Each fuel line must be seamless and made of steel, annealed copper, nickel-copper, or copper-nickel. Each fuel line must have a wall thickness no less than 0.9 millimeters (0.035 inches) except for nonmetallic flexible hose complying with SAE J1942. Fuel hose busted spraying diesel fuel onto the STBD engine lagging causing a fire. Piping needs to be rerouted so that the fuel hose is not laying across lagging. Replace/repair and provide TPO report to the satisfaction of attending Marine Inspector. 46 CFR 143.265(d)

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure that all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Diesel fuel mixed with water was observed in the bilge due to the fuel hose burst. Clean out the bilge, providing receipt for proper disposal or TPO report attesting to the proper disposal to the satisfaction of the attending Marine Inspector. 46 CFR 142.220(a)

Machinery systems had melted plastic from light fixtures and fuel on them. Clean, test the proper operation of STBD Main, replace light fixtures and provide TPO report to the satisfaction of the attending Marine Inspector. 46 CFR 143.205(a)

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. The ventilation shut off was located in the engine room making it impossible for the crew to shut off ventilation during a fire in the engine room. Fix and provide TPO Report to the satisfaction of the attending Marine Inspector. 46 CFR 144.605
