Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II- I/42 or II-1/43. PSCO observed non-operational emergency lighting forward of lifeboat 16, aft of lifeboat 20 and in midship area of vessel. 74 SOLAS 97 III/11.4

Hinged fire doors in stairway enclosures, main vertical zone bulkheads and galley boundaries which are normally kept open shall be self-closing and be capable of release from a central control station and from a position at the door. PSCO observed FSD 1403, 429, and 350 incapable of self closure. 74 SOLAS 01 II-2/41-2.4.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed lifeboat 20 with engine insufficiently secured to engine mounts and fuel oil in the bilge. 74 SOLAS 20 III/20.2

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed lifeboats 16 (stbd engine), 17 (port engine), and 19 (both engines) fail to start. Additionally, lifeboat 17 (engine) could not be turned off. 74 SOLAS 20 III/20.2

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed emergency water locker seized shut on lifeboat 9. 74 SOLAS 20 III/20.2

For determining the proper fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk, as shown in categories (1) to (14) below. PSCO observed storage of combustible materials in category (10) space in fan room 1104. 74 SOLAS 97 II-2/26.2..2

Each section of sprinklers shall be capable of being isolated by one stop valve only. The stop valve in each section shall be readily accessible and its location shall be clearly and permanently indicated. Means shall be provided to prevent the operation of the stop valves by any unauthorized person. PSCO observed absence of tamper alarm on section valve 48 on deck 10. 74 SOLAS 01 II-2/12.2.2

The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. If the signals have not received attention within 2 min an audible alarm shall be automatically sounded throughout the crew accommodation and service spaces, control stations and machinery spaces of category A. PSCO observed manual call point 5164 incapable of activation. 74 SOLAS 01 II-2/13.1.4

Ships of 500 gross tonnage and upwards shall be provided with at least one international shore connection complying with the provisions of paragraph 3. PSCO observed excessive storage of linens and other combustible materials blocking access to the international shore connection valve, just aft of the forward mooring station. 74 SOLAS 97 II-2/19.1

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins.PSCO observed stairway guide deck 14 forward stairwell unfamiliar with duties. 74 SOLAS 20 III/19.2.1

The speed of door closure shall, if necessary, be controlled so as to prevent undue danger to persons. PSCO observed inoperable action bar on FSD 356, 913, 912, and 915. 74 SOLAS 81 Amend II-2/30.4

Every fuel pipe from a storage, settling or daily tank shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside of the space. PSCO observed HFO valves 4, 7, 8 did not close when actuated. 74 SOLAS (81) II-2 / 15.2.5 17ac

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PSCO observed crewmembers on lifeboats #10, #15, and #21 were unfamiliar with the location of the emergency fuel shutoff valves. Additionally, crewmembers in lifeboat #20 struggled with shifting engine in and out of gear. 74 SOLAS (20) III/19.2.1 17ac

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed the deck in pool machinery room on deck 9 had an excessive amount of water on it, creating shock hazard with the electrical panels in the same space. 74 SOLAS (04) II-1 / 40.1.3 10c

All waste-receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed combustible materials inside plastic waste receptacle. 74 SOLAS 97 II-2/18.5 17C

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed light cover missing in way of call point L50-A43. 74 SOLAS 04 II-1/40.1.3 17C

Electrical installations shall be such that: Electrical services essential for safety will be ensured under various emergency conditions. PSCO observed extinguished light in medical disaster cart room. 74 SOLAS 04 II-1/40.1.2 17C

In passenger ships, doors other than power-operated watertight doors, shall be so arranged that positive closure is assured in case of fire in the space. PSCO observed broken hinge on FSD 001, preventing door from self-closing. Additionally, PSCO observed FSD 856 failed to latch. 74 SOLAS 81 Amend II-2/11.2.3 17C

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. The stowage of combustibles is prohibited in a category 10 space. PSCO observed stowage of wood tiles inside the Fan Room #504 (Category 10 Space). Additionally, PSCO observed the stowage of combustible materials inside Fan Room #804 (Category 10 Space). 74 SOLAS 81 Amend II-2/26.2.2 17C

