A hatch exposed to the weather must be watertight. The gaskets for the scuttles to the forepeak, #2 void, the engine room, and the lazarette are deteriorated and ineffective, the closures cannot be made watertight. 46 CFR 179.360

Fire hoses must be of commercial grade and conform with UL 19 standards; the outer jacket of the port fire hose is deteriorated and must be replaced.

The marine employer must have a drug testing program in place. The captain could not provide proof of contract, the list of employees, or the current MIS report. 46 CFR 16.113

An adult life jacket must be provided for each person carried on board a vessel. Two adult life jackets were condemned during the inspection, the master provided two new jackets at the end of the inspection. 46 CFR 180.071

The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float. The life floats were not so marked. 46 CFR 185.604

A vessel must carry either a first-aid kit that meets the requirements in ISO 18813 or a kit with equivalent contents. The existing kit does not include burn preparations, provide vessel with 12 applications of burn preparations. 46 CFR 184.710

All griddles, broilers, and deep fat fryers must be fitted with a grease extraction hood in compliance with § 181.425. The galley griddle hood is not in compliance with 181.425, does not have fixed fire fighting capability and must meet UL 710. The vessel is no longer allowed to use cooking appliance until an approved grease extraction hood has been installed in accordance with USCG approved plans. IAW 46 CFR 181.405 (b)

A self priming, power driven fire pump must be installed on each vessel Of not than 19.8 meters (65 feet) in length that carries more than 49 passengers; vessel's firepump is leaking water from packing but is still functional.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom; vessel 02 plastic trashcans.

Replace plastic wire hangers in overhead of ER w/ a suitable hanger resistant to fire.

Person in command of a documented vessel must have onboard the original Certificate of Documentation onboard; vessel has valid COD but document is not onboard. 46 CFR 67.313

On a vessel of at least 26 ft in length and visual and audio=ble alarm must be provided at the operating station to indicate a high water level in each of the following unmanned spaces; a machinery space bilge; loose wire renders alarm inoperable

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; # 18 breaker in galley panel has two conductors attached.

On a vessel operating on exposed or partially protected waters, an opening port light is not permitted below the weather deck unless the sill of the port light is at least 760 millimeters (30 inches) above the deepest load waterline; Plug or valve pipe in side shell in lazarette

The engine compartment escape hatch is not operational; repair and prove watertight integrity.

A vessel must have an interconnected fire/smoke detection system in all accommodation spaces. The smoke detectors in the passenger accommodation space are either missing or rendered inoperative.

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle; the existing marking have faded and are no longer legible.

All escape hatches and other emergency exits must be marked "EMRGENCY EXIT, KEEP CLEAR" on both sides; the engine compartment escape hatch is not properly marked.

The construction and arrangements of the vessel must allow for safe operations giving consideration to the seaworthiness of the hull; two main support structures (stanchions) in the #1 void have broken welds.

At each inspection of a vessel the operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems; the spot light is not operational.

The vessel must be provided with a satisfactory arrangement for draining any watertight compartment in accordance with 46 CFR 182.500; the bilge manifold is holed due to corrosion.

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment in accordance with 46 CFR 182.455(b)(6); the installed Racor filters have excessive build-up of water and require cleaning and replacement.

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray in accordance with 46 CFR 183.350; the generator start batteries are not protected against shifting and are sitting in water.

At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition in accordance with 46 CFR 176.818; the toilet drain pipes are holed and leaking into the engine compartment, submit a repair proposal and make repairs to the satisfaction of an attending USCG Marine Inspector.

Each vessel subject to 46 CFR Part 178, Subpart C must, in each condition of loading and operation, comply with the applicable requirements of 46 CFR Part 170; weight changes due to modifications/alterations have occurred over the years affecting the vessels lightship characteristics, including but not limited to the replacement of both main engines, replacement of the generator with prime mover, replacement of the A/C unit on the upper deck, and the modification to the chine in which approximately 535 pounds of aluminum angle was added. Accordingly, the operator must submit calculations to the Marine Safety Center for determination if a dead-weight survey, or a full stability test is required to demonstrate compliance with the pertinent regulations and confirm the validity of the vessel's stability letter.

A fire pump on a TL vessel shall be able to discharge 50 gpm @ 60 psi; vessel's gauge was only puting out 30 psi.

The shorepower breaker must be interlocked w/ the vessel's gen breaker; current interlock bar does not prevent both breakers from being closed at the same time

Circuits in panels must be labeled; Switchboard aand galley breaker panel are not labeled.

Employers must ensure that they and their crewmembers meet the requirements of part 16; vessel does not have required documentation for employees or training; a full company audit is to be conducted w/in 30 days

Any vessel of at least 5 net tons which engages in commercial service must have a Certificate of Documentation; the certificate on board is expired.

As appropriate for the intended voyage a vessel must carry adequate and up-to-date charts and navigation publications; the existing chart is out dated.

A manned oceangoing vessel of 40 feet or more in length engaged in commerce must have a garbage management plan; the person in charge listed on the existing plan is no longer employed on board, the plan is out dated.

In accordance with 47 CFR Subpart S Small Passenger Vessels operating beyond 20 nautical miles but not more than 100 nautical miles from the nearest land must be equipped with a MF-DSC radio; the captain is not able to prove that the single side band radio is operational. The vessel is restricted to less than 20 NM from land.

