The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Entire engine room is protected by a fixed fire extinguishing system. The space is not vapor tight which will result in the fire extinguishing system agent to escape limiting fire fighting capabilities. Make protected spaces vapor tight to the satisfaction of the attending USCG Marine Inspectors.

All vessels shall have efficient guard rails or bulwarks on decks and bridges. Various areas of guard rails on the exposed decks showed signs of wastage and damage.

The hydrants for exterior locations may substitute two 11/2 in outlets, each with a 11/2-in hose, supplied through a wye connection. 01 hydrant was found on the STBD side with a single 1 1/2in hose connected.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. Aft door of accommodation space wasted on the bottom affecting weathertight integrity.

Each outlet or junction box must have a cover unless a fixture canopy, switch cover, receptacle cover, or other cover is used. Junction box in the port side passageway in accommodation space was found to be missing a cover. Place appropriate cover over outlet box.

The integrity of any deck in way of a stairway opening, other than a stair tower, shall be maintained by means of “A” or “B” class bulkheads and doors at one level. The integrity of a stair tower shall be maintained by “A” Class doors at every level. The doors shall be of self-closing type. Multiple doors throughout the accommodation spaces did not close properly and are in need of adjustment.

Means and arrangements for securing and maintaining weathertightness shall ensure that the tightness can be maintained in any sea conditions. Gasket around opening in forward engineering space missing in sections affecting weathertight integrity.

Each immersion suit or anti-exposure suit must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the immersion suit or anti-exposure suit. 02 immersion suits were spot checked and were fount with light battery expired.

The lifebuoys with attached self-igniting lights must be equally distributed on both sides of the vessel. 03 lifebuoys were found with missing activation tabs.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced as indicated. Two self-contained breathing apparatus were overdue for annual inspection.

At each inspection for certification, periodic inspection and at other times necessary, the inspector will determine that all fire-extinguishing equipment is in suitable condition. USCG Marine Inspectors attended the vessel on 14Oct2025 to conduct an Annual Inspection. During attendance, inspectors found several items that needed immediate repair. In addition, representatives of the vessel notified the MI’s that both the CO2 and the Halon fire systems were down for service. It was determined the systems were down due to condemned bottles. It was discussed and agreed to schedule the inspection at a later date. The vessel is currently in an “maintenance downtime” till the end of the month. Vessel representatives and CG MI’s agreed it would be in the vessel’s best interest to schedule an inspection after the maintenance period is over. Provide Sector New Orleans with a plan for fire protection for the related spaces while both CO2 and Halon systems are down for service to the satisfaction of the attending MI. In addition, the vessel is restricted to the pier until a Annual COI is completed by Sector New Orleans. 46 CFR 91.25-20(a)

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Drawings of alterations shall be approved before work is started unless deemed unnecessary by the Officer in Charge, Marine Inspection. Submit, for approval, drawings as appropriate for all repairs to be conducted.

In special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. Multiple permanent and temporary cracks and defects were repaired in the port aft ballast tanks. Provide an assessment of structural condition of the alternate/starboard side.

A complete recharge shall be carried for each self-contained breathing apparatus, and a complete set of spare batteries shall be carried for each flashlight. The spares shall be stowed in the same location as the equipment it is to reactivate. Vessel lacked a complete recharge for each SCBA.

Bulkheads of galleys, paint and lamp lockers, and emergency generator rooms shall be of “A” Class construction. Galley did not have structural fire protection.

When a compartment containing the emergency source of electric power, or vital components thereof, adjoins a space containing either the ship's service generators or machinery necessary for the operation of the ship's service generators, all common bulkheads and/or decks shall be protected by approved “structural insulation” or other approved material. Cable penetrations between the generator room and the machinery space below lacked approved insulation.

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. Lifejackets had expired lights attached.

Each vessel must have a line-throwing appliance approved under approval series 160.040. Each vessel must carry the equipment on the list provided by the manufacturer with the approved appliance and an auxiliary line that is at least 450 meters (1,500 feet) long. Vessel lacked 4 line-throwing appliances with auxiliary line and projectiles.

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” Spaces protected by CO2 lacked appropriate signage.

While at a port under the jurisdiction of the U.S., a ship is subject to inspection by the Coast Guard to determine that a valid IOPP Certificate is on board and that the condition of the ship and its equipment corresponds with the particulars of the certificate. An inspection under this section may include an examination of the Oil Record Book. Oil record book entries lacked corresponding receipts.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. Vessel lacked chemical testing records for crewmembers.

