Railing to be replaced in various areas around the vessel. Vessels deck will be cropped and renewed in aft port quarter. All work is scheduled to be completed in LA/LB.

Approved hand portable fire extinguishers and semi portable fire extinguishing systems shall be installed on all vessels. Vessel to replace out of service fire extinguisher in the winch room.

Each lifejacket must be marked in block capital letters with the name of the vessel. Stencil the proper name (SALTA VERDE) on the 04 lifejackets identified.

All diesel fuel tanks exceeding 30 inches in any horizontal dimension shall be fitted with vertical baffle plates where necessary for control of excessive surge. Inspector found a diesel soaked absorbent pad secured around the exhaust pipe of the diesel tank located on the upper deck. This indicates that the tank is susceptible to excessive surge/slosh of diesel fuel.

Each vessel must carry lifebuoys approved under approval series 160.150. Self-igniting light attached to lifebuoy on main deck was not operational. Repair or replace self-igniting light battery or apparatus and demonstrate proper operation.

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. Properly lockout 100A breaker on 480v panel in workshop to prevent person from inadvertently activating breaker that supplies power to disconnected electrical cable.

Properly terminate 480v electrical cable that has been temporarily disconnected from electrical apparatus. Electrical cable had exposed ungrounded conductors and others wrapped with electrical tape.

Compressed oxygen with a hazard class of 2.1 and subsidiary hazard of 5.1 must be stowed "Separated From" acetylene with a hazard class of 2.1. For on deck stowage, this segregation means a separation by a distance of at least 6 m (20 feet) horizontally. MI found 04 oxygen and 02 acetylene compressed cylinders strapped in a bundle next to railing on 2nd deck. Separate oxygen and acetylene tanks by a distance of at least 20ft.

Repair damaged railing on starboard stern quarter of the main deck.

Engine design; all installations shall be of marine type engines suitable for the intended service. Inspector found leaking/worn flywheel seals on both generator prime movers.

Operational readiness; before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The grab lines on all lifebuoys were found to be deteriorated and weakened.

Each cable installation must meet—(1) Sections 25, except 25.11, of IEEE 45-2002. Metallic band strapping used for cable support should be steel and corrosion treated if not a corrosion-resistant material. The support for all cables should be such as to prevent undue sag, but in no case should exceed the distance between frames or 24 inches, whichever is less. Metallic band strapping should be applied so that the cables remain tight without damage to the cable. Metal supports should be designed to secure cables without damage to the armor or the insulation. The supports should be arranged that the cable would be supported for a length of at least 0.5 inch. Inspected identified that plastic zip ties have been utilized to secure electrical wires to the upper bulkhead within the tool room.

In lieu of direct stamping on the pressure vessel, the information required by paragraph (a) of this section shall be stamped on a nameplate permanently attached to the pressure vessel. Inspector found that the data plate on the pressure vessel (air receiver) within the 4600S3 crawler crane has been painted over making required information such as the MAWP illegible.

Insure that the vessel machinery is in satisfactory condition and fit for the service for which it is intended. Inspector identified several hydraulic oil leaks from fittings/connections within the crawler crane machinery space.

Each marine sanitation device (MSD) must be free of design defects that would allow toxic substances to escape to the interior of the vessel. Inspector found that the closure lid on the MSD holding tank was open and venting directly into the space.

Each flexible cord and cable must be without splices or taps except for a cord or cable No. 12 AWG or larger spliced for repairs in accordance with 111.60-19 (IEEE 45-2002 Section 25.11). Inspector found that an extension cord has been cut and spliced to a piece of equipment that is submerged within the marine sanitation device. The splice was intended to be a permanent installation and did not meet the intent or design of a repair as set forth in IEEE 45-2002.

Each circuit breaker must have an interrupting rating sufficient to interrupt the maximum asymmetrical short-circuit current available at that the point of application. Inspector found a circuit breaker in the winch room that was held back (i.e. restricted from tripping) by electrical tape.

