46 CFR 143.205 - During testing of the port and stbd engine, the tachometer in the engine room and the one on the bridge did not mirror each other. In addition, a Code 14 ( low oil temp alarm) was found on the port engine and a Code 16 (low coolant alarm) was found on the stbd engine. Both engines showed low RPMs. Provide a 3rd party technician’s report attesting to the proper operation of both main engines and rectify the tachometer readings.

Vessel was u/w pushing the MCD 140 and had allision with the LCP 8 that was moored at Carline fleet mm 113 resulting in 6' x 8' x 8" inset on the LCP 8 stbd gunnel, #1 wing void. The Hazel Ann was reported not having any damage and the MCD 140 had very minimal damage, approximately 1' x 1' x 2" inset on bow.

IAW 46 CFR 140.605(b), a towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. IAW 46 CFR 144.330, the cognizant OCMI may require review of a vessel's watertight and weather tight integrity.
Vsl was observed listing significantly to the starboard side. While attending, vessel personnel reported that the common watertight bulkhead between the starboard and port potable watertanks was compromised with undetermined length of crack. Additionally it was observed approx 6-9 inches of water contained in the starboard side wing void of the vessel.
Prior to movement, operator shall submit to Coast Guard New Orleans for approval a transit plan to a confirmed repair facility, include actions taken to mitigate list.
Prior to resuming operations vsl shall submit for approval to Coast Guard Sector New Orleans, evidence of satisfactory stability/water tight integrity. Evaluation shall be performed by a Naval Architect or Professional Engineer.
