During further investigation, vessel crew discovered one of the two forward seawater cooling pumps inoperable after flooding in the compartment. Make permanent repairs to the effected pumps and make equipment available for inspection to MI's.

MI received notification that subject vessel discovered a raw water leak from the suction manifold of the forward seawater cooling system which resulted in flooding of the 2C compartment and isolation of various vital systems. Identify cause/extent of failure, provide repair proposal, and make permanent repairs to effected systems.

At least two lifebuoys on the vessel must be fitted with a self-activating smoke signal approved under approval series 160.157. Lifebuoys fitted with smoke signals must also be fitted with lights. Both smoke signals found expired.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel. Noted name and port rubbed off of ring life buoys and SOLAS retro-reflective tape damaged.

Vessels greater than 100 gross tons shall comply with 56.50–60, 56.50–85, and 56.50–90. There are multiple fuel leaks within the fwd and aft f/o transfer piping system. The DG4 automated f/o transfer shut-off is also not operating as designed. Vessel will repair and demonstrate proper operation, including DG4 daytank sensors. Until cleared, automated fuel transfers from the main f/o storage tanks are prohibited and all internal fuel transfers to diesel generator daytanks while vessel is underway must be under continuous remote monitoring by a LCC and/or SSV crewmember. ***This deficiency supersedes deficiency #1, issued 09/27/2023 by Activity 7787870*** Code 705: Deficiency is due by next DD or before 01Feb2024

Vessels greater than 100 gross tons shall comply with 56.50-60, 56.50-85 and 56.50-90. There are multiple f/o leaks on the fwd and aft f/o transfer manifolds. The DG4 automatic f/o transfer shut off is also not operating as designed. Vessel will repair and demonstrate proper operation including DG4 day tank sensors. Until this is cleared automated transfers from the main f/o tanks to the day tanks are prohibited and valves shall remain closed. All f/o transfers must be completed with a qualified PIC onboard the vessel to monitor transfer. 46 CFR 56.50-75

Life jackets must be stowed as follows: Readily accessible, the life jacket stowage positions must be marked with the words "Life Jackets" or appropriate symbol from IMO resolution. Where due to the particular arrangements of the vessel the life jackets required may become inaccessible alternate provisions must be made to the satisfaction of the OCMI include an increase in the number carried. Current stowage arrangement for life jackets on the vessel is required to be corrected to prevent damage of life jackets and accessories. Correct the satisfaction of the attending MI. 46 CFR 199.70 (b) (2)

Nothing in the subpart shall be construed as limiting the inspector from making such tests and inspections as he deems necessary to be assured to the the seaworthiness of the vessel. IAW 47 CFR 80.103 when using DSC (digital selective calling) techniques, coast stations and ship stations must use maritime mobile service identities (MMSI) assigned by the commission. The vessels VHF radio is not currently connected to GPS and the MMSI is input into the radio but with no location. Correct installation to allow for the DSC button to properly notify when pressed during an emergency. 46 CRR 91.27-15

The material, design, construction, workmanship and arrangement of the main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such must be at least equivalent to the standards established by ABS rules. The freshwater cooling for the cooling pump listed as 2 in Thruster room 1 is leaking. Repair to the satisfaction of the attending MI. 46 CFR 58.05-1

Each type I or II hull must have a weather tight weather deck. The flush scuttles that allow access to the f/o transfer spaces below deck have water leaking into the space. Replace gaskets to ensure spaces below the deck line are weather tight. 46 CFR 172.100 (a)

The inspection for certification shall include an inspection of machinery and equipment. Vessel crew is not familiar with how to properly test the bilge alarms in the ballast pump room. Prepare written instructions for proper testing in accordance with the vessels SMS. Test to the satisfaction of the attending MI. 46 CFR 91.25-10

Vessel has dead end wires located above the A/C unit in the thruster drive room 2. Repair to the satisfaction of the attending MI. 46 CFR 111.01-1 (a)(3)

AIS carriage- AIS class A device must be on aboard and properly installed on a self propelled vessel of 65' or more in length, engaged in commercial service. The vessels AIS was not properly functioning. Corrected. 33 CFR 164.46 (b)

