No person may operate an ocean going ship of 400 gross tons and above unless it is fitted with approved 15 parts per million oily-water separating equipment for the processing of oily mixtures and have no discharge connections to the bilge system, oily bilge water holding tank(s), and tank top. Vessel had discharge connections to the bilge system, oily bilge water holding tank (s), and oily water separator.

Each operation described in 33 CFR 151.25 shall be fully recorded without delay in the Oil Record Book so that all the entries in the book appropriate to that operation are completed. Each completed operation shall be signed by the person or persons in charge of the operations concerned and each completed page shall be signed by the master or other person having charge of the ship. Multiple entries in the Oil Record Book were missing or otherwise made incorrectly, and unsigned by the person in charge of the operation. Multiple completed pages in the Oil Record Book were not signed by the Master of the ship. Vessel is to create a new Oil Record Book and demonstrate correct entries to marine inspectors.

A rescue boat is not required for a vessel operating on the continental shelf of the United States, if (1) the OCMI determines the vessel is arranged to allow a helpless person to be recovered from the water; (2) the recovery of the helpless person can be observed from the navigating bridge; and (3) the vessel does not regularly engage in operations that restrict its maneuverability. An OSV serves as its own rescue platform when the use of equipment which remains attached to the vessel is used to affect recovery. The vessel did not have a safety harness to allow a crewmember to be safely lowered to affect the MOB recovery. Vessel procured a safety harness prior to completion of the inspection.

Cargo ships of 300 tons gross tonnage and upwards must be provided with a receiver capable of receiving international NAVTEX service broadcasts. NAVTEX failed the international portion of the self-test. FCC Technician attended the vessel and replaced the NAVTEX and associated equipment. New test was conducted and witnessed by the CG inspection team. Test SAT.

Each battery used in an item of survival equipment and clearly marked with an expiration date must be replaced during the annual inspection and repair if this date has passed. The batteries on the EPIRB and 01 SART on board were both expired. Vessel crew replaced the EPIRB and the SART battery prior to completion of the exam.

The name of the vessel must be plainly marked or painted on each Emergency Position Indicating Radio Beacon (EPIRB) and on each Search and Rescue Transmitter (SART). The name of the vessel was not marked on the EPIRB or either SART. Crew used a p-touch to attach the vessel name to the EPIRB and SARTs.

Means shall be provided for stopping all fans in ventilation systems serving machinery and cargo spaces and for closing all doorways, ventilators and annular spaces around funnels and other openings to such spaces, from outside these spaces, in case of fire. Vessel's ventilation shutdown for the engine space from the bridge is not operable. Vessel restricted to Subchapter L (OSV) operations till ventilation can be properly shutdown from outside the engine space.

Each lifejacket must have a lifejacket light approved under approval series 161.112 or 161.012 securely attached to the front shoulder area of the lifejacket. However, lifejacket lights bearing Coast Guard approval number 161.012/2/1 are not permitted on OSVs certificated to operate on waters where water temperature may drop below 10 °C (50 °F). Vessel has expired life jacket lights and/or lights that will not be permitted if continuing to operate in area in winter months.

Each radio on the vessel must use the same, identical MMSI. 2 VHF radios in the GMDSS suite have different MMSI numbers than those assigned on the vessel's Radio Station Certificate. 47 CFR 80

Each portable fire extinguisher must be inspected, maintained, and hydrostatically tested as required by Chapter 4 of NFPA 10 with the frequency specified by NFPA 10. Carbon-dioxide and halon portable fire extinguishers must be refilled when the weight loss of net content exceeds that specified for fixed systems by Table 132.350. Further, each must be examined for excessive corrosion and for general condition. A tag issued by a qualified servicing facility, and attached to each extinguisher, will be acceptable evidence that the necessary maintenance has been conducted. Portable fire extinguisher for the paint locker was not serviced in past year. Have properly serviced by qualified servicing facility.

A vessel that operates more than a 100 miles from shore must be equipped with A DSC-capable single sideband radiotelephone meeting the requirements of § 80.1101(c)(4) and capable of operating on all distress and safety frequencies in the medium frequency and high frequency bands listed in § 80.369(a) and (b), on all of the ship-to-shore calling frequencies in the high frequency bands listed in § 80.369(d), and on at least four of the automated mutual assistance vessel rescue (AMVER) system HF duplex channels (this requirement may be met by the addition of such frequencies to the radiotelephone installation required by paragraph (a)(2) of this section). SSB radio on the vessel is showing error; cannot exceed beyond 100 NM from shore till properly repaired. 47 CFR 80

Each connection to a battery terminal must be made with a permanent connector, rather than with spring clips or other temporary clamps. Batteries in engine space have wing nuts for battery terminal connection, which is considered temporary. Replace with permanent connector.

A telephone operated from the pilothouse that reaches the master's stateroom, engineer's stateroom, engine room, and crew accommodations that either—(i) Is a sound-powered telephone; or(ii) Gets its power from the emergency switchboard or from an independent battery continuously charged by its own charger. Sound powered phone in engine space is not properly sounding. Repair as necessary.

Nonconductive mats or grating must be provided on the deck in front of each switchboard and, if the switchboard is accessible from the rear, on the deck behind the switchboard. Vessel needs non-conductive mats.

The instruction placards must contain the recommended “Emergency Instructions” listed in § 131.340 that, in the judgment of the cognizant OCMI, apply. The placards must be further designed to address the equipment, arrangement, and operation peculiar to each vessel. On vessel's emergency instructions for "Man Overboard", instructions mention launching a rescue boat. Vessel does NOT have a rescue boat. Re-write emergency instructions as per vessel's actual instructions to recover a man overboard.

Each marine shipboard cable must meet all the requirements for construction and identification of either IEEE 1580, UL 1309, IEC 60092-353 or NPFC MIL-C-24640A or NPFC MIL-C-24643A. Vessel has extension cord running to reefer. Needs to be permanently wired or remove extension cord.

Vessel repairs, renewals, materials, and workmanship shall be satisfactory to the marine inspector. Vessel work on propeller shall be to the satisfaction of the attending marine inspector. 46 CFR 126.110 46 CFR 127.210 (a)

Scope of annual inspection for vsl safety and seaworthy condition shall include inspection of main/aux machinery & piping. MI noted two (02) dry rotted fuel hoses on Bow Thruster and STBD gen in E/R

FIRE PUMP PSI GAUGE IS REQUIRED AT OPERATING STATION. GAUGE ON BRIDGE IS INOP. VSL MUST PROVIDE OPERATIONAL FIRE PUMP PSI GAUGE AT OPERATING STATION. 46 CFR 62.35-15

Generator switchboard and exterior emergency lighting on the starboard survival craft were not operational when on emergency power. Replace bulb to ensure operation when on emergency power.

Vessel did not have a fuel oil transfer diagram at fuel manifold. Vessel shall provide transfer diagram IAW 33CFR155.720.

Replace wasted hose on the number 9 portable fire extinguisher.

Repair leak on the number 2 hydraulic power unit piping.

Plugs on the port aft, port forward, and starboard forward cargo containment areas on deck did not fit properly due to wasted/painted threads.

Gasket located on the enggine room emergency escape hatch does not have the proper weather proof gasket material. Replace current gasket with proper weather proof gasket.

Non-conductive matting placed in inpropper location. Ensure matting is of adaquate size and in propper location in front of the switchboard.

Update MMSI to updated vessel name.
