Unless liferafts can be readily transferred for launching on either side of the ship, additional liferafts shall be provided so total capacity will accommodate 150% total persons on board. Vsl is not equipped with 150% total liferaft capacity on each side.

Before being assigned to shipboard duties, all persons employed or engaged on a seagoing ship which is required to comply with provisions of the ISPS code, shall receive approved security familiarization training. 02 members did not have adequate Security Awareness training for STCW Code.

The validity of calibration certificates should be checked at IOPP renewal surveys. The accuracy of 15 ppm bilge alarms is to be checked by calibration and testing of the equipment conducted by a manufacturer or persons authorized by the manufacturer. OWS bilge monitor calibration was not IAW MEPC.107(49).

Each ship with a rechargeable system with ozone-depleting substances shall maintain an ozone-depleting substances record book. Vsl missing ozone-depleting substance log.

All fuel pipes, pipe connections, and accessories must be readily accessible, protected against mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal liners or straps.

In cargo ships of 1,600 tons gross tonnage and upwards the Administration shall ensure that the lightning of the alleyways, stairways, and exits is such that the access of all persons on board to the launching stations and stowage positions of liferafts is not impeded. 02 Stbd engine room emergency lights inoperable.

CODE 60 - The main steering gear must be operable from the pilothouse by mechanical, hydraulic, electrical, or other means acceptable to the Commanding Officer, Marine Safety Center. This gear and its components must give full followup control of the rudder. MI received report from Operator that Follow-up steering was not working. Vessel was permitted to transit to the Fort Morgan anchorage to await repairs. Owner/Operator must notify local OCMI and receive authorization prior to any further vessel movement. Owner/Operator must submit tech report attesting the satisfactory repair of steering system to the satisfaction of the attending Marine Inspector. **UPDATE** Owner/Operator has requested to operate vessel bypassing the power failure alarm module which has been granted by the OCMI. Vessel must have support vessel within 1 nm at all times, and notify the OCMI immediately if conditions change. Owner/Operator must submit tech report attesting the satisfactory repair of steering system to the satisfaction of the attending Marine Inspector prior to clearance of 835. 46 CFR 58.25-70

In a space described in § 113.25-9(a), where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon, in addition to the general emergency alarm signal. Vessel had multiple general alarm lights not operating properly. Fix all lights not working through the vessel to the satisfaction of a Coast Guard Marine Inspector.

Be located in passenger and crew quarters areas where they can alert persons in spaces where those persons may be maintaining, repairing, or operating equipment, stowing or drawing stores or equipment, or transiting, such as public spaces, work spaces, machinery spaces, workshops, galleys, emergency fire pump room, bow thruster rooms, storage areas for paint, rope, and other stores, underdeck passageways in cargo areas, steering gear rooms, windlass rooms, holds of roll-on/roll-off vessels, and, except those that are accessible only through bolted manhole covers, duct keels with valve operators. Multiple bells for the general alarm system throughout the vessel were found not to be operating properly. Repair / replace all general alarm bells not working to the satisfaction of a Coast Guard Marine Inspector.

Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm set points, the procedure to be followed, and the expected test result. Vessel crew was found to not be testing PSTP’s in accordance with on board approved manual due to lack of proper calibrated test equipment. Vessel will procure proper equipment or utilize approved third party company to demonstrate PSTP’s to the satisfaction of a Coast Guard Marine Inspector.

Each hand portable fire extinguisher shall be marked with a number and the location where stowed shall be marked with a corresponding number at least 1/2 inch high. Where only one type and size of hand portable fire extinguisher is carried, the numbering may be omitted. None of the vessels portable fire extinguishers on board are properly labeled. Label all portable extinguishers to the satisfaction of a Coast Guard Marine Inspector

Where it is demonstrated that the installation of a stairway would be impracticable, a vertical ladder may be used as the second means of escape. Starboard forward pump room upper level emergency escape ladder is damaged and stowed in manner that is not accessible to all crew members in an emergency. Repair ladder and keep ladder stowed in the down position except where necessary for operations. Repairs shall be made to the satisfaction of a Coast Guard Marine Inspector.

