Vessel master reported the M/E #6 cylinder head gasket suffered a failure. Ships crew isolated cylinder and proceeded to anchorage to receive techs on board. Provide RO/class survey attesting to the proper repair and operation of the MDE. 74 SOLAS (2014 Cons.) II-1/26.1

Vessel reported a main engine slowdown alarm due to a low temperature exhaust deviation on the #5 cylinder causing a reduction of propulsion. Conduct repairs to the satisfaction of class and Coast Guard.

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed, and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration. The master of the vessel reported a high temperature alarm on the scavenger air box while the vessel was underway in the USCG Sector St. Petersburg OCMI zone. The main engine was shut down by the crew after receiving the alarm. The crew reported that a temperature sensor for the scavenger air was found faulty and the crew replaced the temperature sensor with an OEM sensor. Provide a class report attesting to the proper operation of the main engine prior to departing the Sector St. Petersburg OCMI zone. The deficiency is not "self-reported" since it has a marine casualty nexus that is required to be reported to the Coast Guard as per 46 CFR 4.05-1. SOLAS 2014 Cons. Ch II-1 Reg 3-1

This vessel is authorized a one-way voyage while abiding by the approved transit plan, from New Orleans, LA, to Tampa, FL. Upon arrival, all cargo must be offloaded, cargo tanks certified gas-free, and conduct permanent repairs that have also been reviewed and approved by the Vessel Response Plan's designated Salvage & Marine Firefighting provider. These conditions stated shall be completed no later than June 15, 2019, and be completed prior to the loading of any cargo.

While loading cargo at Marathon Refinery in Garyville, LA, vessel's installed ballast water management system was unable to prime. Vessel given one-time approval to discharge approximately 18,741 MT of ballast water that did not meet 33 CFR 151.2030 discharge standard into the lower Mississippi River. Vessel must propose alternative means of discharging ballast water IAW approved ballast water management plan to the OCMI, prior to entering the Mississippi River system in ballast.
