The master must ensure all lifesaving equipment is properly maintained and ready for use; stbd life ring plug missing, compromising ring buoyancy.

Oceangoing towing vessel must carry 6 day and 6 night visual distress signals; missing 3 distress signals. Purchased appropriate amount and put on board.

No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. Provide list of employees that have passed a chemical test and enrolled in an random drug and alcohol program.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. Various flammables and combustibles materials were found stored in the following locations: laundry room, space below pilot house, engine room alley, engine room port aft quarter.

Each towing vessel must be fitted with a general alarm that has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station”. Vessel was found without an installed general alarm system for E/R to include a flashing red light and sign.

Stations in the maritime service must be licensed by the FCC either individually or by fleet. MI discovered vessel didn't have a radio station license on board. Provide a radio station license.

Each towing vessel operating on an oceans route must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. The vessel's first aid kid did not have the required items.

Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the Serious Marine Incident. The alcohol testing must be conducted within 2 hours. The vessel's alcohol testing swabs on board were found to be expired.

Electrical connections must be suitably installed to prevent them from coming loose. The vessel's lighting in the CO2 cylinder storage room was not properly mounted and installed.

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure all crew members are capable of performing the duties expected of them during an emergency. The vessel needs to update emergency procedures to reflect securing the main engines prior to activating fixed CO2 system.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists. The following alarms must be provided: high bigle levels. Alarms must be visible and audible at each operating station. The vessel's alarm panel did not specify which bilge space was alarming.

A towing vessel 79 feet or more in length must carry four lifebuoys. The vessel only had three lifebuoys on board at the time of the inspection.

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Vessel does not have a line throwing device onboard.

Each towing vessel must have a fire-detection system installed to detect engine room fires. The system is arranged and installed so a fire in the engine room automatically sets off alarms on a fire detection control panel at the operating station. The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience. A fire detection system/fire control panel is not installed onboard the vessel.

A towing vessel with overnight accommodations and alternating watches (shift work), when pulling, pushing or hauling alongside one or more barges, must have a system to detect when its master or mate (pilot) becomes incapacitated. The system must: (1) Have an alarm in the pilothouse distinct from any other alarm; (2) Require action from the master or officer in charge of a navigational watch, during an interval not to exceed 10 minutes, in order to reset the alarm timer; and (3) Immediately (within 30 seconds) notify another crewmember if the pilothouse alarm is not acknowledged. A Bridge Alerter System is not installed onboard the vessel.

Each towing vessel operating north of lat. 32° N. or south of lat. 32° S. must carry must carry at least one appropriate size immersion suit, approved under approval series 160.171, for each person onboard. The vessel is certificated for an oceans route (exposed route) but does not have immersion suits available.

Each towing vessel 79 feet or more in length operating on oceans and coastwise routes that does not have an installed fixed fire-extinguishing system must have the following: (a) At least two firefighter's outfits that meet NFPA 1971 (incorporated by reference, see § 136.112 of this subchapter); and (b) Two self-contained breathing apparatus of the pressure demand, open circuit type, approved by the National Institute for Occupational Safety and Health (NIOSH), under 42 CFR part 84. The breathing apparatus must have a minimum 30-minute air supply and full facepiece. Vessel does not have onboard firefighter outfits or SCBAs.

A towing vessel that is Certificated for limited coastwise, coastwise, oceans or waters beyond 3 nautical miles from shore on the Great Lakes, whose contract for construction was executed prior to August 27, 2003 must be equipped with either an approved 160-B semi-portable fire-extinguishing system to protect the engine room; or a fixed fire extinguishing system installed to protect the engine room. Vessel does not have a 160-B or fixed fire suppression system onboard.

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. Insulation was missing from the exhaust piping of both generators located within the engine room.