For inspection procedures of marine engineering equipment and systems, see subchapter F (Marine Engineering) of this chapter. The requirements of § 58.10-5 (a), (c), and (d) apply to diesel engine installations. Exhaust pipe installations must conform to the requirements of ABYC P-1. Protective guards shall be provided wherever persons might come in contact with the exhaust system where the temperature exceeds 200°F. Bow thruster engine and starboard pump room engine missing exhaust lagging. Provide proper lagging to the satisfaction of a Coast Guard Marine Inspector.

If more than one device is stowed in a location, the number of devices stowed must be indicated. Immersion suit stowage location did not have the total numbers marked. Properly mark immersion suit numbers at each stowage location to the satisfaction of a Coast Guard Marine Inspector.

Each immersion suit or anti-exposure suit must be marked in such a way as to identify the person or vessel to which it belongs. Three immersion suits were found marked with the wrong vessel name. Properly mark immersion suits to the satisfaction of a Coast Guard Marine Inspector.

Containers, brackets, racks, and other similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Resolution A.760(18) indicating the device stowed in that location. Lifebuoy stowage location missing proper markings. Provide proper markings to lifebuoy stowage location to the satisfaction of a Coast Guard Marine Inspector.

Lockers or spaces containing self-contained breathing apparatus shall be marked “SELF CONTAINING BREATHING APPARATUS”. Lockers near bridge were missing proper markings. Properly mark all containers to the satisfaction of a Coast Guard Marine Inspector.

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight doors. All emergency escape scuttles for both engine rooms and pump rooms have areas of corrosion requiring attention to include holdback devices and knife edges. Repair or replace all emergency escape scuttles to the satisfaction of a Coast Guard Marine Inspector at next drydock but no later than 31DEC2025.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made. Vessel has 1/2" X 8" hole in STBD #2 wing tank. Submit plans to local OCMI for review berfore conducting repairs.

Descale & Provide U/T in 10" x 10" grid & Pit Gauging report for suspect wasted areas in side shell as marked and identified from frames 14-25 Port Side Pump room & area IWO of Doubler behind switchboard to ensure wastage is not greater then 25%. Submit Repair Proposal for 3 wasted frames in that area.

Repair Freshwater Expansion tank in STBD Pump room leaking in vicinity of Welding Machine.

Descale and provide U/T shots iwo of vertical weld seam as marked in Port Engine Room.

Provide CO2 Warning signs at entrances to Protected Spaces.

C/R wasted Paint Locker Vent.

The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel has an approximately 1/8" hole in side shell: located 2 inches aft of frame 85 and 1.5inches above the main deck line on stbd side shell. Vessel shall implement temporary repair to the satisfaction of the attending ABS Surveyor under the delegation of Loadline Authority. Provide Coast Guard with ABS Report upon completion of temporary repair.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel has an approximately 1/8" hole in side shell: located 2 inches aft of frame 85 and 1.5inches above the main deck line on stbd side shell. Vessel personnel shall provide proof of adequate integrity of the side shell in the surrounding area of the reported hole.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel has an approximately 1/8" hole in side shell: located 2 inches aft of frame 85 and 1.5inches above the main deck line on stbd side shell. Vessel personnel shall provide a permanent repair proposal, affect permanent repairs approved by the Coast Guard, and satisfactorily test the repair.

A certification that the ship has passed an inspection must be entered into the ship's log by the inspecting technician. During inspection of the ship's radio equipment the technician noted that the vessel's AIS was inoperable due to not displaying the heading or rate of turn information. Provide technician's report attesting to the proper operation of the AIS as well as valid FFC certificates. 47 CFR 80.59

All vessels must submit Notice of Arrivals(NOA) within the following times: If voyage time is 96 hours or more, at least 96 hours before arriving at the port or place of destination; or less than 96 hours, before departure but at least 24 hours before arriving at the port or place of destination. Vessel did not submit NOA within the required timeframe.

Inspection for certification shall include an inspection of the structure. Several sea chest were noted to be severely wasted and in need of potential repair Prove adequate material thickness to the satisfaction of attending Marine Inspector at next drydock in September 2020.

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Vessel does not have approved PSTPs. Vessel has submitted its PSTPs to MSC.

A protected space must be fitted with an approved available alarm that must sound automatically. Vessel's 30 second time delay did not function in correct time for FWD port and stbd pump rooms. 46 CFR 95.15-30. Correct prior to dredging operations.

