The OCMI has reasonable cause to believe that the vessel's internal survey program is deficient based on the totality and severity of deficiencies discovered during the COI inspection. The vessel is hereby directed to conduct an external vessel audit, by the TPO (TVIB), in the presence of a representative of the OCMI.

Major lube oil leak discovered from port main engine lube oil filter; excess oil accumulation in the engine room bilge. Repair lube oil leak and remove excess oil throughout engine room bilge.

The following additional deficiencies were discovered during the COI inspection: •Floating electric water light contained the incorrect battery model type; •Port searchlight found to be inoperable; •MMSI number missing from installed VHF radio database; •Potable water pump discharge hose torn & leaking water; •Port generator switchboard power available light extinguished; •Alarm set-point testing had not been completed for main engines, generators & hydraulic steering fluid levels •No record of pressure relief valve being tested; •Pilothouse access ladder grab rail sheered from base. Correct the deficiencies listed above to regain compliance under subchapter M.