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed wire rope on crane at liferaft station number 7 was improperly spooled. 74 SOLAS 20 III/20.2 17C

When double swing doors are permitted, they shall have a latch arrangement which is automatically engaged by the operation of the door release system. PSCO observed latching arrangement on FSD 359 failed to latch door properly. 74 SOLAS 81 Amend II-2/30.4 17C

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PSCO observed stairway guides assigned to numbers 8 and 9 aft stairways and number 4 forward stairway were unfamiliar with required knowledge. Additionally, PSCO observed crew members assigned to lifeboat number 12 were unfamiliar with how to operate emergency steering and general knowledge relating to flares, TPAs, and how much water each passenger shall receive. PSCO observed crew assigned to lifeboat number 14 were unfamiliar with how to shut off engine and crew assigned to lifeboat 21 were unfamiliar with how to operate the fuel shut-off. 74 SOLAS 20 III/19.2.1 17C

Fire-extinguishing appliances shall be kept in good working order and be available for immediate use at all times. PSCO observed a pinhole leak in firehose at station number 86. 74 SOLAS 97 II-2/21 17C

The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.l to 26.4. PSCO observed combustible materials stored in a category 10 space in fan room 1004 and 902. 74 SOLAS 81 Amend II-2/26.1 17c

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed that engine mount in lifeboat 4 was loose, missing bolts, and caused engine to vibrate dangerously when operated. 74 SOLAS 20 III/20.2 17c

In passenger ships, doors other than power-operated watertight doors, shall be so arranged that positive closure is assured ;n case of fire in the space, by power-operated closing arrangements or by the provision of self-closing doors capable of closing against an inclination of 3.5 opposing closure and having a fail-safe hook-back facility, provided with a remotely operated release device. PSCO observed fire screen doors 305, 803, 838, 837, 831, 815, 801, and 1106 failed to close and latch properly. 74 SOLAS 81 Amend II-2/11.2.3 17c

Hinged fire doors in stairway enclosures, main vertical zone bulkheads and galley boundaries which are normally kept open shall be self-closing and be capable of release from a central control station and from a position at the door. PSCO observed double leaf doors leading from fwd stairway to the Normandie Main Lounge only closed on one side. Additionally, hinged door 508 in fwd stairway failed to close. 74 SOLAS 01 II-2/41-2.4.1 17c

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed a hydraulic oil leak in lift machinery room on deck 11 stbd side. Until deficiency is rectified, equipment must be locked out/tagged out IAW vessel SMS. 74 SOLAS I/11 16ac

Every crewmember shall be given instructions which shall include but not necessarily be limited to operation and use of the ship's inflatable rafts, problems of hypothermia, first-aid treatment for hypothermia and other appropriate first-aid procedures, special instructions necessary for use of the ship's life-saving appliances in severe weather and severe sea conditions, operation and use of fire extinguishing appliances, and risks associated with enclosed spaces and on-board procedures for safe entry. PSCO observed a lack of knowledge of locations of equipment on lifeboat as well as survival procedures. 74 SOLAS 20 III/19.4.2 16ac

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO observed bent sprinkler head in pantry next to Blue Iguana restaurant. 74 SOLAS 20 II-2/14.1.1

The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.l to 26.4. PSCO observed missing sections of A-60 insulation in the stbd ceiling of the evaporator room in a category 11 space, which is below the engine control room, a category 1 space. 74 SOLAS 97 II-2/26.l 16ac

Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. PSCO observed the straps securing several liferafts on both port and stbd side were badly deteriorated. 74 SOLAS 20 III/20.3 50ac

Accommodation and service spaces, stairway enclosures and corridors shall be fitted with an automatic sprinkler system complying with the requirements of reg 12. PSCO observed items stored higher than sprinkler head in storage locker in crew lounge near RSD 7.4.9. 74 SOLAS 01 II-2/41-2 10c

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. PSCO observed the stbd ventilation intakes on deck 7 fwd were seized and nonfunctional. 74 SOLAS 81 Amend II-2/16.9 10c

74 SOLAS II-2/18.5 10C All waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed plastic waste receptacles in the lido galley

74 SOLAS 81 Amend II-2/29.2 10c. Stairway enclosures shall have direct communication with the corridors and be of sufficient area to prevent congestion, having in view the number of persons likely to use them in an emergency. In so far as is practicable, stairway enclosures shall not give direct access to cabins, service lockers, or other enclosed spaces containing combustibles in which a fire is likely to originate. PSCO observed drums of cleaning chemicals stored underneath stairway 290.