The master, operator, or person who is in charge of a ship shall ensure that all garbage is discharged ashore or in accordance with §§ 151.66-151.73. The following factors, among others, may be considered by enforcement personnel in evaluating compliance with §§ 151.51 through 151.77: Records, including receipts, of garbage discharges at port reception facilities. Vessel lacked garbage receipts.

The owner of a vessel subject to inspection under section 3301 shall-(1) ensure that such a vessel is equipped with a vessel master key control, signage, and surveillance requirements regarding the Coast Guard's Sexual Assault and Sexual Harassment Prevention and Response programs for commercial vessels. Follow guidance in implementation IAW Coast Guard Policy Letters 23-04, 23-05, 23-06.

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight doors. Vessel lacked two means of escape from welder's room.

Electric installations on vessels must ensure:(2) Protection of passengers, crew, other persons, and the vessel from electrical hazards. Vessel #3 Generator Main Switchboard was not locked out and tagged out.

Entries shall be made in the Oil Record Book on each occasion, on a tank to tank basis if appropriate, whenever any of the following machinery space operations take place on any ship to which this section applies—(2) Internal transfer of oil cargo during voyage. Vessel conducted internal transfers that were not logged into the ORB.

Audible alarms must generate sound pressure of at least 10dBA above ambient noise levels existing during normal operations w/ the ship underway in moderate weather. Fire Detection audible alarm in the portable accommodation module was not producing audible noise.

Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valves located at the tank. Vessels starboard side emergency shutdown valve #1, emergency shutdown valve #2 and EMD high suction valve inoperable. 46CFR56.50-60(d)

Each firehose on each hydrant must have a combination solid stream and water spray firehose nozzle approved under subpart 162.027 of this chapter. Two fire nozzles at station #4 and on fire nozzle at station #8 were not of the approved type. 46CFR95.10-10

Ocean going sailing vessels and barges shall be provided with pumps connected to the bilge main as required under subpart 56.50-55(b)(1). Vessel bilge pump inoperable. 46CFR56.50-55(b)

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. Junction box in engine room storage space missing caps. 46CFR111.05-3(a)

Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valves located at the tank. Vessels port side #1 fuel tank shutoff valve inoperable. Vessel crew repaired valve on the spot. 46CFR56.50-60(d)

Dredges in or near a channel or fairways, in operations likely to restrict or affect navigation of other vessels must comply with the Bridge-to-Bridge Act. Vessels certificate was expired, obtain current certificate. 46CFR80.1001

Electric installations on vessels must ensure the protection of passengers, crew, other persons, and the vessel from electrical hazards. Radio equipment in bridge was found with multiple dead-end wires. Repair or remove. 46CFR111.01-1(a)

It is intended plans be submitted as will clearly show the vessel's arrangement and required equipment. Vessel fire safety plan does not match current arrangement of safety equipment and does not label numbered portable fire extinguishers. 46CFR91.55-1

Portable extinguishers must be inspected at least once every 12 months. Vessels extinguisher for the paint locker had not been serviced since 2021. 46CFR97.15-60

A hydrostatic release unit (HRU) used in a float-free arrangement must be approved under approval series 160.162. All of the liferafts’ disposable HRU's were installed with an exp. date longer than the 2 year exp. date permissible. 46 CFR 199.130(c)(7)

Portable fire extinguishers and their stations must be numbered in accordance with 46CFR97.37-23. Vessels extinguishers were not numbered. 46CFR95.50-10(e)

Each liferaft must be arranged for float-free launching and the arrangement must ensure that the liferaft, when released and inflated, are not dragged under by the sinking vessel. All of the liferafts’ (08) painter lines were installed incorrectly to the hydrostatic releases, preventing them from being cut from the vessel in an emergency. 46 CFR 199.130(c)(7)

Each vessel must carry lifejackets approved under approval series 160.155, 160.176 or 160.177. Two of the lifejackets were condemned due to deterioration of the buoyant material inside the lifejacket. 46 CFR 199.70(b)

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. 12 lifejacket had expired lights. 46CFR199.70(b)(4)(i)

The inspection shall be such as to insure that the vessel, as regards the structure, boilers and other pressure vessels and their appurtenances, piping, main and auxiliary machinery, electrical installations, lifesaving appliances, fire-detecting and extinguishing equipment, pilot boarding equipment, pollution prevention equipment, and other equipment, is in satisfactory condition and fit for the service for which it is intended. Vessel was still in process of completing multiple repairs from drydock and was not ready for inspection.

