Key personnel must complete training in 33CFR101.650(d)(2) by January 12, 2026. Company does not have a CYSO and all key personnel have not yet completed the training as of this inspection. 33 CFR 101.650(d)(4) 705: 60 days

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Tropical Storm Philippe generated high winds and turbulent seas in the USVI & BVI area. O/O reported that the M/V ADVENTURER took bow damage after a cargo vessel broke free from its moorings and allided into the M/V ADVENTURER. Prior to carriage of passengers, make vessel available for MIs to a conduct damage survey. Provide repair proposal to USCG for OCMI review/acceptance prior to conducting repairs.

Generator space: Visible daylight seen through 8" gouge penetrating the sideshell approximately .5 feet below the main deck. 2. Steering gear space: Approximately 10ft transverse x 5 vertical hull plating and adjacent longitudinal and transverse frames severely damaged including broken welds, twisting, compression, cracking, warping, and bulging. Visible daylight seen through separation between stern and main deck. 3. Port bow: approximately 8' length by 6' vertical sideshell plating with through-hull penetrations visible daylight seen, sideshell-main deck separation and damaged adjacent framing including twisting, compression, cracking, warping, and bulging. 4. Bow stem approximately 8 ft visible warped. 5. Bulwarks and railing on bow twisted, warped and cracked. Railing above bulwark broken off. Approximately 10' on starboard bow and 10' on port bow. 4 stanchions on starboard bow bulwark twisted with broken welds.

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Remove dead end wiring/junction box near port side steering gear compartment port bulkhead. Port side compressor motor wiring connections in E/R are the twist cap type. Provide pressure type fittings IAW 46 CFR 183.340

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port shaft cutlass bearing damaged and no longer serviceable. Replace bearing.

46 CFR 177.300 Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Make repairs to cracked welds found on damaged port shaft stern tube, strut skirt and strut.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Remove port rudder for inspection of rudder stock.

When required by the marine inspector, a portion of the inspection must be conducted while the vessel is underway so that the hull and internal structure can be observed. Conduct satisfactory sea trials in the presence of attending USCG marine inspector.

Prove proper operation of pressure relief valves on both installed air receivers.

Provide proof of LRIT exemption.

Provide current EPIRB hydrostatic release.

Provide a means separating child life jacket storage from adult storage.

Remove broken refrigeration machinery from below deck in forward accommodation space.

Soft patch discovered on center main diesel engine turbo cooling line. Make permanent repairs.

Main switchboard missing interlock. Provide interlock to prevent closing of both generator breakers at the same time. Label all other breakers.

Replace hand bar on front of switchboard to prevent a person from accidentally falling into it.

Provide exhaust lagging for all exposed exhaust piping before the cooling water inlet.

Repair exhaust leak on outboard of starboard engine.

Make permanent repairs to hull inset on starboard bow.

Replace missing/broken hose clamps on exhaust cooling lines in lazarette.

Install blanks in missing breaker panel in wheel house breaker panel.

Label port side emergency fuel shut-off.

Replace two broken windows on starboard side main cabin.

Prove enrollment of all crewmembers in company dapi program.
