All diesel fuel tanks exceeding 30 inches in any horizontal dimension shall be fitted with vertical baffle plates where necessary for control of excessive surge. Inspector found a diesel soaked absorbent pad secured around the exhaust pipe of the diesel tank located on the upper deck. This indicates that the tank is susceptible to excessive surge/slosh of diesel fuel.

Compressed oxygen with a hazard class of 2.1 and subsidiary hazard of 5.1 must be stowed "Separated From" acetylene with a hazard class of 2.1. For on deck stowage, this segregation means a separation by a distance of at least 6 m (20 feet) horizontally. MI found 04 oxygen and 02 acetylene compressed cylinders strapped in a bundle next to railing on 2nd deck. Separate oxygen and acetylene tanks by a distance of at least 20ft.

Each cable installation must meet—(1) Sections 25, except 25.11, of IEEE 45-2002. Metallic band strapping used for cable support should be steel and corrosion treated if not a corrosion-resistant material. The support for all cables should be such as to prevent undue sag, but in no case should exceed the distance between frames or 24 inches, whichever is less. Metallic band strapping should be applied so that the cables remain tight without damage to the cable. Metal supports should be designed to secure cables without damage to the armor or the insulation. The supports should be arranged that the cable would be supported for a length of at least 0.5 inch. Inspected identified that plastic zip ties have been utilized to secure electrical wires to the upper bulkhead within the tool room.
