Emergency lighting must automatically actuate upon failure of the main lighting system. Vessels emergency lighting did not actuate properly.

All machinery and equipment installed must be suitable for its operation, intended purpose, and to afford adequate protection from causing fire, explosion, and personnel injury. Goose neck vent was not properly affixed to the lube oil tank in engine room.

Electrical equipment on a vessel must be installed so that it protects passengers crew, other persons, and the vessel from electrical hazards including fire. Vessel had one (01) extension cord and two (02) power strips found on bridge and near entry to engine room.

Each battery must be secured to protect against shifting with the roll and pitch of the vessel. Batteries near port generator not properly secured.

Irrespective of build date, a vessel operating on a coastwise or oceans route must meet 46CFR181.405 and 181.450. Vessel did not have interconnected detectors or visual means of alarm in berthing spaces.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard or rail. exhaust fan on PORT side of E/R had broken cover.

Cable and wiring requirements: (i)(1) the use of twist-on wire nuts is permitted under the following conditions. the connections must be made within an enclosure and the insulated cap of connector must be secured to prevent loosening due to vibration. Vessel had wiring with twist on style connectors, that were not in enclosure nor were secured to prevent loosening.

On a vessel more than 65' in length the public address system must be fixed installation and be audible during normal operating conditions throughout the accommodations spaces and all other spaces normally manned by crew members [name withheld] system was not audible below main deck.

Each entrance to a space storing carbon dioxide or protected by carbon dioxide must be conspicuously marked. Carbon dioxide spaces were not marked.

Vessel must have a seaworthy hull. Hull is cracked at port generator exhaust.

All emergency exits must be mared with 2 inch letters. Emergency escapes were not marked with 2 inch letter.

Each life float must have a waterlight. 2 waterlights were cracked and 1 did not function.

other lights exhibited cannot be mistaken for navigation lights. Deck lights were red and easily mistaken.

Pressure indicating gages' upper range must be 1.2 to 2 times the relief pressure. Gage was more than 2 times relief pressure.

Abandon ship drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of abandon ship drills or training. Vessel shall ensure abandon ship drills are being logged appropriately to include a drill scenario and training topics. 46 CFR 185.520(f)(2)

The owner or master shall instruct each crewmember, upon first being employed for the first time on a particular vessel and at least once every three months as to the duties the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented. The training entry shall include the date of the training and the general description of the training topics. The vessels owner or master could not provide logs or evidence of training IAW vessel emergency procedures. Vessel shall conduct training and provide evidence to the attending MI. 46 CFR 185.420(a),(c)

Each EPIRB must be tested monthly using the integrated test circuit and output indicator, to determine that it's operative; the EPIRB test shall be logged or otherwise documented. Vessel logs did not indicate a test of the EPIRB. Ensure monthly tests are being documented in the vessel log. 46 CFR 185.728(c)

Engines #1, 2, and 4 have oil leaks. Repair oil leaks.

Wiring for shower float switch are not properly spliced. Repair wiring so that it is appropriately insulated.
