Vessel is restricted to domestic voyage until repairs have been made to the satisfaction of the attending marine inspector.

On April 5, 2023, the OSG Endurance experienced a loss of power to the bridge and a loss of steering control. 46 CFR 143.205

Resolution: Technician report indicated a failed battery selector switch was the cause of the issue. Vessel made temporary repair with a spare switch. Switch replaced to oriiginal and proper operation verified to the satisfaction of the attending ABS Surveyor.

The Safety Management System should include procedures ensuring that non-conformities are reported to the company, investigated, and analyzed with the objective of improving safety and pollution prevention. SMS chapter ENV-01 Section 5 states when a activity poses an environmental risk, QR-SQE-25 Environmental Aspects Assessment Matrix is required to be followed Additionally chapter MR-04 requires a corrective action report to be generated if a defect presents a hazard to the crew or the environment. No objective evidence was provided to indicate that such reports were generated for the following: 1) Ballast water treatment system not capable of treating water that exceeds the water clarity standards required for UV intensity to be effective. 2) System found showing the following alarm: W154 UVR(1): UV intensity USCG low. System flow maintained at 50% Process not compliant. An internal audit is required to be conducted focusing on requirements for report non-conformities as outlined above.
