A vessel of more than 19.8 meters (65 feet) in length must carry at least one rescue boat unless the cognizant OCMI determines that: The vessel is sufficiently maneuverable, arranged, and equipped to allow the crew to recover a helpless person from the water. Vessel's rescue platform when deployed over the side was angled inward which minimizes effective recovery area for crew member trying to recover a person from the water. Vessel to install vertical stanchions to the side of the hull to improve rescue platform angle when deployed over the side within 30 days. 46 CFR 180.210(a)(1)

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date Coast Pilots and Light Lists. Vessel had out of date Coast Pilot and Light List on board. Vessel to obtain current Coast Pilot & Light List within 30 days. 46 CFR 185.20-35(c)(d)

Standby vessels must have, at least, the following equipment: MI emailed vessel rep with all required Standby Vessel Endorsement equipment. Vessel rep to provide satisfactory objective evidence of required equipment or request endorsement be removed from COI. 33 CFR 143.405(a)

Wiring joints and splices shall be mechanically secure and made in junction boxes. Marine inspector shall examine spaces to see no accumulation of oil or other matter which might create a fire hazard. Several dead end wires as well as hydraulic oil was observed in operating station. 46 CFR 183.05-50(b) 46 CFR 176.25-50 OLD T

At each initial and subsequent inspection of certification, examination of condition of exterior of hull to take place at the satisfaction of the marine inspector. A large crack, allowing ingress of water, was observed on the stern rub rail. 46 CFR 176.25-5(a) OLD T

Suitable covers, guards or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Guard on Port Generator was missing. 46 CFR 177.35-15 OLD T

At each initial and subsequent inspection for certification and reinspection the marine inspector shall examine and test the main and auxiliary steering apparatus to the extent necessary to determine that its condition is satisfactory and that it is fit for the service intended. Vessels on deck hydraulic steering ram was missing securing devices on each of the rams securing pins, preventing the accidental backing out of the pins. Vessel shall replace secure devices prior to departure. 46 CFR 176.25-35

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. During inspection, life float rack bars installed to prevent shifting were installed too tight, preventing the ability for the life floats to float free in an emergency situation. Vessel shall loosen rack bars to allow for proper installation prior to departure. 46 CFR 176.25-50(a)

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. During inspection, vessel ventilation above stove was observed missing grease screen and had excessive accumulation of grease buildup around vent area/fan. Note that this was a deficiency identified during last years annual inspection conducted by MSU Morgan City. Vessel shall clean and replace ventilation screen prior to departure. 46 CFR 176.25-50(b)

All escape hatches and other emergenct exits shall be marked on both sides using at least 1-inch letters: Emergency Exit, Keep Clear unless such markings are deemed unnecessary by the [name withheld] hatch leading from the engine room onto the main deck was not permanently marked on both sides. Vessel shall permanently mark Emergency Exit, Keep Clear in 1-inch letters on both sides of the escape hatch. 46 CFR 185.30-15(a)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts. The vessels chart for area of operation was printed on October 10, 2020, updated up to week 40/20 and did not contain any chart corrections, nor were chart corrections being performed onboard. Vessel shall ensure charts are kept corrected/reviewed for corrections upon LNM release, for intended area(s) of operation. Vessel company shall ensure any updates are performed prior to departure and throughout the validity of the COI. 46 CFR 184.420

Each Class 1 door on a vessel in ocean service must be designed to withstand a head of water equivalent to the depth from the sill of the door to the margin line but in no case less than 10 ft. (3.05 meters). The material condition of the knife edge near the bottom was wasted away and small holes were visible located on the main deck cabin watertight door knife edge. Vessel made temporary repair using epoxy to seal the knife edge. Vessel shall make permanent repairs to the main deck cabin watertight door knife edge to ensure watertight integrity within 14 days. 46 CFR 170.270(b)

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Dryer exhaust hose was torn, allowing accumulation of lint into the space and in vicinity of the engine exhaust. Vessel shall repair dryer exhaust hose prior to departure. 46 CFR 176.25-50(b)

Standby vessels must have, at least, the following equipment: A fire monitor with a minimum flow rate of, at least, 500 gallons per minute. The material condition of the fire monitor pump while in operation was unsatisfactory as it leaks excessive water in the bilge while in operation. The vessel does not have a storage tank for waste oil, thus causing a potential pollution hazard to remove accumulated oil and water from the bilge. Vessel shall make permanent repairs to fire monitor pump so as to not leak excessively into the bilge. Vessel is not authorized to act in a standby vessel until permanent repairs are made. 33 CFR 143.405(a)(12)

Standby vessels must have, at least, the following equipment: A fire monitor with a minimum flow rate of, at least, 500 gallons per minute. The material condition of the fire monitor piping located in the laundry room is corroded in a small section of the pipe just before it exits the space. Vessel shall make permanent repairs to fire monitor piping located in the laundry room just before exiting the space. Vessel not authorized to act in a standby vessel service until permanent repairs are made. 33 CFR 143.405(a)(12)

Hatches exposed to the weather must be watertight. Main deck hatch would not properly close and seal. Vessel shall make main deck hatch watertight and seal properly. 46 CFR 171.124(a)

All electrical equipment shall be so constructed so uninsulated parts cannot be inadvertently touched. Distribution panel in the machinery space had large opening with direct access to uninsulated parts. 46 CFR 183.10-10(a)

Bilges and other spaces must not have accumulation of oil or other matter which might create a fire hazard. Bilges were found to have significant oily water accumulation. 46 CFR 176.25-50(b)

Where liquid level indicating gages are installed on diesel fuel tanks, they shall be of heat resistant materials. Fuel tank sight glass is made of warped plastic. 46 CFR 182.20-25(5)

Vessel shall be fitted with ground tackle. Vessel's onboard anchors were seized and not serviceable with inaccessible anchor line. 46 CFR 184.10-1(a)

A power driven vessels shall exhibit sidelights. Port side light bulb was extinguished. 33 CFR 83.23

A person may not employ or engage an individual in a position required to hold a MMC unless that individual maintains a current medical certificate. The Master's medical certificate was expired. 46 CFR 15.401(c)

Vessels draft exceeds the limitations of the stability letter.

Cable penetrations on the engine room overhead were missing packing to prevent fire, smoke or flooding.

Back up power to navigation equipment was improperly wired to the charger not the battery. Vessels navigation equipment and radios did not have back up power.

AFT engine room bilge suction pipe from manifold was missing.

Mount for the fire monitor fire pump is deteriorating. Additionally, the horizontal stiffener has experienced significant corrosion. To be replaced at the next dry dock.

Repair excess leaks on fire pump and replace pressure gauge.

Renew both air receiver relief valves which are painted shut.

Remove old vsl name from all PFD's and life rings.

Obtain foam nozzle with like threads with fire hose and renew foam pickup in line proportioner.

Renew flame screen on #5 vent pipe.

Label both bridge engine remote shutoffs.
