Drills and instruction. The master of a towing vessel must ensure that each crewmember participates in a fire-fighting drills and receives instruction at least once each month. All crewmembers must be familiar with their fire-fighting drills duties, and specifically how to: Fight a fire in the engine room and elsewhere onboard the towing vessel, including how to operate all of the firefighting equipment onboard the towing vessel, Stop any mechanical ventilation system of the engine room and effectively seal all natural openings to space… Vessel deckhand was unaware of requirements of fighting a fire. Pushed the ventilation button and stated it was the fuel shut off valve, and had multiple other items during the drill which questioned knowledge. After review it was determined the vessel had only conducted 01 fire drill since April and the current Deckhand had not participated in a fire drill onboard the vessel.

Vessel General Alarm was inoperable and reported to the company on 10JUL2021 and operated until 25JUL2021. Vessel operator nor the Master notified the Sector and continued to operate in a hazardous condition. Only after notification of a scheduled in-service inspection was a technician hire “Eagle Control” to repair the General Alarm.

"To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The valve must be installed in the fuel piping directly outside of the fuel oil supply tank." During the on board inspection, discovered the remote fuel shut off valve is not directly aft of the tank, but rather an estimated 6 feet from the tank located mid-engine room. This incorrect placement allows the main fuel supply line that crosses across the engine room to remain full of fuel in the event of a fire, furthermore, if there is a break in the fuel line, fuel will not be able to be remotely shut at the tank, creating an unsafe condition.