Gauges to indicate engine lube oil pressure must be provided for all propulsion engines, the gauges must be readily visible at the operating station; the lube oil gauge for the port engine is not operational.

Electrical equipment must be installed and maintained to protect personnel and the vessel from electrical hazards; there is an electrical box under the operating station console which is missing the cover.

Each life float approved under 46 CFR 160.027 must meet the requirements of 160.010 in which the pendants must be fitted approximately 18" apart, around the outside of the float, and must be at least 1/4" diameter and 12' long, secured in the middle; the existing pendants are deteriorated.

A vessel of more than 65’ in length must carry a minimum of three life buoys; the port side life buoy is deteriorated and reaching the end of its service life.

A vessel must carry a first aid kit, approval number 160.041, or a kit with an equivalent contents; the existing kit is not an approved type and does not appear to have the proper inventory.

A hatch exposed to the weather must be watertight; the hatch to the #1 void does not seal properly.

Where flexible non-metallic piping (hose) is permitted in piping systems it must meet the required specifications for its intended purpose and be otherwise acceptable to the OCMI; the hose on the suction side of the mechanical bilge pump IWO the stbd engine is deteriorated.

At each inspection, the owner/ operator shall be prepared to conduct test operations of machinery to include machinery essential to teh routine operation of the vessel including generators; the generator prime mover has a severe jacket water leak at the pump and the pump bearings are worn.

Portable fire extinguishers must be inspected and maintained in accordance with NFPA 10; the annual service has not been conducted and is overdue, additionaly, one extinguisher indicates no pressure on the gauge and must be replaced.

At each inspection for certification of a vessel the managing operator shall be prepared to conduct tests to prove the hull structures and appurtenances are sound, including through hull fittings and valves; the sewage discharge pipe is holed between the through hull penetration and the sea valve allowing water ingress. Temporary repairs are in place to allow the vessel to transit to drydock for permanent repairs to be carried out.

The master of the vessel shall ensure that the EPIRB battery is replaced prior to its expiration date; the existing battery expired in May 2020.

Exhaust pipe shall be gastight throughout its length; the port main engine exhaust pipe is holed and is leaking cooling water into the engine compartment.

Bilges shall be kept free of excessive water so as to avoid an unsafe condition; the #1 and #2 void compartments have excessive water in the bilges.

Electrical equipment must be installed and maintained to protect personnel and the vessel from electrical hazards; the distribution panel in the galley and the engine room have openings which require blank covers.

The name of the vessel must be marked in clearly legible letters on each piece of lifesaving equipment; the marking on the lifebuoys and the life float paddles are faded and not legible.

Each life float paddle must be lashed to the float to which it belongs; the paddles are not attached to the floats.

A vessel must carry adequate and up-to-date navigation publications for the intended route; the vessel does not have the Notice to Mariners on board.

Vessels over 65 feet in length must have an FCC Bridge-to-Bridge Certificate issued upon satisfactory inspection; the vessel does not have a current certificate.

Hatches on the main deck shall be watertight. The lazerette and engine room hatch are not operational.

Vessels over 65 ft must have 3 ring buoys. The grab line on 2 of 3 is deteriorated. Replace with 24" orange CG approved.

All vessels shall have arrangements to pump all watertight spaces. The pipe assembly to the forepeak suction is inoperable. The valve and strainer need to be re-thread.

Vsl shall be provided with appropriate navigation publications. Provide excerpts of updated coast pilot and light list.

Vsl over 65 feet shall have a fire pump capable of 60 PSI at 50 GPM. Current pump is 40 psi with one hose open.

The SSB shall have two sources of power. Move the current configuration to provide a second source of power for 3 hours duration.

Lifejackets shall have approved reflective tape. Ensure all jackets have adequate tape.

Fuel vents shall be fitted with 30 x 30 stainless mesh. Current mesh is not 30 x 30.

Panel boards shall have no openings. The main engine room panel is missing 2 blanks and the galley panel is missing 1.

Vsl over 65 ft shall have contrasting draft marks forward and aft. The forward draft marks are not contrasting.

Electrical installations shall be to the satisfaction of the [name withheld] the following: a)provide ground for battery charger case b)label 12 volt panel in pilot house c)provide appropriate overcurrent protection for main output from converter d)install permenant 120 volt outlets as needed in pilot house

The vessel is currently broadcasting (1) Zero Length and Beam, (2) Zero Draft.

Provide proof of user fee payment.

Repair the pinhole leak in the bilge piping IWO the #1 void.

Provide new reflective tape for three jackets where deteriorated.

Ten life jackets were condemned due to the condition, provide new life jacket. The vessel is restricted to ten fewer persons until corrected.

The heat exchanger for the generator prime mover is wasted and leaking, replace heat exchanger.

Replace two fuel hoses on the port main engine due to deterioration.

STBD engine driven mechanical bilge pump was not operational.

Small crack on port MDE raw water sea suction pipe at through hull weld allowing water to leak into bilge.

Ship service generators pulley guard was loose.

Bilge manifold was missing a check valve for the steering gear room suction pipe.

Port MDE raw water supply pipe to heat exchanger on inboard side was leaking sea water.

Missing port and stbd B-II mounting bracket in main passenger cabin on main deck.

Install plug in F/O tank drain (located in engine room).