The assignment of load lines is conditioned upon the structural efficiency and satisfactory stability of the vessel, and upon the provisions provided on the vessel for effective protection and that of the crew. A hole was found on the port side of superstructure to engine room.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Vessel's life rafts were ashore getting serviced at time of inspection.

Vessel's Portable Accommodation Module (PAM) Serial CG1537262 is damaged and door would not properly close. Vessel restricted from utilizing PAM and limited to carrying 48 industrial passengers until repaired as per manufacturer to the satisfaction of the attending marine inspector.

The emergency power source must have the capacity to supply all loads that are simultaneously connected to it. Emergency generator is inoperable.

A vessel engaged in dredging or underwater operations, when restricted in her ability to maneuver, shall exhibit the lights and shapes prescribed in paragraphs (b)(i), (ii), and (iii) of this Rule. Vessel's dredge lights were in the process of being moved.

All crew spaces are to be constructed and arranged in a manner suitable to the purpose for which they are intended. Vessel's lever room/pilot house windows are in the process of being replaced.

Means shall be provided for closing all openings to the space protected from outside such space. Fire dampers in both center and stbd engine room intake does not close.

There shall be at least two means of escape from all general areas where crew may be quartered or normally employed. Throughout the accommodation spaces the second means of escape are blocked by various items being stored. In the avetron room one door would not open and had caution tape posted on it.

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Two emergency lights in engine room were inoperable.

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval. Emergency lights are not identified on fire and safety plan.

Each outlet or junction box must have a cover. In spud room, junction box in overhead is missing cover.

Equipment for interior locations not requiring exceptional degrees of protection must be manufactured to at least NEMA 250 Type 1 with dripshield or IEC 60529 IP 11 as specified in IEC. Vessel missing drip shield on battery charger in spud room.

Each berth must have a light. Master’s stateroom and Operator’s state room are missing bunk lights. Operator’s state room has exposed wiring where bunk light should be. Multiple other stateroom lights did not work tested.

The Marine Safety Center may approve any arrangement, fitting, appliance, apparatus, equipment, calculation, information, or test that provides a level of safety equivalent to that established by specific provisions of this subchapter. Vessel is utilizing multiple unapproved power strips through out accommodation spaces.

Each flexible cord and cable must be connected to a device so that tension is not transmitted to joints or terminal screws. Vessel's speaker horn in engine room is hanging via it's wires.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. No receipts on board as required by garbage management plan. Prove garbage management plan is being followed.

The shipboard oil pollution emergency plan must be available on board. Current revision of the SOPEP not on board. Prove updated copy of SOPEP is on board vessel.

Previous wire penetration at top of generator room leading into accommodation house needs to be capped or plugged to maintain fire boundary. 46 CFR 92.07-10 (c)

Two (02) Class A fire doors from generator space into accommodation space and one (01) Class A fire door from engine room into accommodation space do not fully close and latch. Make repairs and prove proper operation. 46 CFR 92.07-10

Inspectors found wastage beyond allowance and holes in the vessel's superstructure. Vessel has placed temporary repair patches on those areas. Permanent repair must be made at drydock or by 31Jan2021, whichever occurs first.

In general, compliance with the standards established by ABS will be considered satisfactory evidence of structural efficiency. Web frames 27 and 28 on port side of engine room, as well as frame 41 in engine room are holed. Vessel has place a doubler on those frames. Permanent repair must be made at drydock or by 31Jan2021, whichever occurs first.

Complete but simple instructions for the operation of the fire extinguishing systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. Vessel created temporary instructions for CO2 system and Halon system until permanent placard from manufacture arrives.

Drawings of alterations shall be approved before work is started unless deemed unnecessary by the Officer in Charge, Marine Inspection. Shipyard is adding a fire pump to the vessel in addition to the existing ones. Vessel must submit plans of modification to the Marine Safety Center for approval. Once plans are approved and any comments are resolved, system installation must be installed per approved plan and verified by the attending marine inspector.

No repairs or alterations affecting the safety of the vessel with regard to the hull shall be made without the knowledge of the Officer in Charge, Marine Inspection. Went onboard vessel and found unauthorized crop and renew of port side shell plating as welll as a weld on 20 inch crack located port side IWO pump room hatch. Vessel to provide USCG with detailed repair proposal for review and approval.

