The vessel experienced an equipment failure while transiting navigable waters of the US which adversely affected its meneuverability and posed a hazard to vessels and ifrastructure along the Houston ship channel. Submit for review a class report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanant repairs have been completed and that the vessel full and normal manuverability has been restored. 74 SOLAS (14) II-1/26

The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The SMS should provide for spacific measures aimed at promoting reliability of such equipment, The vessel experienced equipment failure while transiting the navigable waters of the US which adversly impacted its fitness and capability to safely maneuver the Houston ship channel. After review of the critical equipment list. PSCO discovered that the SSDE are not included. Failure of the #3 SSDE governor provide objective evidence that the company has failed to identify that failure of this equipment will result in a hazardous situation.
