Shut off valves at the tank must be operable from outside the compartment. The shut off valves pull straight when full opened. The valves and pull cable shall be adjusted for more consistant closing operation. This shall be completed within 1 week. 46 CFR 182.455

Each ring life buoy on a vessel must be approved in accordance with 160.050 in subchapter Q of this chapter. The grab line on the ring life buoy was frayed and the retro-reflective material was peeling. 46 CFR 180.70

Rails must be installed near the periphery of all decks accessible to passengers or crew and be capable of withstanding a 200 lbs. force. The Stbd bow railing was loose at its mounting base. This should be properly fastened. 46 CFR 177.900

An exposed hazard,such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. The vessel did not have a guard installed for belt in engine room compartment. 46 CFR 177.960

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. A pre-engineered fixed gas fire extinguishing system must: be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector as per 46 CFR 181.420. The FireBoy preventer pin was found installed in the way of the manual remote activation pull cable. The crew immediately remedied the deficiency and the system is ready for immediate use. 46 CFR 185.100

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. The remote shutoff valves were found frozen open when tested by the crew. The crew serviced the valves and remedied the deficiency. 46 CFR 182.455

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The engine compartment forward bulkhead was found with three 2" diameter holes in it which permits the fixed gas extinguishing system agent to escape the engine compartment. 46 CFR 182.465(h)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. The operator installed blade type fuses in the DC electrical system. 46 CFR 183.380(l)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC station license certificate was found expired. 46 CFR 184.502

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The operator installed trim tabs on the transom using a standard wood screw to attach the trim tab under the waterline. The wood screw was found penetrating the transom in the engine room and presents a hull penetration hazard if it backs out of the transom FRP. 46 CFR 177.100

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. The lanyard connecting the water light was approximately 18 inches, and was not attached around the body of the life ring buoy. 46 CFR 180.70(d)(2)

Remove bow thruster motor from service and disconnect from power source due to fire. 46 CFR 183.320
