Crew members did not know how to fight a fire on the vessel. Provide adequate training to crew members so they are able to demonstrate ability to fight a fire aboard the vessel.

Various Bilge sensors were inoperable, re-configure or replace faulty sensors, familiarize crew with test procedures and conduct satisfactory test of all installed bilge alarms, and to the satisfaction of the attending Marine Inspector.

If twist-on types of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Various uncovered junctions utilizing twist connectors found in the machinery space including the steering fluid low level alarm sensor. Install appropriate covers on all junction boxes as required.
