Compliance with the intact stability requirements applicable to each vessel, using a total weight of passengers and crew carried. Vessel has flooding in bow thruster room that need repairs prior to future voyages.

(a) The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Vessel reported a loss of directional control from Z-drive when transiting southbound at MM 725.5 UMR, and made way to Winona, MN. Determine cause, provide repair plan, and technical repair report upon completion.

§ 71.55-1 Permission required.(a) No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. (b) Drawings of alterations shall be approved before work is started, unless deemed unnecessary by the Officer in Charge, Marine Inspection. § 71.55-5 Inspection required.(a) An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. At the next vessel lay up period in Jan 2024, the STBD side shell where it was holed and temp. repaired on 05Jun2023 will be required to have NDT X-ray shot taken on the welding of the doubler plate and the results submitted to the New Orleans OCMI for review. Additionally, provide to this office a PE report attesting that the hull meets "Original Design Structural Standards". This must be completed prior to 28Feb2024 to the satisfaction of the cognizant OCMI.

Hatches and other openings. (a) It shall be the responsibility of the master to assure himself before leaving protected waters that all exposed cargo hatches of his vessel are closed and made properly tight. Currently the cargo hatch at the bow can now be made watertight with the new gasket and knife edge repairs. But it is not operating as designed from both sides due to the hatch warping and dogs needing repairs. This hatch is in need of replacement and must be replaced with an IN-KIND hatch. The date of compliance will be 28Feb2024 at the ending of the next maintenance period that begins 05Jan24. The crew must follow the written instructions on the securing of the hatch and water testing after each closure.

46 CFR 71.55-1 Permission required. (a) No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. § 71.55-5 Inspection required. (a) An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. Main seachest in starboard side lower engine room has a temporary synthoglass repair on seachest vent pipe, before the vent cut-off valve. Vessel is authorized to continue with current passenger carrying voyage. An enhanced temporary repair must be conducted on sea chest vent pipe prior to the next passenger carrying voyage. In addition, the crew shall conduct hourly visual rounds of sea chest piping until enhanced temporary repairs are made. Enhanced temporary repair shall be inspected by an attending Marine Inspector. Permanent repairs to the sea chest vent pipe must be conducted at next drydock.

Escape round hatch in the space forward of WTD1 (Chemical Locker) is seized open. Repair round hatch to allow for opening/closing operation.

Main cargo hatch on forward main deck has damaged knife edge in two locations and the watertight gasket is deformed throughout hatch gasket recess. Make repairs to cargo hatch knife edge and hatch gasket to regain watertight integrity and proper seal prior to next passenger carrying voyage.

Steam and Exhaust Piping: (3) Steam lines and registers in non-accommodation and non-public spaces must be suitably located and/or shielded to minimize hazards to any personnel within the space. xposed steam piping in port-side lower engine room adjacent to Azipod hydraulic supply tank and centerline, overhead lower engine room aft of oil separating tank.Renew insulation on exposed steam piping to shield hazard.

Lighting Fixtures: (b) Each fixture globe, lens, or diffuser must have a high strength guard or be made of high strength material. (d) Lighting fixtures must be installed as follows: (2) Each fixture and lamp holder must be fixed. Globe fixture directly above starboard Azipod missing guard and glass enclosure. 48” lamp fixture in starboard-side, lower engine room area forward of chiller unit is not fixed/mounted. Renew globe fixture guard and enclosure. Re-mount 48" fixture in designated location.

General: Electric installations on vessels must ensure: (2) Protection of passengers, crew, other persons, and the vessel from electrical hazards. 48” lamp fixture in starboard-side, lower engine room area forward of chiller unit has exposed electrical wiring; lamp is inoperable. Repair inoperable lamp and secure exposed wiring

(a) The following plans must be submitted to the Coast Guard for approval in accordance with § 50.20-5 and § 50.20-10 of this chapter: (7) Design Verification and Periodic Safety test procedures described in subpart 61.40 of this chapter. § 62.20-3 Plans for information. (b) One copy of a qualitative failure analysis must be submitted in accordance with § 50.20-5 of this chapter for the following: (a) (1) Propulsion controls Until the approved DVTP plans for the new bow thrusters have been received and the systems have been satisfactory tested IAW these plans and witnessed by a Coast Guard Marine Inspector, these thrusters must be locked out and tagged out and not used. An assist vessel will be in attendance at all times when the vessel is underway until this deficiency has been cleared.

Repair the port Z-drive to the satisfaction of the attending USCG Inspector prior to departing next port, Natchez, MS.

The watertight hatches on the bow of the vessel, in particular the Bow thruster hatch and the anchor chain hatch are wasted and require replacement. They currently can be made watertight but are in need of renewal. Current proposal is to have them replaced within 60 days. Replaced hatches to be to the satisfaction of the Coast Guard by April 18, 2022.

