To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover.
During testing of remote fuel shutoff valves, the starboard side day tank and double bottom fuel tank both experienced failures of remote shutoff mechanical components rendering them non-operational.

The SMS should include procedures ensuring that nonconformities, accidents and hazardous situations are reported to the Company, investigated and analyzed with the objective of improving safety and pollution prevention.
The USCG issued a 835 to the vessel on 03 August 2021 referencing Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing in case of fire. Both port and stbd engine room ventilation closures/closing devices were found inoperable. The 835 was closed out by USCG on 05 August on 2021.
No objective evidence was provided that the vessel nor the company had included this deficiency in their NCR tracking date to prevent future occurrence. Further there is no procedure in the SMSM to instruct Master/Crew to forward the USCG issued 835 to DPA & Concerned Department.

Evidence:
The vessel raised a work repair request on 03 June 2021 for Repairs to the Bridge window. The Repair Requisition was approved by the company but its still in open state and pending repairs to date. Further no action was taken by the company to initiate a Management of Change (MOC) to extend or address a approximate timeline for completion of the repairs.
Nature of Non-Conformance:
The requirements of maintaining repairs in proper time frame was not followed.

Resolution: (1) Main switchboard breaker charge motor assembly was replaced. (2) A loose connection on a blocking diode in the PLC input circuit was re-connected by the service technician. (3) A loose faulty fuse was replaced by service technician.
