Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Port side watertight doors leading into the galley, and into engine room do not seal properly when fully closed. 46 CFR 144.320(b) 50c Port side porthole leading into engine room is cracked. Replace gasket for the doors. Replace porthole glass.

All lifesaving equipment must be tested and maintained in accordance with the minimum requirements of § 199.190 of this chapter. EPIRB has not been tested monthly, Capt unfamiliar with testing process. Monthly test not logged. 46 CFR 141.235(a) 50c

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. First aid kit is not complete, some of the medicine is expired. Replace as needed. 46 CFR 140.435 16c

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Mechanical ventillation dampers are wasted. Replace to ensure proper seal. 46 CFR 144.605 16c

A navigation assessment entry must be recorded in the TVR, official log, or in accordance with the TSMS applicable to the vessel. Entire month of October was not recorded in official logbook. 46 CFR 140.635(d) 16c

46 CFR 143.205 (a) All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Multiple fuel leaks were observed on the port and stbd generator fuel pump connections. Repair to the satisfaction of the attending marine inspector.

46 CFR 143.230(a)(5) Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided:(5) High bilge levels; Bilge alarm in aft enginroom bile inoperable. Repair/Replace to the satisfaction of the attending marine inspector.

46 CFR 143.555 A towing vessel, other than an excepted vessel, must have a backup or a second power source that has adequate capacity to supply power to essential alarms, lighting, radios, navigation equipment, and any other essential system identified by the cognizant OCMI or a TPO. The stbd generator is inoperable. Make repairs to the satisfaction of the attending marine inspector.

46 CFR 142.220(a)Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a) All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Engine room bile had excess oil and water in the bilge. Pump out bilges to the satisfaction of the attending marine inspector.

46 CFR 142.325 (a)(1) Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with paragraphs (a) through (d) of this section; or a portable pump, and hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. (a) A fixed fire pump must be capable of: (1) Delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot-tube pressure of at least 344 kilopascals (kPa) (50 pounds per square inch (psi)), and a flow rate of at least 300 liters per minute (lpm) (80 gallons per minute (gpm)) Pump was only able to provide 35 PSI.

46 CFR 140.610 (b) The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Port and stbd hatches had to be forced open and gaskets were deteriorated. Make repairs to the satisfaction of the attending marine inspector.

46 CFR 143.220 (c) Flammable and combustible products must not be stored in machinery spaces, unless they are stored in a suitable container that meets the requirements of § 142.225 of this subchapter. Oil was stored in the stbd side engine room.

46 CFR 143.560(a)(2) Each distribution panel or switchboard on a towing vessel must be: Totally enclosed and of the dead-front type. Panel in forward engine room had openings to the panel.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Vessel did not update digital GPS chart plotter with latest charts. Vessel did not have paper charts available. Update GPS plotter and obtain charts of area of operation. 46 CFR 140.705 (b) 16 C

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Vessel had 02 extinguished navigation lights on mast and 01 extinguished towing light on stern mast. Replace lights with approved fixtures. 46 CFR 140.720 16 C

All seals and gaskets must be properly maintained to prevent leaks of flammable liquid into the machinery space. Found a steady drip of diesel from the gasket for the diesel pump on the port side generator.

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge act. The vessel does not have a current Bridge-to-Bridge radiotelephone station certificate. 47 CFR 80.1005

Each vessel must carry on board an approved Vessel Security Plan (VSP) and a letter of approval from the Marine Safety Center. No approved VSP is available for this vessel. 705c: Due prior to towing a barge IAW 33 CFR 104.105

Each towing vessel must have a fire detection system installed to detect engine room fires and maintained in accordance with the manufacturer's design manual. Found paint covering the heat detector in the aft of the engine room and a loose heat detector on the starboard side of the engine room.

Paints, coatings, or other flammable or combustible products onboard must be stored in a designated storage room with a 40-B portable fire extinguisher located near the storage room. No portable fire extinguisher was located in the vicinity of the paint locker.

Each towing vessel must be maintained and operated so as to minimize fire hazards. Found metal support wiring holding the crankcase hose was chafing the hose and creating a hazardous condition over the port side main engine.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. A battery cap was missing on a battery bank outside of the operating station. Additionally, butterfly wing nuts were found on several batteries inside the machinery space. Replace with permanent type connectors.

The master must ensure all lifesaving equipment is properly maintained and ready for use at all times. Found an expired battery for the floating electric water light on the starboard side.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The middle light on the towing vertical line arrangement did not have a bulb and the vessel could not energize navigation lights other than the port and starboard running lights.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Vessel was missing three ball day shapes.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Search light, controllable from the vessel's operating station and capable of illuminating objects at a distance of at least two times the length of the tow. The vessels search lights could not be energized at the time of the inspection.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Illuminated magnetic compass. The magnetic compass did not have a light for night time operation.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. An exterior light on the starboard side bridge had a live wire exposed without a bulb and housing.

The official number of the vessel preceded by the abbreviation "NO." must be marked and permanently affixed to the interior structural part of the hull. The #3 from the Official Number was not affixed.

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard. Due to a change in ownership, submit Form CG-3752 "Application for Inspection of U.S. Vessel" to schedule the inspection for certification prior to conducting operations.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase in period. This vessel is one of the 50% of the vessels that have not obtained a valid COI. The vessel shall be made available for Coast Guard inspection within 90 days. If the action is not completed prior to this date, the action for this deficiency shall be changed to a code 60 (rectify deficiencies prior to movement) and the vessel will no longer be permitted to operate.
