The technical and/or operational deficiencies documented below, along with the owner/operator's failure to notify the Coast Guard of these conditions IAW 46 CFR 137.215(d) provide objective evidence of a serious failure and lack of implementation of the vessel's TSMS. IAW 46 CFR 137.212, the owner/operator is required conduct an external survey, followed by an external audit, satisfactory to the OCMI, after corrections from the survey are completed.