The main steering and rudder stock must be of adequate strength for and capable of steering the vessel at maximum ahead service speed. The #2 azimuth steering motor for the #2 thruster was not operating as designed. Repair/replace to the satisfaction of the attending MI. 46 CFR 58.25-10 (b)

Exhaust pipe installations must conform to the requirements of part 1 section 23 of NFPA 302 incorporated by reference. Exhaust piping on the DG6 and DG2 were missing lagging, replaced in kind. Corrected. 46 CFR 58.10-5 (d)

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Of the vessel's 4 azimuth thruster propulsion units two currently have only 1 of 2 motors. Thruster # 1 and 4 must be repaired to the satisfaction of the attending MI. 46 CFR 58.10-5

When more than 12 nautical miles from the nearest land, any discharge of oil or oily mixtures into the sea from a ship other than an oil tanker or from machinery space bilges of an oil tanker is prohibited... One of four spaces containing diesel generators on board the vessel has standing fuel oil in the space. The #4 generator space experienced a leak of f/o, which penetrated the bulkhead cable penetration to the deck, washing into the sea. Approximately 25 gallons. The vessel owner will submit a transit plan before entering 12 nautical miles to cover mitigation of any further discharge into the sea. Once permitted to enter port the generator room #4 will be cleaned and surveyed by the attending MI before departure. 33 CFR 151.10 (a)

The number 4 generator's prime mover has an unknown fuel oil leak causing f/o to fill the compartment. Day tank has been secured and prime mover is not longer leaking additional f/o. All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. (46 CFR 58.10-10 Diesel Eng sends to Gas Eng). Of the vessel's 4 azimuth thruster propulsion unit #2 is not operable. The two motors required to operate the unit are not operating as designed. Repair to the satisfaction of the attending MI. 46 CFR 58.10-5

Each electric propulsion installation must meet section 4-8-5/5.5 of ABS Steel Vessel Rules. Arrangements for electric propulsion generators are to be such that propulsion can be maintained in case of failure of a power supply. In accordance with the Droneship Operations Manual, the following operations are conducted remotely and do not require personnel onboard the droneship: Activation of propulsion equipment. Prove operation of remotely activating propulsion equipment from the Launch Control Center.

Deck edges are protected by wire rope guard rails in two courses. Install second course around the periphery of the deck. DBA # 47

Nontank vessels subject to this subpart may not operate upon the navigable waters of the United States unless in compliance with a vessel response plan (VRP) approved under § 155.5065. Obtain a Non-Tank Vessel Response Plan. DBA # 14

Vessel will maintain digital records and work orders to cover the requirements for an unofficial logbook. Formalize documentation process for emergency drills and training for both dockside and offshore operations. DBA # 62

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Submit updated PSTPs and demonstrate compliance with DBA item #35. DBA # 35

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Establish DAPI Program and complete satisfactory audit. DBA # 20

Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Install missing machinery guards for accessory drives on generators 1-6. Repair cracked fuel pump guard on GEN2/3.

Exhaust pipe installations must conform to the requirements of ABYC P-1 and part 1, section 23 of NFPA 302. Repair exhaust leak on GEN1 and GEN5. Replace cracked exhaust bellow on GEN5. Provide missing exhaust flange bolt/nut on GEN5.

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Ensure all jacket water hoses have double hose clamps. Replace chaffed fuel supply line on GEN2. Jacket water leaks found at the j/w pumps on GEN4/5.

Droneship will display navigation lights according to COLREGS 72 rules. Ensure compliance with DBA item #1 and fix loose wire on starboard running light. DBA # 1

The respective piping systems installed on ships and barges shall have the necessary valves and relief valves for safe and efficient operation of the vessel. Repair air release valve in aft fuel oil pump room. Repair local position indications on FVAA300 and FVAB300 in fwd. fuel oil pump room.