During preparation and launching, the survival craft, its launching appliance and the area of water into which it is to be launched are illuminated by lighting supplied from the vessel's emergency source of electrical power. Vessel embarkation station is not illuminated by emergency lighting. Provide emergency light for the survival craft embarkation station.

After loading & prior to departure & at all other times necessary to assure the safety of the vessel, the master or person in charge shall determine that the vessel complies w/all applicable stability requirements in the vessels trim & stability book, stability letter, COI, LL certificate, as the case maybe. The vessel may not depart until it is in compliance w/ these requirements. Vessels lifting operations are permitted for the manitowac 4000w crawler crane w/140' boom as per LL & stability letter. Vessel is not permitted/allowed to operate w/the manitowac 4600s3 crawler crane onboard until new stability calculations have been approved.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. The barge changed from two decks to three adding another level of accommodation spaces. Provide documentation of plan approval or submit plans for approval for the modification conducted on the barge. Industrial personnel are prohibited from entering accommodations until proper plan approval is complete.

Immersion suits were missing lights.

Conduct low oil pressure alarm and shutdown on generators.

Make repairs to barge upon internal inspection of bow rake and #1 stbd spud well as indicated by attending marine inspector.

Complete successful annual exam.

Replace one missing adult Type I PFD.

Remove partially discharged B-II extinguisher from vessel. No replacement required as this is excess equipment.

Repair and prove proper operation of cargo tank valve position indicator devices.

Properly dead end numerous unused wires in cargo pump engine room.

Install lagging for generator exhaust manifold.

Prove proper operation of the emergency shutdown for the starboard cargo pump. Barge is restricted from using starboard cargo pump for cargo operations.

Provide one B-V semi-portable extinguisher in the cargo area IAW 46 CFR Table 34.10(a). This requirement must be completed prior to transiting with persons onboard.

Provide one six person life float IAW Title 46 Subchapter W and local policy.

Repair oil leak on lube oil gauge line on starboard cargo pump prime mover.

Repair fuel oil leak at fuel hose on port cargo pump prime mover as indicated by attending marine inspector.

Properly support wire connected to water temp transducer on port cargo pump prime mover where wire is laying on throttle linkage.

Add retroreflective tape to two liferings.

Add class cover to starboard prime mover pressure gauge.

Replace welding cables at battery terminals with approved marine grade cable.

Submit plans for battery cable installation prior to replacing cable.

Remove tire fender obstructing plimsel mark in way of sbd side of barge.

Re-fasten electrical outlet on bulkhead in way of accommodation space.

PROVIDE OVERSPEED, LOW LUBE OIL, AND HIGH JACKET WATER ALARM TEST AND SHUT DOWN TEST.

PROVE SATISFACTORY OPERATION OF ALL ALARMS, CONTROLS, AND SHUTDOWNS ON CARGO PUMPS AND GENERATOR PRIME MOVERS.

TEST THE CARGO PUMPS RELIEF VALVES AND CARGO PIPING AS PER 33 CFR 156.170 DURING NEXT CARGO OPERATIONS AND IN NO CASE LATER THAN 31DEC87 AND PROVIDE EVIDENCE OF THE SAME TO THE OCMI, MSO PUGET SOUND.

MAKE PERMANENT REPAIRS TO THE APPROXIMATELY 5' X 6' AREA INSET IN THE FWD RAKE, PORT SIDE, FRAMES 1 TO 1 1/2, BETWEEN THE TURN OF THE BILGE AND THE 2ND SIDELONG UP.

VEE AND RE-WELD SIDELONG #4 TO FRAME #18 IN #4 STBD CARGO TANK.

TEST CARGO PUMP RELIEF VALVES, AND CARGO PIPING IAW 33 CFR 156.170 AND PROVIDE EVIDENCE OF THE SAME.

MAKE REPAIR TO #2 SIDELONG IN WAY OF FRAME #5 IN #1 STBD CARGO TANK.
