All life saving appliances and arrangements shall comply with the applicable requirements of the Code. The launching mechanism shall be arranged that it may be actuated by one person from a position on the ship's deck and from a position within the survival craft. The launching mechanism to be actuated by a person inside of the survival craft was found uninstalled and laying outside of the #1 lifeboat. The crew re-installed it into the lifeboat prior to the completion of the inspection. SOLAS 74 (2004 cons.) Reg 34 LSA Code Ch VI 6.1.2.2

The company and the ship shall comply with the requirements of the ISM Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship, and protection of the environment. The various tasks should be assigned defined and assigned by qualified personnel. MI observed a fire detector isolated on the fire detection system bridge panel. The following was identified through additonal discussions with the crew members: the company has not not established written procedures or checklists using the onboard safety management system for when fire detectors are isolated from the detection system and the crew or contractors are performing maintenance onboard. The crew will verbally pass any fire detection isolation to the bridge watchstander and use an unofficial green logbook, however, there is no written procedure for this. The onboard unofficial verbal procedures for isolating the fire detection system fail to include: notification to the master when isolating single or multiple detectors from the fire detection panel, notifying the master whent the isolation is complete, establishing a fire watch/additional fire fighting precautions for the space/section when the system is isolated. The company and the ship shall comply with the requirements of the ISM Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish and maintain procedures to control all documents and data which are relevant to the safety management system. The company should ensure that valid documents are available at all relevant locations, changes to documents are reviewed and approved by authorized personnel, and obsolete documents are promptly removed. The onboard safety management system referred as SQMS requires the crew to use the program "Watchkeeper v 3.51" in order to log STCW work/rest hours. The controlled document in the SQMS that refers to this program is MAN-056-Compliance. MI requested to review the Watchkeeper program and identified that the license expired. The crew are unable to use this program as per the SQMS and document MAN-056-Compliance is still valid or has not been revised in the SQMS. SOLAS 74 (2020 cons.) Ch IX Reg 3.1 ISM Code Part A 11.1

In addition to complying with the specific provisions for fire integrity of bulkheads and decks of cargo ships, the minimum fire integrity of bulkheads and decks shall be as prescribed in tables 9.5 and 9.6. The following Class A fire doors were found with hold backs: galley doors 01-107-3, fwd paint locker door 2-10-2, stairwell door located at frame 85 3-20-0, and the compacted trash storeroom door. The door between the scullery and galley was found with a fan electrical cord installed in between the two spaces which prevents the door from closing. SOLAS 74 (2004 cons.) Ch. II-2 Reg 9.2.3.3.1

Ventilator openings shall be provided with weather tight closing appliances. The ventilator located on the A deck at frame 18 was found heavily corroded and mis-alligned with the gasket, preventing it from closing or dogging down. The ventilator is marked as 1-18-1. ICLL 1966 Ch. 1 Reg 19.4

Each power-operated sliding watertight door shall be provided with an individual hand-operated mechanism. It shall be possible to open and close the door by hand at the door itself from both sides. The handle for the WTD manual operation, fwd of the door, located at 5-86-2 was missing during the inspection. The crew constructed a new handle after the inspector departed the vessel. The manual operation of the WTD using the new handle was not witnessed by the [name withheld] 74 (2004 cons.) Ch II-1 Reg 25-9.2

Fire-fighter's outfits shall comply with the Fire Safety Systems Code. A firefighter’s outfit (ensembles) shall consist of axe with a handle provided with high-voltage insulation. In accordance with MSC/USCG MOA dated 22AUG2024, Annex III – Modification of Standards, 5.c, vessel was unable to produce two or four axes per fireman’s ensembles. SOLAS 74 (2004 cons.) Ch. II-2 Reg 10.1 FSS Code 3.2.1.1.5

Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fighting systems and appliances that they may be called upon to use. A crew member used a damage control hose for fire fighting during the fire drill. The damage contorl hose has a different thread than the fire hoses and nozzles. The crew member was unable to thread a fire nozzle on the damage control hose and after several minutes, a different crew member brought another fire hose to replace the damage control hose. The crew member was provided additional training by the officers onboard after the drill was completed. SOLAS 74 (2004 cons.) Ch. II-2 Reg 15.2.2.1

The company and the ship shall comply with the requirements of the ISM Code. The company should ensure that the policy is implemented and maintained at all levels of the organization both ship based as well as shore based. Numerous food stores on board were found to be passed their labeled expiry date with no documentation provided either extending their lifecycle or deeming them unfit for human consumption per NAVSUP P-486, Vol I, Rev 9 Food Service Management. SOLAS 74 (2020 cons.) Ch IX Reg 3.1 ISM (2018) Part A/2.2; MSC – USCG MOA 2024 – Annex III/5 (MSC-ACP)

Vessel reported a hole in the bottom of the centerline oily water holding tank. The hole has been plugged with a 2" plug. Vessel is to conduct permanent repairs IAW ABS requirements.

The condition of the ship shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Numerous numbers of the overboard swing check scuppers on the RO/RO decks above the bulkhead decks are stuck in place and are not able to swing. Prior to carrying cargo ensure that the overboard swing check scuppers are all operating freely.

A training manual shall be provided in each recreation room or in each crew cabin there is no SOLAS Training Manual in “Supercargo Pers Lounge” 01-124-2 provide a SOLAS Training Manual.

Fire-fighting systems shall be readily available for immediate use. Fire station FM V-52/FM CO V 2-85-2 is blocked by racks of steel. Clear an access to the fire station and make ready for immediate use.

The in-line proportioners (aerator) still have duct tape around the screens on B-Deck as fire-fighting systems shall be readily available for immediate use, remove all the tape.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner, the starboard emergency light by fire station #18 has a damage junction box. Make proper repairs.

The deck light above vent opening by location 1-45-4 has a mounting that is broken make proper repairs so the so installation will not to cause injury when handled or touched in the normal manner.

Make proper notifications to RO reguarding OCM casulty for the machinery OCM.

Replace grab line for life buoys with approved type.

Properly document all required entries on vessels confined space entry forms in accordance with the vessels SMS.

Prove proper operation of anchor windlasses #3 and #4.

Prove proper operation of port CPP

Vessel is operating with reduced propulsion (one siezed shaft) and is required to have the assistance of two tugs while transiting inside the Sector Hampton Roads Regulated Navigation Area (RNA). One tug must be of 3000 horsepower or more; & the other must be of 2000 horsepower or more. Cite: 33 CFR 160.111(c)

The vessels crew was unable to properly operate the OWS. Prove proper operation of OWS to the satisfaction of Coast Guard Marine Inspector.