46CFR76.25 Where an automatic sprinkling system is installed, the systems must comply with NFPA 13 (Incorporated by reference; see 46CFR76.01-2. Currently the Sprinkler system flow valves were not able to be tested today due to some issues with the water/air in the system. Also the [name withheld] report submitted today was not satisfactory or acceptable. The report shows that they inspected 20 flow valves and the alarm panel shows there are 22 flow control valves installed. Also the report shows 7 tamper valves but only listed 4 by identification numbers. (reason given, there was no more room on the form) unsatisfactory answer. Vessel must provide an accurate report for the testing of this system and test again with the USCG on Friday 18Feb2022.

No repairs affecting the safety of the vessel with regard to machinery or equipment shall be made with out the knowledge of the local OCMI. The Ship Service Diesel Generator (SSDG) #2 is INOP due to internal fire. Please submit in writing a repair proposal to include scope of work. *** 705C *** Prior to operating the #2 SSDG with passengers on board, demonstrate proper operations of SSDG #2 under load to the satisfaction of the attending CG Marine Inspector.

All marine engineering details, including aux. machinery shall be designed, constructed, and installed in accordance with Subchapter F-Marine Engineering. Vsl struck debris field while underway, causing unknown damage to stbd stern z-drive system. Vsl to provide tech report and repair proposal to Coast Guard. Any repair work conducted shall be done with prior approval of the cognizant OCMI and in accordance with 46CFR50.05-10.

Every crewmember must participate in at least one abaondon-ship drill and one fire drill every month. Drills must be held before sailing when a vessel enters service for the fire time, after modification of a major character, or when a new crew is engaged. This vessel has been in laid up service for over 6 months and several of the crew members are new to this vessel. Drills shall be completed by [name withheld] prior to vessel carryin gpassengers for hire. 46 CFR 199.180 (c)

The VSO must ensure that at least one seucrity drill is conducted at least every 3 months, except when a vessel is out of service due to repairs or seasonal suspension of operation provided that in such cases a drill must be conducted within one weeks of the vessel's reactivation. Vessel has been laid up for over 6 months and several crew members are new to this vessel. A security drill shall be completed by [name withheld] prior to vessel carrying passengers for hire. 33 CFR 104.225 (b)

Alarm Signals intended for use in sleeping compartments may have a minimum sound level of 75 dB(A) measured 3 feet (1 meter) on axis, and at least 10 dB(A) measured 3 feet (1 meter) on axis, abover ambient noise levels with the ship under way in moderate weather. Vessel has 14 cabins that the speakers do not annunciate at all or do not annunciate when value on control know is on "0" level. Vessel is not allowed to carry passengers in the following cabins until cabins are verified in compliance by regulations. Cabins: 505, 523, 458, 457, 410, 408, 404, 402, 349, 332, 201, 160, 162, 152. 46 CFR 11325-12 (d)

Properly service the stbd rescue boat portable fire extinguisher (resolved, replaced with a serviced spare).

Clean the stern navigational lights (resolved).

Due to an earlier allison, an approximate 15 ft X 3 ft inset to the deck overhang, port side midship, resulted. Minor damage to a stairwell enclosure also resulted.

#2 SSDG is being overhauled. Vessel can operate with 2 remaining generators. Upon completion, the PSTPs must be used to test engine automation. This must be witnessed by an inspector prior to being placed in service. As an alternative the maufacturer tech reps can complete testing with PSTPs and document tests. Chief Engineer to witness and provide report to the Coast Guard.

Install proper battery charger for battery bing used as back up in pilot house.

Update safety procedures to include the Jacket Water testing for each generator. System is installed.

Multiple Daisy Chained power strips found in provisions office, remove and provide additional outlets as needed.

BLKHD penetrations observed in river grill liquor locker without fire stop.

Washing Machine in mates office using temporary electrical wiring , provide permanent installation.

Damage to hull near interface with pontoon on port side mid-point of boat.

Discharged due to initial action during small galley fire (grill).

The #2 SSDG shutdown while U/W northbound due to a suspected electrical problem with the easYgen electronic control module. Make permanent repairs to the control module and prove the proper operation of the generator to the local attending Marine Inspector.

Fire caulking above general alarm in electrical locker is missing.

Breaker cover for breaker box EP 210 BX in EDG room is missing.

In paint locker, white light sections are loose, red light swings and is not mounted.

FWD bulkhead in paint locker is missing and damaged fire caulking.

Rags and paint cloths improperly stowed within paint locker. Remove fire hazards from this space.

Improper caulking for bulkhead penetrations in aft part of compartment.

Outlet under the bar in the Front Porch Bar area is loose and needs to be properly mounted.

Lighting within the 3rd deck service stairs are mostly out. Repair or replace lights.

