The pressure guage for the port side air receiver had too small of a range (guage tops out at 200psi, same MAWP as the tank and below the allowable range of the relief valve). Vessel to too install a pressure guage with a range 1.2-2x the setting of the relief valve (relief valve was set to 190psi).

Each towing vessel must be operated in compliance with applicable pollution prevention laws and regulations. The stuffing boxes below both dripless shaft seals had oil in them and electric bilge pumps set to automatically pump overboard, which could create an unlawful pollution incident. Stuffing boxes should be cleaned and the automatic bilge pumps removed and the discharge piping secured.