74 SOLAS 81 Amend II-2/30.4 10c. Fire doors in main vertical zone bulkheads and stairway enclosures, other than power-operated watertight doors and those which are normally locked, shall be of the self-closing type. The speed of the door closure shall, if necessary, be controlled so as to prevent undue danger to persons. PSCO observed the speed of the doors 816, 817, 819, 839 was reasonable, however, they did not reopen on contact with an object and would therefore pose a danger to persons.

74 SOLAS 97 III/7.1.2 10c. At least one lifebuoy on each side of the ship shall be fitted with a buoyant lifeline complying with the requirement of paragraph 2.1.4 of the Part B: Code equal in length to not less than twice the height at which it is stowed above the waterline in the lightest seagoing condition, or 30 m, whichever is greater. PSCO observed that lifeline on starboard side lifebuoy was of insufficient length.

74 SOLAS 04 II-1/20.2 10c. The bulkhead deck or a deck above it shall be weather right. All openings in the exposed weather deck shall have coaming of ample height and strength and shall be provided with efficient means for expeditiously closing them weather tight. PSCO found weather tight door 183 to be inoperable. Additionally, the weather tight door n the vicinity of lifeboat 21 was found to be heavily corroded on the bottom frame and no longer weather tight.

74 SOLAS 20 III/34 LSA 1.2.2.7 10c All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. All life-saving appliances shall be fitted with retro-reflective material where it will assist in detection. PSCO observed the retro-reflective tape on lifeboat 19 needs to be renewed, and the retro on top of lifeboat 21 was missing entirely.

74 SOLAS 81 Amend II-2/26. 10c. The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.1 t0 26.4. PSCO observed rags, towels, and linen storage in the fire main shore connection locker, door 6-690, as well as shore connection locker adjacent to passenger room U-2.

07126 - Fire Safety - Every oil fuel pipe, which if damaged would allow oil to escape from a daily service tank situated above the double bottom, shall be fitted with a valve which is capable of being closed from a safe position from outside the space. PSCO observed two out of the three valves in section #5 did not close and three out of three valves in section #6 did not close. SOLAS 74/78 (97 Cons). Ch. II-2/15.5 17c

07105 - The construction of all doors and door frames in "A" class divisions, with the means of securing them when close, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent material. PSCO observed the top of the frame on fire screen door 0.18, main vertical zone 7, was damaged allowing the passage of air when the door was in the closed position. SOLAS 74/78 (97 Cons.) Ch. II-2/30.2 17c

07105 - Fire Safety - Fire doors in main vertical zone bulkheads and stairway enclosures, shall be self-closing. PSCO observed fire screen door 8.10, main vertical zone 7 did not completely close when activated. SOLAS 74/78 (97 Cons.) Ch.II-2/30.4 17c

11101 - Life Saving Appliances - The condition of the ship and it's equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed unserviceable fuel caps in lifeboats 4, 6, 10, and 12 resulting in the leaking of diesel fuel into the lifeboat from the fuel tanks. SOLAS 74/78 (2020 Cons.) Ch.I/11 17c Vessel was able to make temporary repairs in order to proceed underway, however, PSCO changed the deficiency to a 40a/c.