Portable CO2 extinguishers improperly stowed within "Squad Locker 2" (left free-standing).

CO2 bottle for soda fountain in Engine Room Bar improperly stowed within loose bracket. Regulator observed to be damaged by suspected impact with refrigeration unit, presenting hazard to crew. Replace gauge & regulator and mount bottle properly.

Gasket in fire screen door 17 observed to be in poor repair. Replace.

Food warmer in main galley observed with electrical wires pulled out of side of equipment. Repair.

Within Main Galley, in the scullery area water was observed leaking from pipes under the table creating a slipping hazard for the crew. Repair/replace.

Within the crew galley, the fire door on the left side of the space was not able to close properly as it was jamming against the deck. Repair/replace.

Within the generator room, remove all cardboard and rags from bilges. Fire hazard.

Leaks observed from all 3 generator fuel filters going into containment. Repair/replace.

Fuel oil ine brackets in generator room missing bolts. Repair/replace.

Insulation above #3 generator falling from overhead. Repair/replace.

Fuel oil sounding pipes in boiler room being held open. Remove means to hold open sounding pipe valves.

Prior to leading passengers clean all cardboard trash from the incinerator room currently a fire hazard and there is no passage through the space.

Prior to loading passengers correct the latching hanle on the newly installed fire screen foor. Currently the door will not latch properly when it closes using the door closure.

Prior to loading passengers find and replace the missing fuel shit off tool for the incinerator room fuel tank shut off valve on the outboard of the room.

This fire door in the galley that leads to the crew hold is sprung. Today a full length from handle was found in the door hinge holding the door open. This fire door is now sprung but will close, but still required replacement. Complisnce for the replacement is 30 days.

Remove hold backs on doors for the emergency generator room and paint locker.

Ensure Fire Station #1 is properly outfitted and do not store equipment and gear in front of the station.

Ensure the exposed exhaust fans blades that are installed in the upper portion of the door, in the store locker (closet), by fire screen door #44 are guarded and that the hole in the metal bulkhead is plugged. The hole is just inside the door, on the upper left side bulkhead and appears to be from an abandoned electrical wire stuffing tube.

Clean the debris (gloves, rags, toilet paper…) from behind the Mates Office washer and dryer.

Ensure the vessel has the proper lighting configuration for a vessel not under command as per COMDTINST M16672.2D.

Ensure the vessel has the right amount of day shapes (3) and lighting configuration for a vessel aground as per COMDTINST M16672.2D.

Properly adjust the lower door latch on fire screen door #12.

Replace the damaged retro on one PFD, located on top of the port, outboard, PFD locker.

Replace the missing overhead beam structural insulation in the incinerator room.

Ensure the overhead panels in the suspended ceiling in the Pursers Office are repaired. The metal support brackets are bent or dislocated and the panels are not properly supported.

Repair exhaust leak on #2 generator.

Repair lagging on hot water heater elbow and #3 generator exhaust.

1. The crew room door #00109 kick out panel is damaged and replaced with a temp panel. Install proper kick out panel in the door. Until the knock out panel has been replaced no one should use this room.

2. There is some overhead insulation missing in the store room compartment on the fourth deck located aft in the crew space. Replace insulation with the proper type for the space A-60.

3. The galley door to the incinerator room has again been damaged so it will not close properly all the time. Make proper repair so the door will close properly with any ventilation on and or replace the door.

4. In the lower level of the engine room port side wire nuts connections need to be properly connected and placed in a junction box. See Chief engineer for location.

5. Noted all life rafts were not properly installed, the sea painter to the weak links were not hooked up properly, this was corrected on the spot.

6. There were two life ring lights that did not have the proper screw down connections for the batteries, these were replaced on the spot.

7. Located in the Stbd “Z” room there is a deck scuttle that has a broken drain line and a section of the gasket missing, repair and replace as needed.

Make proper and permanent repairs to the number 2 SS/DG, test the repaired equipment and have a tech. report completed for the repairs and testing. Contact the local OCMI and have the repaireed ssytem examined and observed the safety test procedures for the engine. All must be completed to the satisfaction of the attending USCG Marine Inspector.

Make temporary arrangements and permanent repairs to the #2 generator.

Installed CO2 system in vow thruster room is inoperative.

Installed CO2 system in incinerator room is inoperative.

Provide guard for paint locker CO2 bottle control head to prevent inadvertent discharge.

Stbd "Z" drive failure. Input shaft is believed to have failed.

Several doors weere found to not close properly and some had gaps. Properly adjust to approved operation of self closing doors.

Some fire dampners failed to completely close. Verify proper closure of fire dampners throughout vessel.

Vessel's stbd Z-drive experienced a gear box failure. Vessel permitted to transit to Natchez, MS to execute repairs.
