The cognizant OCMI may require review of a vessel's watertight and weathertight integrity. This review may be performed by an individual who meets the requirements of §144.140. The review may include an examination of a plan that shows the original placement of decks and bulkheads.
- Vessel has 2 hull penetrations due to wastage in the forepeak around 2" in diameter approximately 2" above waterline.
- Stbd side ballast tank had 2 hull penetrations due to wastage at the seam of the stbd transverse frames approximately 4' long 1/2" wide.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route.
- Stbd side potable water tank 1,2,3 replace hatch gaskets.
- Port side potable water tank 1,2,3 replace hatch gaskets.
- Port side watertight door leading to engine room was
found wasted through.
- Stbd side watertight door gasket was missing.
- Stbd and port side ballast tank hatch cover replace gaskets.
- Replace Aft centerline hatch cover leading to the steering room compartment.
- Ventilation hole cut into forpeak tank approx. 5 inches in diameter, open to elements.
- Replace all gaskets for hatch cover and scuttle leading to steering compartment.
- Weathertight door leading from the engine room to the pilot house deck outside the house is not weathertight.

Each towing vessel operating on oceans, coastwise, limited coastwise, or beyond 3 nautical miles from shore upon the Great Lakes must carry a Category 1, 406 MHz satellite Emergency Position Indicating Radio Beacon (EPIRB) that meets the requirements of 47 CFR part 80.
Vessel did not have an EPIRB on board.

Sector San Juan has found the below deficiencies to be objective evidence that serious substandard conditions have not been pro-actively managed by the vessel's owner/operator, leading to a lack of safety culture on board the vessel.
