Due to the objective evidence in the below deficiencies, the vessel is not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM code. PSCO’s collected objective evidence that the vessel is not following SMS procedures 9.3. PSCO observed an inoperable fire pump at the port aft due to a broken impellor as well as a soft patch on the fire main that was not reported. Vessel could not produce evidence that the vessel or company notified their Recognized Organization or Flag Administration of the serious safety deficiencies. An external audit is required.