Cylinders must be tested and renewed as required by 46 CFR 147.65. Cylinders must be rejected when they are severely corroded on the bottom 18 inches of the cylinder. Replace cylinders, hydrostatic test cylinders, or provide tech report attesting that wastage is within accepted tolerance. Correct prior to dredging operations. 46 CFR 91.25-20(a)(2)

A vessel shall exhibit, in the fore part an all around white light. Vessel's primary anchor light is not working. No operating during night or low visibility. 33 CFR 83.30(a)(i)

All enclosed spaces shall be properly vented. Ventilation motor for paint locker is inoperable. When deficiency is corrected, Marine inspector should ensure paint locker CO2 system pressure switch secures paint locker ventilation motor. 46 CFR 92.15-10(a)

The general structural strength of the vessel shall be sufficient and info shall be provided to the commandant when requested. ABS, provide Coast Guard a report attesting to longitudinal strength of hopper coaming, regarding cracks and wastage. 46 CFR 42.13-5.

An audible and a visible alarm must activate in the pilothouse for steering gear failures. Vessel's steering gear alarm panel only displayed a visible alarm. Audible alarm was inoperable. 46 CFR 58.25-25(d).

Where natural ventilation is provided for spaces protected by a CO2 system, provisions shall be made for easily and effectively closing off the ventilation. The lower port engine room fire dampener did not fully close. Prove effective dampener closing to attending USCG MI. 4646 CFR 15.15-35(b)

Electrical installations on vessels shall be designed so that the safety of crew shall be ensured. Reset button on the #2 steering control panel was missing. Repair to the satisfaction of attending USCG MI. 46 CFR 111.05-15

Fixtures shall not be used as connection boxes. Lighting fixtures in the stbd and port steering compartments were wired to provide power to a receptacle. Correct to the satisfaction of attending USCG MI. 46 CFR 111.75-20(a)(6)

All marine engineering details and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with Subchapter F. Both propulsion shaft seals are leaking seawater significantly. Repair to the satisfaction of attending USCG MI. 46 CFR 90.20-1

Means shall be provided for closing all other openings to the space protected by CO2 from outside such space. Cable penetrations in the upper stbd E/R are not sealed. Repair to the satisfaction of attending USCG MI. 46 CFR 95.15-35 (c)

The applicable standards established by the ABS Steel Vessel Rules may be used as the standard for design and construction for auxiliary machinery. Vessel's incinerator exhaust piping is severely corroded and has multiple holes. Repair to satisfaction of USCG MI. 46 CFR 58.01-5

Portable electrical cords may only be used for the connection of portable lamps or appliances. Non-permanent electrical wiring is installed for the port and stbd shaft seal pumps. Correct to the satisfaction of attending USCG MI. 46 CFR 111.60-5

The revised NOx Technical Code 2008 shall be applied in certification, testing and measurement procedures for the standards set forth in this regulation. Vessel modified pump engines in September 2019 and has documents from engine manufacture explaining the engines do not comply with EIAPP standards. Provide EAIPP and technical files to the satisfaction of attending USCG [name withheld]/13.8

Test procedures must be submitted to the Coast Guard Marine Safety Center for approval. Current test procedures onboard do not provide sufficient detail for testing machinery and equipment. Submit test procedures to CG Marine Safety Center for approval. 46 CFR 61.40-1

All enclosed spaces within the vessel should be properly vented. All supply ventilation screens in machinery spaces are clogged with dirt and debris. Clean to the satisfaction of Marine Inspector. 46 CFR 92.15-10

The general construction of the vessel shall be such to minimize fire hazards. Lack of overall general cleanliness is creating a fire hazard on upper level switchboard panels and wire runs and main engine piping and bulkheads in lower engine rooms. Clean to the satisfaction of Marine Inspector. 46 CFR 92.05-1

The general construction of the vessel shall be such to minimize fire hazards. All machinery space bilges are coated in oil, oily rags and debris. Clean to the satisfaction of Marine Inspector. 46 CFR 92.05-1

Inspection for certification shall include an inspection of the structure. The port engine room seachest and supporting gussets are wasted. Prove adequate material thickness to the satisfaction of attending Marine Inspector. 46 CFR 91.25-10

Fiddley, funnel, or machinery space in an exposed position on the freeboard or superstructure deck shall be fitted with strong covers of steel and capable of being secured weathertight. Machinery E/R stacks had holes which were filled with silicone. Make repairs to the satisfaction of attending USCG MI. 46 CFR 42.15-35

