46 CFR 151.45-4 The COI must be endorsed, inspected, and approved for the carriage of flammable or combustible liquids, and such permit will serve as a permit for the vessel to operate. The Cargo Authority Attachment (CAA) is being reviewed by MSC to add additional cargos as per the owners request. Operations cannot take place until the CAA is validated and the COI is endorsed for the carriage.

111.105-5 in order to maintain system integrity each individual electrical installation in a hazardous location must not be installed in a manner that will compromise system integrity or safety. The shore tie cable, where entering the sealed cable tube, was pulled out exposing to the elements the individual wires within the sheathing. The sealed control panel box, STBD side amidships, was missing wire seal bushings compromising the integrity of the panel.

The Cargo discharge piping of all tank vessels shall be tested at least once each year for tightness, at the maximum working pressure. Examination of hydrostatic test for modified Cargo Pipe installation revealed port forward cargo valve and the aft suction valve for tank # 2 was leaking.
