Pumps, pipes, valves, and fittings in essential systems on vessels must meet ABS Rules for Building and Classing Steel Vessels Under 90 Meters in Length. A deck hatch to a fuel tank was not properly secured, leading to the unintentional discharge of fuel into the water. Further investigation determined that the underlying cause of the overflow was a failure in the return line from the main service tank to a designated bunker tank. Inspect, repair, or replace the faulty return line to restore proper fuel flow and containment. Bilge should be kept free of oil. Too musch oil in engine room bilge. Remove oil.

Fire hazards should be minimized. Small fuel leak on starboard main diesel engine. Make repairs.

On 09 April 2023 at approx. 16:00 the ITV ERIC HANEY experienced problems with the starboard MDE while underway with barges at mile 748 Ohio River. The Master of the vessel tied the ERIC HANEY off a a fleet located nearby for company repair personnel to troubleshoot on 10 April 2023. On 10 April 2023 the company repair personnel were unable to start the starboard MDE. Vessel required to proceed to a shipyard in Paducah, KY without barges for troubleshooting and repairs to the satisfaction of the vessel's TPO, ITOW. Contact Sector Ohio Valley duty marine inspector at [phone withheld] when repairs are completed and before placing the vessel back into service.

Clean all oil from bilge spaces and deck plates

Remove plastic trash can from engine room. Place used fuel filters in non-flamable container without standing fuel oil mixture in bottom.

Repair leaks on hydraulic steering system, clean and remove all drip buckets, rags, and diapers.

Remove standing fuel in fuel drain buckets on deck of day tank passageway.

Replace broken valve on hydraluic steering system

Place guard on rotating machinery on fuel pump and hydraulic steering pumps

Provide line diagram at fuel transfer station in accordance with transfer procedures.

Provide operational gauge on fire pump and fix water leak.

Provide proof of fuel system evaluation for clean supply of fuel to MDE.

Vesse1 sustained damage to starboard engine clutch and possible gear rendering the starboard engine inoperable. Prior to moving vessel from mile 281 on the Upper Mississippi River, company shall notify Sector Upper Mississippi on where they plan to have repairs completed and how they intend on bringing damaged vessel to repair facility.

Permanent repairs shal1 be completed to the starboard engine and operational tests conducted prior to the vessel departing port where repairs occur. Repairs must be completed to the satisfaction of an attending Coast Guard marine inspector. If company chooses to use their Third Party Organization (TPO) for oversight of repairs, a notification from TPO accepting these repairs must be submitted to the cognizant Coast Guard OCMI.

Vessel lost electrical power during transit to St. Louis for repairs to starboard engine from an earlier casualty. Prior to moving vessel, company must identify another towboat to provide assistance for remainder of transit to shipyard for repairs. Once safely moored at shipyard vessel is to contact Sector Upper Mississippi River command center.

Vessel must troubleshoot and determine reason for loss of power and make appropriate repairs if necessary to prevent recurrence of issue. Repairs shall be completed to the satisfaction of an attending Coast Guard marine inspector. If company chooses to use their Third Party Organization (TPO) for oversight of repairs, a report from the TPO accepting these repairs must submitted to the cognizant Coast Guard OCMI.

Engine room B-II's and B'V were depleted during a clutch fire.

Port gear box seal is leaking; which is allowed gear oil to spray on clutches and catch fire.

Lack retro tape on life preservers

The vessels fixed fire detection system was missing the certification letter from a registered professional engineer.

The vessel was lacking strobe lights that can be seen at all points in the engine room.

The vessel was missing general alarm strobe lights and audible alarms in other spaces located within the engine room aswell as other machinery spaces such as streering room and generator room.