The machinery space is equipped with leak detection and hydrocarbon sensors, which indicate if any significant fuel oil leaks or high fuel vapor concentrations occur in the space to prevent any potential flame or asphyxiation risk when the tank is opened. Calibrate hydrocarbon sensor in aft fuel oil pump room. DBA # 24

As outlined in the SpaceX Recovery SMS, section 07.13, upkeep/cleaning procedures for maintaining bilges are to be conducted only by offloading bilge water and oily waste in port to a shore side facility. Remove hydraulic oil from under the deck plates in T1. DBA # 25

Each panelboard must have a noncombustible enclosure that meets §§ 111.01-7 and 111.01-9. 208 panel missing blank in GEN1 space. DBA # 76

All other openings to a protected space must be capable of being closed. Louvers not closing all the way on GEN5.

The operators of each vessel and facility engaged in the transfer operation shall retain a signed copy of the declaration of inspection on board the vessel or at the facility for at least 1 month from the date of signature. Ensure all Declarations of Inspections are filled out/filed for all transfers for the required time period.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. The means for securing these doors weathertight shall consist of gaskets and clamping devices or other equivalent means and shall be permanently attached to the bulkhead or to the doors themselves, and the doors shall be so arranged that they can be operated from both sides of the bulkhead. Water tight doors and hatches on the 4160v generator and thruster spaces did not seal properly and were unable to be opened. Adjust WTD and hatches to facilitate proper sealing and operation.

The COFR Operator of a vessel must establish and maintain (or cause to be established and maintained) evidence of financial responsibility. The current COFR is for “all non-tank barges up to 6000 gross tons.” Vessel’s classification has changed from barge to industrial vessel. Update COFR as appropriate.

All vessels shall have efficient guard rails or bulwarks on decks...The height of rails or bulwarks shall be at least 39 1/2 inches from the deck...At exposed peripheries of the freeboard and superstructure decks, the rails shall be in at least three courses, including the top. Vessel railing has been removed.

Approved fire extinguishing systems may be used or required in locations delineated in this section on the following vessels. (1) On all self-propelled vessels other than yachts and fishing vessels. The case-by-case approval (CORR: 2020-0269, dated June 15, 2020) is no longer valid due to modifications to the spaces for which the systems were designed.

Systems protecting a single space not exceeding 6,000 cubic feet in gross volume may be installed without a stop valve if a suitable horizontal means of escape from the space exists. The enclosures for two separate generators have been combined, enlarging the gross volume of the space to 10,232.9 cu.ft.

In general, compliance with the structural standards established by the ABS will be considered satisfactory evidence of structural efficiency. Provide repair plan for making permanent repairs to the deck fracture on the main deck amidships.

On vessels of 1,000 gross tons and greater, a fixed carbon dioxide or clean agent system as described in 46 CFR subpart 95.16 must be installed in any space that contains internal combustion or gas turbine main propulsion machinery, or auxiliary machinery with an aggregate power of 1,000 b.h.p. or greater, or the fuel oil units of such machinery, including purifiers, valves, and manifolds. The FM200 fire protection system for the SSDG# 1 is currently not operational.

All fuel pipes, pipe connections, and accessories shall be readily accessible. The piping shall run in sight wherever practicable, protected against mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal liners or straps without sharp edges. Fuel lines going to Gen-sets on deck are not properly secured or protected from mechanical damage.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. SSDG # 3 had wasted/corroded fuel lines.

Shutoff valves of a suitable type shall be installed in the fuel supply lines, one as close to each tank as practicable, and one as close to each carburetor as practicable. Where fuel tanks are installed below the weather deck, arrangements shall be provided for operating all shutoff valves at the tanks from outside the compartments in which they are located, preferably from an accessible position on the weather deck. Fuel shut-off valve to/from fuel tank for low-voltage generator set is missing valve handle.

Exhaust pipe installations must conform to the requirements of ABYC P-1 and part 1, section 23 of NFPA 302. There was missing lagging on various areas of SSDG#1-6 and around turbo on 208V generator set.

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. The 480V generator set had plastic bowl on secondary fuel filter.

On vessels of 1,000 gross tons and greater, a fixed carbon dioxide or clean agent system as described in 46 CFR subpart 95.16 must be installed in any space that contains internal combustion or gas turbine main propulsion machinery, or auxiliary machinery with an aggregate power of 1,000 b.h.p. or greater, or the fuel oil units of such machinery, including purifiers, valves, and manifolds. Temperature sensors in generator enclosures are not properly affixed.

