Each tank vessel must undergo dry dock, internal structural, and cargo tank internal examinations. As per MSM Vol II guidance the barge wishes to be designated as not in service but not gas freed. Vessel is granted a one year extension to its ISE requirements. The barge must maintain a valid COI, including annual and periodic inspections. The vessel must be boarded and checked for damage and/or cargo integrity at least once a week by an owner's rep. ISE must be performed prior to the barge going back into service. Vessel is allowed 4, one year extensions; not to exceed 4 years from the original ISE due date. At the end of the fourth extension the barge must undergo all required hull examinations or its COI must be surrendered.

Resolution: After reviewing gas free certificate and competent person log visually inspected and air tested all repairs. Air test and visual inspection of repairs revealed no deficiencies. Cleared PTP Barge is fit for route and service specified. inspection complete

Due to allision with I-40 bridge, the vsl sustained a 50' long inset of approx. 12" at the deepest point IWO #3P and 4P wing voids. The side shell inset caused the bottom plate to be pushed downward, and based on the divers findings, the plate was ripped from the transverse deep frames causing holes in the bottom plate of approx. 6" x 18" hole (5 ea). All cargo is to be offloaded and a PTP issued.
Action required: 17 - Rectify deficiencies prior to departure

Resolution: Inspected at National Marine for repair checks. Identifyed 2 crab leaks during PSI checks, leaks on welds were corrected on the spot. All other repairs and materials used were acceptible and to the satisfaction on the attending marine inspector.