09209 - Working and Living Conditions - All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in a normal manner. PSCO observed exposed dead-end wiring from converter boxes F17 and F2. SOLAS 74/78 (2004 Cons.) II-1/45.1.3 40 a/c

11131 - Every crew member with assigned emergency duties shall be familiar with these duties before the voyage. PSCO observed lack of knowledge and familiarity with the launching of life rafts in accordance with the ship's procedures. 74 SOLAS (20) III/19.2 17 AC

11129 - The function of the detection system shall be periodically tested to the satisfaction of the administration by means of equipment producing hot air at appropriate temperatures or smoke or aerosol particle having the appropriate range of density or particle size. PSCO observed crew using unapproved heat gun to test heat detectors throughout the ship. 74 SOLAS (97) II-2 / 13.1.13 50 AC

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed missing light for lifejacket in locker across from stateroom 463. Several life jacket lights failed to activate in the locker starboard side on deck 9. 74 SOLAS (20) III/20.2 50BC

08199 - The number of joints in such piping systems shall be kept to a minimum and where practicable, leakages from high pressure oil fuel pipes shall be collected and arrangements provided for an alarm to be given. PSCO observed fuel leak alarms on high pressure oil fuel lines for number one, two, three, and five SSDG were masked/silenced. 74 SOLAS (97) II-2 / 15.5.1 10C

Space is categorized as 13. Additionally, paint stores and other materials are improperly stored in CAT 2 stairwell outside Boatswain's workroom. SOLAS 81 Amend II-2/26.2.2

Stairway enclosures shall have direct communication with the corridors and be of sufficient area to prevent congestion. Deck 3 (I95 corridor) contained significant storage causing congestion. SOLAS 81 Amend II-2/29.2

THE MACHINERY SYSTEMS AND FITTINGS SHALL BE OF A DESIGN AND CONSTRUCTION ADEQUATE FOR THE SERVICE FOR WHICH THEY ARE ENTENDED. PORT AZIPOD REDUCED TO 50% POWER/CAPACITY FOR PROPULSION. 74SOLAS(2004)II-1.26.1 / 16AC

Fire doors in main vertical zone bulkheads shall be self-closing. Fire screen door1206 did not close - 74SOLAS(97) II-2/30.4.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order. Lifeboat #21 was unable to clutch ahead and astern during drills - 74SOLAS(14) III/20.2

The transitional source of emergency electrical power required by paragraph 3.1.3 shall consist of an accumulator battery suitably located for use in an emergency which shall operate without recharging while maintaining the voltage of the battery throughout the discharge period within 12% above its normal voltage and be of sufficient capacity and so arranged as to supply automatically in the event

Whenever an accident occurs to a ship or a defect is discovered which affects the completeness of its...or life-saving appliances or other equipment, the master or owner shall immediately report it to the appropriate authorities of the port State. The vessel failed to notify the port States that LB No. 3 is 1157.6 kg overweight from original weight; therefore needing to reduce the passenger count.

All lifeboats shall have inherent buoyancy or shall be fitted with inherently buoyant material which shall not be adversely affected by seawater, oil or oil products, sufficient to float the lifeboat with its equipment onboard. LB #3 is 1157.6 kg overweight from its original weight; causing a need for a reduction in passenger count to 134 persons onboard LB #3.

DOOR FRAMES IN 'A'CLASS DIVISIONS, WITH THE MEANS OF SECURING THEM WHEN CLOSED, SHALL PROVIDE RESISTANCE TO FIRE AS WELL AS TO THE PASSAGE OF SMOKE. THE DOOR FRAME LEADING TO THE INCINERATOR ROOM/GARBAGE ROOM WAS TACK WELDED WITH GAPS THAT WOULD ALLOW PASSAGE OF SMOKE. SOLAS 74 (2014 CONS.) II-2/REG 9.4.1.1.2. 16AC.

PASSENGER SHIPS CARRYING MORE THAN 36 PASSENGERS SHALL BE EQUIPPED WITH AUTOMATIC FIRE DETECTION SYSTEM COMPLYING WITH THE REQUIREMENTS OF THE FSS CODE. F&B LOCKER ON DECK 7 HAD COMBUSTIBLE MATERIALS STACKED ABOVE THE SPRINKLER HEADS. SOLAS 74 (2014 CONS.) II-2/ REG 10.6.1.1. 10C.

Each circuit shall be protected against short circuit and overload. During air conditioner compressor No.2 shut-down, the power supplied circuit breaker failed to trip offline. Continued operation in shut-down condition caused a fire at the compressor.

Vessel's grey water drain piping located in stbd steering gear room has two (2) soft patches. SOLAS 74 Amend Reg 11
