A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Vessel was found to have damage to the stern head log, port side. There is an area, approximately 2ft x 2ft, that needs to be cropped and renewed.

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have under or around each loading manifold and each transfer connection point, a fixed container or enclosed deck area that, in all conditions of ship list or trim encountered during the loading operation, has a capacity of at least two barrels. Port and Starboard containments were partially filled and no longer capable of meeting containment requirements. Remove product as required.

4" crack located in the port aft ballast tank at the knuckle below the water line. NDT completed and found all metal surrounding crack within 75% of original. Crop out and replace a plate 18" x 18" x .5" IAW NAVIC 07-68. Conduct pressure test of repair in the presence of a Coast Guard Marine Inspector.

Cargo piping systems shall be arranged and fabricated in accordance with this section and Subchapter F. Piping, including valves, pipe fittings and flanges, conveying vapors, gases or liquids whose temperature exceeds 150 °F., shall be suitably insulated where necessary to preclude injury to personnel. Steam pipe insulation wasted. Replace insulation as required.

The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel representative reported a 4" crack located in the port aft ballast tank at the knuckel below the water line. Vessel rep has applied a temp repair preventing water intrusion. Vessel will transit directly to Bunola PA to discharge cargo and then dry-dock in Georgetown PA for inspection and repairs. While underway the following precautions will be made: - Have a dewatering pump on standby - Conduct regular sounding of space to monitor water level and log in towing vessel record. Once in drydock, the vessel operator must submit a repair proposal containing permanent repair plans.

4" long in the port transition knuckle

Barge has (3) 0"-3" x 1/16" hairline fractures in the #1 Stbd bilge knuckle, at aprox 12", 24", and 30" aft of the fwd bulkhead.

Small hole was found in port bow bilge knuckle. Owner cropped out a 4' x 6' section of the port bow rake bilge knuckle.