All other openings to a protected space must be capable of being closed. Doors, shutters, or dampers must be installed for openings in the lower portion of the space. Openings in the upper portion of the space must be capable of being closed off either by permanently installed means or by the use of canvas or other material normally carried on the vessel. Crop and renew wasted door framing on SSDG#2 and SSDG#3

Fixed agent cylinders must be stored outside the space being protected. Numerous FM-200 container pipe penetrations are too large for the pipe and do not adequately protect the cylinders. Piping, fittings and nozzles must be installed in accordance with the manufacturers operation manual. The FM 200 nozzle in the aft Command and Control module was obstructed by ventilation.

Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire with the frequency specified by NFPA 10. Establish scheduled maintenance program for all firefighting equipment.

On vessels of 1,000 gross tons and greater, a fixed carbon dioxide or clean agent system as described in 46 CFR subpart 95.16 must be installed in any space that contains internal combustion or gas turbine main propulsion machinery, or auxiliary machinery with an oil units of such machinery, including purifiers, valves, and manifolds. No fire suppression is installed in either 7P or 7C Fuel Oil Pump Rooms.

The shipboard oil pollution emergency plan must be onboard and identify the principal characteristics of the vessel. Fuel tanks have been added below deck and the SOPEP does not capture this arrangement.

Proposed removal of the motor generator and replacement with electrical transformer must be approved, installed to the satisfaction of, and demonstrated operation acceptable to attending USCG Marine Inspector.

No repairs or alterations affecting the safety of the vessel with regards to the hull, machinery, or equipment shall be made without the knowledge of the Officer in Charge, Marine Inspections. Fuel oil system coming from tanks 7P and 2S is currently not approved by CVC or MSC. Vessel is not authorized to use this fuel oil system until such approvals are received and workmanship tested and verified by local OCMI.

Electrical installations on vessels must ensure: protection of passengers, crew, other persons, and the vessel from electrical hazards. Vessel's current electrical arrangement condition does not meet previously approved electrical plans. Vessel is to address all comments on MSC letter E2-2100642, which identified numerous potential electrical safety hazards to the vessel and personnel.

No repairs or alterations affecting the safety of the vessel with regards to the hull, machinery, or equipment shall be made without the knowledge of the Officer in Charge, Marine Inspection. Evaluate structural damage identified in 1C, 6P, and 8C and provide a repair plan proposal to the OCMI.

The means of securing and maintaining weathertightness shall be to the satisfaction of the Administration. Replace worn and deteriorated hatch cover gaskets.

Vessels that operate in salt water must be examined in accordance with the intervals set forth in Table 91.40-3(a). Make vessel available for internal structural examination not later than 09 Apr 2021.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery or equipment shall be made without the knowledge of the Officer in Charge, Marine Inspections. Vessel is to tag out, lock-out temporary low voltage generator while underway or under tow.

Vessel shall have efficient guardrails or bulwarks on deck. The section of the vessel does not have any guard rails for fall prevention.

General arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads, together with particulars of the fire alarms, detecting systems, the sprinkler installation (if any), the fire extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating systems including particulars of the master fan controls, the positions of dampers, the location of the remote means of stopping fans, and identification numbers of the ventilating fans serving each section. Vessel did not have a fire control plan.

Initial inspection shall be such as to ensure that the electrical installation fully comply with the applicable regulations and are in accordance with approved plan. The vessel's on-board electrical installation does not match what was approved by MSC.

The initial inspection is to determine that the arrangement material and their installation meet this chapter and the approved plans. Unable to verify proper operation of electrical distribution system on-board.

Initial inspection shall be such as to ensure that the machinery installation fully comply with the applicable regulations and are in accordance with approved plan. Unable to verify installation and test the operation of thrusters and their associated components.

Electrical equipment must be arranged to prevent damage to the equipment. Two Seavan in the aft section of the vessel were actively leaking water from the overhead, thus creating an electrical hazard.

