documentation has been expired since 30JUN2021. Vessel is required to produce current Certificate of Documentation to the Coast Guard.

Vessel operator must produce current list of employees enrolled in the DAPI program.

All vessel crew members must demonstrate basic familiarity in operating all systems in regards to emergency response. Crew members misaligned bilge pumping system while testing suction in lazerrette space. Master must conduct re-familiarization training with crew members in regards to operating bilge pump system.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Identified active water leak at PORT main engine gear driven bilge pump.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Posted Certificate of Documentation onboard was expired as of 30 JUNE 2020.

Provide report stating replacement transmission is "In Kind" replacement.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its design limitations. The starboard transmission is not operating as intended. Provide a third party technician report attesting to the cause of failure and the condition after repairs. Repairs and sea trials to be made to the satisfaction of attending marine inspector. 46 CFR 182.200

Engine room fire dampers do not fully close in the event of a engine room fire due to severe corrosion.

Fuel Oil supply cut off valves do not fully close.

All vessels shall carry the required amount of pyrotechnic distress signals.

Steering ram installation bolts are loose. Tighten and torque as needed.

Vessel does not have a Bridge to Bridge FCC Certificate as required.
