The location to mount the block-and-tackle pulley for hoisting is to low and requires the crewman to lean approximately 4ft over the side nearest the person in the water to tie a knot in the hosting line to attach the pulley. This process repeats itself multiple times before the person is hoisted high enough to grab and drag on board. A requirement has been issued to the further evaluate the recovery procedures and provide an assessment of how the recovery of a helpless person can be effectively completed using the vessel itself.