Fire hydrants must be of sufficient number and so located that any part of the vessel, other than main machinery spaces, accessible to persons on board while the vessel is being navigated and all cargo holds may be reached with at least 2 streams of water from separate outlets. Fire main at stations #9 and #29 is holed and leaking. Additionally, 10 sections of fire main in the engine-room had unauthorized temporary repairs (6 clad weld and 4 soft patches). This condition is rendering ineffective stream of firefighting water from outlet. Effect permanent repairs ensuring proper operation of firefighting arrangement.

On vessels of 500 gross tons or more there must be at least one shore connection to the fire main available to each side of the vessel in an accessible location. While the shipboard fire pumps are out of service due to maintenance, the vessel depends on shore-side connection operation for firefighting. Inspectors found port shore-side connection valve inoperable and holed, and Stbd damaged so as to impede the connection from providing sufficient water pressure at any fire station. Prove proper operation of shore side connections.

The assignment of load lines is conditioned upon the structural efficiency and satisfactory stability of the vessel, and upon the provisions provided on the vessel for effective protection and that of the crew. Wastage and holes on the hull, bulkheads, and pipes adversely effect watertight and weather-tight integrity, create down-flooding points that affect subdivision, and allow the unintended movement of fluids into compartments not considered for the vessel’s stability. Inspectors found wastage beyond allowance, severe pitting, and holes in vessel’s structures that are critical to its stability and subdivision, to include: 1) #3P diesel fuel tank, leaking into the engine compartment, 2) #3S fuel tank and overflow pipe, leaking fuel into the adjacent sea-chest void, 3) on the weather-deck to EDG space, 4) on the superstructure to the 01 deck accommodation spaces and 02 bridge deck, 5) on ballast ,void, lube oil and fuel (vents) 6) on the stbd sea-chest and port raw water manifold and 20" pipe at sea-chest intake, 7) on the mechanical ventilation for EDG and Main space Engine room are holed, and 8) hole in Side shell on stbd side at frame 34. The vessel may not sail until it provides a comprehensive structures gauging report to identify the extent of wastage, and effect permanent repairs.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. 20 inch crack in a structural beam was found on port side support for dredge head. This condition could lead to the collapse of dredge head crane. Effect permanent repairs that meet ABS structural standards.

In general, compliance with the standards established by ABS will be considered satisfactory evidence of structural efficiency. Significant wastage in starboard sea chest valve void space. Show wasted plate complies with ABS structural standards.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. Holes were found on frame #41 in E/R . Make permanent repairs.

Each bilge suction must lead from a manifold unless approved by MSC. Bilge manifold aft suction has weld leaking bilge water. Prove proper operation of bilge system.

Electrical installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Throughout the entire vessel, but especially on the bridge, multiple power strips and extension cords are being utilized due to location and availability of existing receptacles. Provide proper outlets as required.

If the installation of the electrical equipment does not ensure a positive ground to the metal hull or equivalent conducting body, the apparatus must be grounded to the hull with a grounding conductor. Vessel’s grounding wire for equipment under bridge is connected to the hull with a metal clamp. Remove temporary arrangements and make permanent.

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. The conductor going into the battery charger for the bridge navigation equipment is exposed. Ensure conductor is not exposed.

In a space described in 113.25-9(a), where the general emergency alarm signal cannot be heard over the background noise, there must be a red flashing light or rotating beacon, in addition to the general emergency alarm signal. Light for general alarm on the port side of engine room is not flashing. Prove proper operation of emergency light.

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. Throughout vessel there are multiple dead end wires with electrical tape wrapped around the end of the wire. Properly terminate all identified wires.

All safety and relief valves for use on pressure vessels shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at a pressure which does not exceed the "maximum allowable working pressure" of the pressure vessel. Air receiver for aft crane has a broken safety relief valve. Prove proper operation of relief valve.

Complete but simple instructions for the operation of the fire extinguishing systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. Instructions posted for operation of the generator room fire suppression system was that of an old system.

Emergency lighting should provide illumination in the portable accommodation modules (PAM) for the same period required for the host vessel. Neither PAM had working emergency lighting. Prove proper operation of emergency lighting.

Prior to installation aboard vessel, diagrams of the following systems shall be submitted for approval: Fuel oil service, bilge and ballast piping. Vessel has made modifications to their engineering systems and vessel representative could not produce approved plans.

There shall be at least two means of escape where the crew may be quartered or normally employed. In the portable accommodation module the door leading to the emergency exit was locked.

