Each rudder angle indicator system must have a transmitter at the rudder head that is actuated by movement of the rudder with the angular movements of the rudder transmitted to a remote indicator or indicators. Readings presented from rudder angle indicators on the bridge were inconsistent with readings at the local steering position. Correct during next dry dock period. 46 CFR 113.40-10(a)

Existing arrangements, materials, and facilities previously approved shall be considered satisfactory so long as they meet the minimum requirements of this paragraph and they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. MI's observed sprinkler manifold valves dripping. Repair to the satifaction of the attending MI at next DD. 46CFR76.23-90(a)(1)

It shall be the duty of the master to see that all watertight doors...are in proper operating condition. MIs observed watertight doors #1 and #2 with loose dogs and #4 with a worn gasket. 46CFR78.17-3(a)

The generator set must be shut down automatically upon loss of lubricating oil pressure, overspeed, or operation of a fixed fire extinguishing system in the emergency generator room. While testing the installed CO2 system, the marine inspector witness that the emergency generator failed to shut down. 46 CFR 112.50-1(g)

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Marine Inspector (MI) observed the windings on the gear motors and the starboard generator had excessive grease build up. Clean to remove danger of fire. Cite: 46 CFR 72.03-5

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. MI observed the six (6) pressure valves in the emergency generator room had no record of testing. Test and record accordingly. Cite: 46 CFR 77.03-1

§ 76.10-5 Fire pumps.(a) Vessels must be equipped with independently driven fire pumps in accordance with table 76.10-5(a). (c) Each pump shall be capable of delivering water simultaneously from the two highest outlets at a Pitot tube pressure of approximately 50 p. s. i. Where one or both of these outlets is a 11/2-inch siamese fitting, both branches of the siamese fitting at each such outlet shall be utilized for the purpose of this requirements. (d) Fire pumps shall be fitted on the discharge side with relief valves set to relieve at 25 p. s. i. in excess of the pressure necessary to maintain the requirements of paragraph (c) of this section or 125 p. s. i., whichever is greater. Relief valves may be omitted if the pumps, operating under shutoff conditions, are not capable of developing a pressure exceeding this amount. Currently the pressure and the relief valve operation is not IAW this CFR cite. Make proper repairs prior to sailing. Contact USCG Inspections SECTOR NOLA when repaired. Deficiency Cleared 28Apr2022.

§ 72.03-5 Fire hazards to be minimized.(a) The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Currently the Bow Thruster engine H/P F/O line to number 5 cylinder is leaking at the engine head. Replace this defective F/O line and contact the USCG inspection at SECTOR NOLA when completed. Vessel not to sail until repaired. Corrected 28Apr2022 Cleared.

Vsl broke away during hurricane IDA went aground. Shall provide damage survey report and transit plan to DD prior to movement.

46 CFR 58.05-1 - The arrangement of auxiliary machinery must be at least equivalent to the standards established by ABS. It was reported that vessel's bow thurster failed due to a log stuck in the propellor. Provide technician's report attesting to proper operation of the bow thruster.

46 CFR 58.05-1(a) - The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Vessel representative reported that the vessel's bow thruster again failed, divers confirmed bent propeller due to previous debris casualty. Conduct proper repairs and provide a technician's report. Operational testing to be conducted with a CG Marine Inspector in attendance, prior to the carriage of passengers.

The CO2 activated door closure for the engine room was being held open by a coat hanger instead of being held open by the CO2 activated closure solenoid. A VERY DANGEROUS issue which was corrected on the spot. This deficiency was issued and cleared during the inspection.

During the testing of the EDG and its automatic starting and power supply system, it malfunctioned and the generator did not come on line. Problem resolved but not the satisfaction of the USCG inspector. Provide a third party electrical evaluation of the problem and supply a report with findings and corrections prior to getting the vessel underway.

1. Prior to getting the vessel underway with passengers the vessel must show compliance with 33CFR 26.01 through 26.09 FCC regulations for radio communications after it was reported by pilots communications could not be established. Also to ensure the radio equipment is not at fault a radio tech. will need to examine the ships radios and make a report on their condition. This must be submitted the USCG.

10" to 12" of accumulated water on the bilge under #2 MDE due to visible leak coming from port shaft seal supply valve sea chest. Provide repair plan and timeline to correct issue.

Two foundation bolts on Bow Thruster hydraulic equipment were loose. Make appropriate and permanent adjustment to secure bolts.

No emergency steering instruction in steering room. Provide permanent posted instruction for engineers onboard not familiar with the system.

Vessel does not have an up-to-date drill logs (fire drill, MOB, security). Binders are disorganized and include different vessel's drill report. No PVA onboard during inspection.

Sprinklers above rescue boat are not operational. Clean and test sprinklers to the satisfaction of attending Coast Guard Marine Inspector.

Securely fasten cable runs with metalic fasteners throughout engine and fuel compartments.

F/O transfer hose on board SR# 10825515-1 does not match any hose record on board. Remove hose or place documentation on board.

Drill log shows SSMCP and Fire drills conducted by both shifts on 6/3/18 but only one corrosponding drill sheet could be found. Find and log second drill sheet.

The drill log folder is in sad shape and requires attention drill logs falling out an d not secure. Fix same.

The pilot house alarm panel malfunctions when the test light is pushed all lamps go out instead of on for testing. Repair same.

The secondary anchor light is not operating. Correct same.

Currently on board PMS schedule not being followed or logged correctly. Correct this issue and schedule a complete automation test for the entire vessel.

Fire pump not taking suction.

Light not working.

Escape hatch to main deck not watertight.

Make with number of persons.

3 E-light not working on the 01 deck. Fix prior to carring passengers on the 01 deck.

Replace threaded flange on bilge piping. current one is pitted thrrough 50% of the flange.

Water tight door indicater lights are on when the door is closed instead of being on when the door is oper as required by 46 CFR 170.255(e)

Double nut or nyloc nut needed on foundation bolts.

Clean or replace clogged flame screen on oily-water tank vent.

Replace 26 child lifejackets throughout vessel. Current lifejacets were deteriorated.

Replace 2 emergency lights on the car deck.

Replace/Repair stbd solenoid for stbd SSDG that adjusts the governor speed and provide engineer's statement of satisfactory operation/installation.

Throttle control failed on stbd engine. Repair to the satisfaction of Marine Inspector. Provide technicians report.

Provide technician's report to secnola attesting to damage and repair. Prior to sailing.
