All electrical engineering details and installations shall be designed and installed in accordance with subchapter J of this chapter. Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Area in forward stores, used for housing fire fighting gear, does not have an adequate emergency source of lighting. 90.25-1(a) 46 CFR 111.75-15(c)(2)

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. The EOS "A" Class Fire door has an improperly installed gasket. Repair or replace to the satisfaction of the attending USCG Marine Inspector. 46 CFR 92.07-10(c)

Each muster station and embarkation station must be adequately illuminated by lighting with power supplied from the vessel's emergency source of electrical power. Starboard embarkation light is inoperable. Repair or replace to the satisfaction of the attending USCG Marine Inspector. 46 CFR 96.06-1 46 CFR 199.110(c)

Each immersion suit or anti-exposure suit must be marked in such a way as to identify the person or vessel to which it belongs. Immersion suits are not appropriately marked. Rectify to the satisfaction of the attending USCG Marine Inspector. 46 CFR 96.06-1 46 CFR 199.70(c)(3)

Immersion suits or anti-exposure suits must have the following attachments and fittings: Each immersion suit or anti-exposure suit must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the immersion suit or anti-exposure suit. Rectify to the satisfaction of the attending USCG Marine Inspector. 46 CFR 96.06-1 46 CFR 199.70(c)(4)(i)

Demand instrumentation displays must be clearly readable and immediately available to the operator. Bridge and Engine alarm panels both display incorrect date and time for alarms. Rectify to the satisfaction of the attending USCG Marine Inspector. 46 CFR 96.03-1 46 CFR 62.25-20(c)

The integrity of any deck in way of a stairway opening, other than a stairtower, shall be maintained by means of “A” or “B” class bulkheads and doors at one level. The integrity of a stairtower shall be maintained by “A” Class doors at every level. The doors shall be of self-closing type. Holdback hooks, or other means of permanently holding the door open will not be permitted. Galley "A" Class door was installed with a holdback hook. Deficiency rectified. 46 CFR 92.07-10(d)(4) 46 CFR 92.07-10(g)

Radio equipment required by this subpart must be maintained to provide the availability of the functional requirements specified in § 80.1081 and to meet the performance standards specified in § 80.1101. Vessel GMDSS equipment (INMARSAT) was in error mode, and last known GMDSS (with exception of EPIRB) checks were logged in 2021. Provide objective attesting to proper function of GMDSS equipment. 47 CFR 80.1105

Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm set points, the procedure to be followed, and the expected test result. Vessel was not conducting Periodic Safety Test Procedures IAW approved CG test procedures, must do so in presence of CG MI. 46 CFR 61.40-10(a)

Starboard shaft starting overheating. Vessel Master decided to shutdown engine to prevent any furthur damage. Vessel transited to Corpus for repairs and be cleared by Class society.

Each self-propelled vessel must have duplicate light sources for the side, masthead and stern lights. Port side duplicate side light is not operational.

Immersion suits and their locations are not reflected in the safety plan.

Provide proper seal for the vent in the starboard Z drive room.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. A damage survey must be conducted to assess damage and required repairs to hull, between frames 6-35, port azimuth thruster, and fuel tank and its appurtenances. Any temporary or permanent repairs must be made to the satisfaction of the attending marine inspector. Prior to departure from current location, you are required to submit a comprehensive salvage plan for approval, including rigging of the vessel, pollution mitigation of the exposed fuel tanks, and maintaining structural/watertight integrity of the port azimuth thruster. Following salvage, you must submit a comprehensive transit plan that describes how the vessel will be moved and where the vessel will be moved to from its current location. A strict communication schedule will be established with the Coast Guard for the duration of the transit. You are not authorized to proceed until the Coast Guard has reviewed your transit plan.

A survey, either general or partial, according to the circumstances shall be made every time an accident occurs which affects the safety of the vessel or the completeness of its life saving appliances, fire-fighting or other equipment, or whenever any important repairs or renewal are made. Prior to movement, copies of all surveys relating to reported loss of propulsion will be provided to satisfaction of the attending Marine Inspector.

Vessel experienced the loss of both generators, but was able to restart generator number two. The cause of the generator loss could not be determined at sea. Vessel is to proceed to Miami to have a technician complete diagnostics on the cause of the casualty and provide a report to the local OCMI. Vessel must also demonstrate proper operation of both generators prior to departure from Miami.

Construction of vsl must be made to minimize fire hazards. Cable transit run in engine room has to be flush w/ bulkhead section. It is petruding bulkhead by 2 inches and athwartship passageway bulkhead and overhead penetration not adequate.

Vessel shall have 08 Life rings, 50% must be fitted with self igniting lights. Currently vessel life rings have only 25% outfitted with self igniting lights.

Life Buoy with self activating smoke signals must be stowed that when released fall directly into water without striking any part of the vessel. Currently Life Buoy with smoke signal when released will land on deck and not enter water.

Life Buoys fitted with smoke signals must also be fitted with lights. Currently Life buoys fitted with smoke signals are not fitted with lights.

Vessel does not have a rescue boat and regularly engages in operations that restricts maneuverability. Vessel must submit proposal to local OCMI on how it plans to meet the requirement of a rescue boat.
