All piping, controls, valves, and alarms must be inspected; and the operation of controls, alarms, ventilation shutdowns, and pressure-operated dampers for each fixed fire-extinguishing system and detecting system must be tested, to determine that the system is operating properly. Marine inspectors observed that the pressure switch for the vessel's CO2 system was heavily corroded. Upon testing the switch ventilation for the vessel's machinery space did not shutdown. Repair and test system by a qualified technician to the satisfaction of the attending marine inspector prior to movement. 46 CFR 142.240(a)(5) 60c

The vessels containment had no means of closing the drsin hole. Must rectify prior to fueling operations.

Resolution: Hydrostatic test was conducted at 35 psi (working pressure of the system) and maintained pressure for over an hour. Attending CGMI inspected the repair area as well as the rest of the keel cooler. No leaks were detected; all sat.

Record keeping- Company did not do DOT required background check in the past on new employees, representative DER at the audit was under the impression that having new employees take a pre-employment drug test fulfilled the requirements.
