Resolution: The vessels CE determined that the flooding was due to an open sea chest valve and clogged check valve. The sea chest was closed and the check valve cycled which stopped the flooding.

Vessel reported a loss of directional control from Z-drive when transiting southbound at MM 725.5 UMR, and made way to Winona, MN. (b) Drawings of alterations shall be approved before work is started, unless deemed unnecessary by the Officer in Charge, Marine Inspection. § 71.55-5 Inspection required.(a) An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. At the next vessel lay up period in Jan 2024, the STBD side shell where it was holed and temp. repaired on 05Jun2023 will be required to have NDT X-ray shot taken on the welding of the doubler plate and the results submitted to the New Orleans OCMI for review. Additionally, provide to this office a PE report attesting that the hull meets "Original Design Structural Standards". This must be completed prior to 28Feb2024 to the satisfaction of the cognizant OCMI.

Hatches and other openings. (a) It shall be the responsibility of the master to assure himself before leaving protected waters that all exposed cargo hatches of his vessel are closed and made properly tight. Currently the cargo hatch at the bow can now be made watertight with the new gasket and knife edge repairs. But it is not operating as designed from both sides due to the hatch warping and dogs needing repairs. This hatch is in need of replacement and must be replaced with an IN-KIND hatch. The date of compliance will be 28Feb2024 at the ending of the next maintenance period that begins 05Jan24. The crew must follow the written instructions on the securing of the hatch and water testing after each closure.

(a) No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. § 71.55-5 Inspection required. (a) An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. Main seachest in starboard side lower engine room has a temporary synthoglass repair on seachest vent pipe, before the vent cut-off valve. Vessel is authorized to continue with current passenger carrying voyage. An enhanced temporary repair must be conducted on sea chest vent pipe prior to the next passenger carrying voyage. In addition, the crew shall conduct hourly visual rounds of sea chest piping until enhanced temporary repairs are made. Enhanced temporary repair shall be inspected by an attending Marine Inspector. Permanent repairs to the sea chest vent pipe must be conducted at next drydock.

46CFR76.25 Where an automatic sprinkling system is installed, the systems must comply with NFPA 13 (Incorporated by reference; see 46CFR76.01-2. Currently the Sprinkler system flow valves were not able to be tested today due to some issues with the water/air in the system. Also the [name withheld] report submitted today was not satisfactory or acceptable. The report shows that they inspected 20 flow valves and the alarm panel shows there are 22 flow control valves installed. Also the report shows 7 tamper valves but only listed 4 by identification numbers. (reason given, there was no more room on the form) unsatisfactory answer. Vessel must provide an accurate report for the testing of this system and test again with the USCG on Friday 18Feb2022.
