A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter (H). On May 3, 2022, a fire occurred within the generator room resulting in significant damage to the machinery, electrical equipment, wiring, and structural fire protection within the surrounding area. Prior to repairs and the installation of new machinery and electrical equipment plans shall be provided to Marine Safety Center for approval.

The vessel is currently in a layup status for Boiler Repairs (Port Boiler) and Boiler Replace3ment on STBD Boiler along with some upper deck repairs. Prior to getting the vessel underway, complete the replacement of the STBD Boiler and repairs to the PORT Boiler. Complete all required inspection intervals to the satisfaction of the USCG Inspector.

2.Currently all life rafts certificates have expired, vessel in maintenance period so prior to getting the vessel underway provide current life raft servicing reports.

3. Current fire protection report from [name withheld] has deficiencies that require correction, Galley equipment will not shut down upon activation of the fire protection. Correct prior to operating the galley.

4.No drills were witnessed today due to vessel in maintenance period, prior to carrying passengers, drills must be witnessed by the USCG.

An abandon-ship drill and a fire drill, as described in 199.180, must be conducted on each passenger vessel at least weekly. Drills could not be conducted due to lack of crew on board resulting from local Covid-19 restrictions. Conduct drills IAW 46 CFR 199.180, and provide SIP documentation/associated logs to satisfaction of CG Marine Inspector prior to movement. 46 CFR 199.250(a) 46 CFR 199.180 46 CFR 78.17-50

If a survival craft is recovered by electric power, the electrical installation, including the electric power-operated boat winch, must meet the requirements in subchapter J of this chapter. If a survival craft is recovered by any means using power, including a portable power source, safety devices must be provided that automatically cut off the power before the davit arms or falls reach the stops in order to avoid overstressing the falls or davits, unless the motor is designed to prevent such overstressing. Provide proof of manufacturer-installed winch safety device, or install safety device & test to satisfaction of CG Marine Inspector prior to movement. 46 CFR 199.153(k)

Each item of survival equipment with an expiration date must be replaced during the annual inspection if the expiration date has passed. Rescue boat wire bridles have no markings or documentation to prove maximum static load and servicing. Correct to satisfaction of CG Marine Inspector prior to movement. 46 CFR 199.190(f)(3)

Items of equipment, which are in use on vessels, previously approved by the Commandant but not complying with the latest specification requirements may be continued in use so long as they are maintained in good and serviceable condition to the satisfaction of the Officer in Charge, Marine Inspection, until replacement is deemed necessary by such officer or as specified in the regulations. Hot water heater installation in crew galley/dishwashing service area has significant corrosion to electronics housing and vessel bottom. Replace hot water heater with new/serviceable unit or electrically isolate and tag-out system to satisfaction of CG Marine Inspector within 14 days.

Doors, other than those which are normally locked shall be of the self closing type against a 3 and 1/2 degree list. Upon arriving at the vessel there were issues with the port and starboard doors admidships on the 2nd deck and with the starboard foward door on the 2nd deck. These were rectified while inspectors were onboard. There is remains an issue with the portside door leading to the engine room. This issue needs to be corrected by the end of the next maintenance period.

Ships position shall be continuously and automatically provided to all relevant radio communication equipment. The ship's position and MMSI number for the vessel were not currently displaying on either VHF reciever. (46 CFR 80.1083(f))

The vessel is not in compliance fully with 46CFR Part 16.230 (a)Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who: under 16.230 (a)(3) which requires random drug testing for personnel who are "specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies." The definition of Crew Member is under 46CFR 16.105 under crew member (#2). Provide a proposal to make the vessel in compliance with these requirements. The proposal will need to be to our office by Friday 31 August 2018. Once proposal has been received and accepted the above will need to be complied with within 14 days.

Provide fire control plan for vessel to confirm boundary on portside of galley with opening in bulkhead.

Clearly mark CO2 pull station for paint locker aft of galley

Repair or replace reeefer aft of the galley. Remove all stowed equipment and goods on top of galley equipment.

Relabel distribution board panel in galley with new equipment installed (ie. Reefer and hot box)

Newly installed deck covering in galley does not have appropriate documentation showing it meets the requirements of NVIC 9-97 (CH-1) 2.2.1. Provide sufficient documnetation to attending MI.

Several Lifejackets found with insufficient Retroreflective material. Correct to satisfaction of attending MI.

Life Jacket stowage lockers rigged in such a way as to impede the release of Lifejackets and a portion of the cable broke during the course of our test. x2 stowage lockers still need to be re-rigged, required vessel to complete modifications to include new cables prior to 12Mar18.

During response to small fire in storage locker. ID'd damage to electrical wiring (2 cables along beam) w/in space. Completely replace wiring w/ equivalent marine grade wiring to the damaged nonvital lighting system after properly locking/tagging system according to ship's safety procedures. Fix prior to use.

Pump out bilges free of excess oily/water prior to sailing.