There must be ground detection for each electrical propulsion system and ship's service power system. Operator not able to prove proper operation of a ground detection system on-board.

Electrical equipment must be arranged to prevent damage to the equipment. Distribution panels inside medium voltage generator boxes are exposed to wet environment and are showing signs of corrosion.

All electrical installations in hazardous locations must comply with the requirements of IEEE. Vessel was unable to prove electrical installations in the rocket processing space meet IEEE standards.

All pressure vessels shall satisfactorily pass the hydrostatic test prescribed by regulations. No documentations was provided to verify the N2 expansion pressure vessel in the N2 Seavan was hydrostatically tested.

Each panel board must have a circuit directory card holder with designation of each circuit. Two breaker panels in the aft "command and control" Seavan did not have circuit directories.

Electric installations on vessels must ensure protection from electrical hazards. Cables and wires in the aft "command and control" Seavan were not properly secured in accordance with acceptable standards.

The material design and arrangement of main propulsion machinery shall meet these standards in ABS Rules. Aft port thruster has active hydraulic leak when in operation.

The vessel has a portable generator and fuel tank on deck that may be used to provide power to the 480 volt system and has not been reviewed or approved by MSC.

Prove proper installation of electrical distribution systems as per the plans that were submitted to MSC.

Prove proper operation of electrical distribution system after installation is completed.

Prove proper operation of thrusters to include safety devices, remote control, and general operations.

Prove Proper operation of all medium voltage generators under load.

Determine cause of water leaks in the SEAVAN and electrical rooms and make appropriate repairs.

Ensure all electrical equipment (switchboards, panel boards, distribution panels, controllers, etc) meets the appropriate degree of protection; distribution panels inside medium voltage generator boxes, electric equipment in spaces with actively leaking overhead.

Install Ground Indication/Detection.

Install Non-Conductive Deck covering along each switchboard OR prove current covering is sufficient.

Install emergency lighting in each Thruster room.

Install required Racor fuel filter Metal bowl on each Medium V Generator.

Demonstrate installation pressure tests for all pressurized systems, including “Rocket Processing” equipment. Tests must be conducted it 1.5 times the maximum allowable working pressure, in the presence of a USCG Marine Inspector.

Demonstrate hydrostatic testing of new section of piping installed in aft fire main.

Remove/relocate equipment blocking aft eyewash station and make easily accessible.

Ensure all panelboards and switching unit is provided with a circuit directory and that all blanks are installed in all panel openings.

Re-route air conditioning condensation drain lines so that they are not passing over electrical equipment.

Install means to prevent cables from chaffing and ensure all cables are secure and/or supported in accordance with IEEE Std 45.

Identify source of oil observed within aft, port thruster and, if a leak is found, affect repairs.

Ensure all electrical appliances in the Hazardous Location Space are installed iaw IEEE.

Based on current operational restrictions due to COVID-19 and IAW ALCOAST 100-20, the required Annual Inspection for Certification is deferred until not later than 15 June 2020; at which time the Annual Inspection will commence. The COI issued 15 January 2016 will remain valid, but a satisfactory USCG Inspection must be completed prior operation after 15June2020.

Vessel shall not operate diesel engines during international voyage. Vessel shall obtain an Engine International Air Pollution Prevention Certificate (EIAPP) from the EPA, and submit to OCMI for documentation and approval of International Air Pollution Certificate.

Perform periodic inspection.

Make permanent repairs to original scantling on deck (6) 10'x40' plates and wing support structure - prior to rocket operations.

Conduct annual COI inspection, prior to condcuting recovery operations.

Conduct post landing barge internal structural exam prior to next barge rocket recovery operation.

Conduct operation test of all propulsion thrusters.

Conduct test procedures for propulsion system.

Test emergency stops for all machinery.

Provide Load Line Certificate prior to operating outside the boundry line.

Repair combing rail and stanchions on port side.

Replace discharged portable fire extinguishers.

Repair port side power & cable supply lines & conduit and prove proper operation of all equipment/machinery IAW approved test procedures.

Replace damaged hydraulic hose on the stbd bow thruster with SAE J 1942 with hose end fittings that comply with SAE J 1475.