A survey shall be made every time a defect is discovered which affects the safety of the vessel or whenever important repairs are made. Vessels has pitting over 1/4" on #3 Stbd fuel tank. Provide corrective action plan acceptable to the OCMI.

Engine design. All Installations shall be of marine type. Engines suitable for the intended service. Raw water leaks were found on stbd sea chest and port cat raw water manifold. Provide corrective action plan acceptable to the OCMI.

A survey shall be made every time a defect is discovered. Reduction gear for pump motors are leaking gear oil. Provide acceptable repairs to the attending marine inspector.

A survey shall be made every time a defect is discovered which affects the safety of the vessel or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made. The material and the workmanship of such repairs or renewals are in all respects shall be satisfactory, and that the vessel complies in all respects with the regulations. Vessel is leaking fuel from 3 Port into engine room. Provide acceptable repair plan to OCMI addressing corrective actions for leak on fuel tank number 3 Port.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel has approximately 3"x1" crack on the tank top of cargo tank #__. Vessel shall affect permanent repairs approved by the Coast Guard and test tank to 5 psi prior to movement.

Non self-propelled vessels shall be provided with pumps connected to the bilge main. Vessel is unable to prove proper operation of the bilge pump suction.

The structural standards of the vessel must be in compliance with the standards establisted by ABS. MI identified a cracked structural member in the E/R and a hole in the deck in the swing winch room.

Electral installations on vessels must ensure protection of crew and the vessel from electrical hazards. MI identified mutiple cable terminations and junction boxes that were incorrectly placed into service. Also a 480V cable not removed from service.

Machinery capable of injuring personnel shall be provided with covers or guards. Main engine shaft guard is damaged.

Vessel subject to load line have special requirements as to the closure of openings in water tight bulkheads. The self-closing device for the WTD in the port and stbd entrance to the crew lounge needs to be adjusted.

Vessel must have all emergency lighting in proper operation. MI identified the #11 e-light port side main deck is not working.

Each item of survival equipment with an expiration date must be replaced during the annual inspection. The battery installed in the ring lifebuoy was the incorrect style.

Approved hand portable fire extinguishers shall be installed on board. Vessel was missing all portable extinguishers in both of the PAM's installed.

Each fire detector should be approved under 46 CFR 161.002 with satisfactory operation to the USCG. Vessel rep was unable to find a can of smoke to conduct tests on the PAM's detection system.

Emergency lighting should provide illumination for the same period required for the host vessel. Both PAMs had various E-lighting that did not work.

Each general alarm signal should be marked "GENERAL ALARM - WHEN EMERGENCY ALARM SOUNDS GO TO YOUR STATION" in red letters at least 1/2 inch high. None of the general alarms were labeled in either PAM.

A general arrangement plan should clearly show location on the host vessel where the PAM is installed, details of the accommodation spaces, means of escape, guard rails, lifesaving, and firefighting appliances. Vessel did not have an updated general arrangement plan on board.

The fire and safety plans should be updated to reflect the additional safety equipment associated with PAM installation and be posted in a conspicuous location inside the PAM. Neither PAM had an updated fire and safety plan posted.

There shall be at least two means of escape from where the crew may be quartered. Vessel was storing various objects in areas where a means of escape was located.

The general construction shall be as such to minimize fire hazards and electrical hazards on board. Vessel has the hot water heater wired through an extension cord that runs down to the wall in the PAM's.

Each immersion suit must be designed so that a light is attached to its front shoulder. Vessel did not have a light on any immersion suit located in the PAMS nor were the locations of the immersion suits marked.

An inspection shall be made whenever any important repairs or alterations are undertaken. Vessel discovered and reported a pinhole leak in an internal fuel tank. Tank has been emptied and will remain empty until permanent repairs are made. Permanent repairs are to be made to the satisfaction of the USCG within 60 days at next maintenance availability.

A 6"x 1.5" crack at the #1 port fuel tank top and damaged handrail stanchion was caused by a collision with a tug. The vessel shall make temporary repairs to fuel tank top and stanchion by removing fuel within the tank and isolating the tank from the fuel systems. The opening shall be minimized and sealed to prevent down flooding as discussed with vessel chief engineer. The vessel shall submit a repair proposal for permanent repairs and once approved, complete the repairs no later than 15FEB17. 46CFR 91.45-1, 46CFR 92.01

Time delay for halon system did not activate in proper amount of time. Replace time delay. 46 CFR 95.16-90 and 46 CFR 95.16-45. Prior to operating machinery in effected space.