All vessels shall be provided with satisfactory bilge pumping and piping capable of pumping from and draining any watertight compartment. Discharge piping in port shaft seal pump has a soft patch and has pin-hole leaks. Make repairs to the satisfaction of attending USCG MI. 46 CFR 56.50-50

Electrical installations on vessels shall be such that the safety of crew and vessel will be assured from electrical hazards. The following electrical hazards were identified throughout the vessel: Port upper E/R crane power panel has a open stuffing tube and another stuffing tube is not properly secured. General service pump in the port E/R lower level has an improperly installed stuffing tube. Heat detector in upper port pump room is not properly mounted to bracket. Receptacle box is upper port pump room is not properly mounted to bulkhead. Breaker panel in the port pump room has a blank breaker which needs to be covered. Electrical cables in Port E/R above port SSDG are not supported sufficiently and heat detector is not properly mounted. Affix repairs to the satisfaction of attending USCG MI. 46 CFR 111.05-15(a)

The inspection for certification shall be such as to insure the vessel in regards to piping and auxiliary equipment are in satisfactory condition and fit for service. Soft patch was installed on #1 raw water pump in the stbd lower pump room. Make permanent repairs to the satisfaction of attending USCG MI. 46 CFR 91.25-10

Manual alternate control systems must be operable in an emergency and after a remote or automatic primary system failure. The air transfer valve failed to assume control of the port engine. Repair to the satisfaction of the attending USCG MI.

Vessel operator discovered damage to reduction gear during servicing. Effect repairs and provide technician's report attesting to repairs. Conduct operational testing in the presence of a CG Marine Inspector prior to departure.

Certification of the engine conforming that it operates within the limits set forth in paragraph 3. Vessel is missing EIAPP certificate and engine technical files.

Before the vessel leaves port and at all times during the voyage, each life saving appliance must be in working order and ready for use. Rescue boat would not start.

In addition to emergency power sources required under part 112 of this chapter each vessel must have at least two electric generating sources. STBD E/R gensent had mechanical failure.

All fluid power and control system and components shall be tested to allowable working pressure. Valve stack in E/R STBD is leaking and has wire nuts installed on removed DCV.

All installations shall be of marine type engines suitable for intended service. STBD dredge pump engine has F/O leak on #1 CRL.

Received report vessel suffered failure of bearing in the STBD reduction Gear while underway. No repairs affecting the safety of the vessel with regard to machinery shall be made with out the knowledge of the [name withheld] repairs to STBD reduction gear. Provide OCMI with attending ABS surveyors report and tech reports. Conduct operational test in the presence of CG MI.

Vessel's hull was breached by breach in bottom plating. Submit repair proposal to USCG, repair breach in accordance with repair proposal, and schedule inspection of repair to the satisfaction of the attending Coast Guard marine inspector.

Make temporary repairs to crack in vessel deck forward of the superstructure prior to transitting to Cape May, NJ.

Submit a repair proposal for permanent repairs to crack in vessel deck to local Coast Guard OCMI and ABS Surveyors.

Hole in stbd jet pump suction line.

AF01- Make permanent repairs to 1/4" hole found in the pipe between the sea chest and the valve in the Port Engineroom. Provide repair proposal and ABS Survey to this office forreview and approval.

Prove proper operation of #2 steering pump.

Replace 24 damaged immersion suits with new ones.

47,227 gallons of oily water discharged to shore between 7/3/16 to 7/18/16 was not logged into ORB. Chief Engineer and Captain must ensure oily waste transfers are porperly logged into ORB.

STBD pump room heat detector cap was ajar, retighten cap on heat detector.

Port side pump room heat detector is zip=tied in place. Permenantly mount heat detector to satisfaction of CGMI.

Fuel oil bunker line does not have drip sampler installed. Install drip samplor to meet MARPOL Annux IV requirements, to satisfcation of CGMI.

AF01- Make repairs to 2" crack found on the port side, near frame 35. Provide repair proposal and dive survey results to this office.

Discovered leaks from cracked hull bottom plate iwo frame 57 void, port hull. Cropped and renewed (6" x 27" x 1' plate).

Properly discover the extent of damage and conduct repairs to effect completeness to the vessel's bow thruster. Class/USCG to attest to completion of necessary repairs and any potential safety concerns to the vessel.
