DELTA DEANNA

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. At approximately 1500 local time, on 05NOV2025, Sector San Francisco received a report that the ITV DELTA DEANNA (O/N: 1274217) ran soft aground near Point Molate, north of the Richmond-San Rafael Bridge. Provide a statement attesting to the satisfactory structural and watertight integrity of the hull and satisfactory condition of the propulsion appendages to include the propeller, rudder, and shaft. Additionally, sound all tank and void spaces. 46 CFR 140.205(a)

The owner of a vessel subject to inspection under section 3301 shall- (1) ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to the individuals described in paragraph (2); (2) establish a list of all crew, identified by position, allowed to access and use the master key and maintain such list upon the vessel, within owner records and included in the vessel safety management system; (3) record in a log book information on all access and use of the vessel's master key, including- (A) dates and times of access; (B) the room or location accessed; and (C) the name and rank of the crew member that used the master key. Vessel did not have a master key control system in place. Implement to the satisfaction of the MI. 46 USC 3106(a)

Each operation described in paragraphs (d), (e) and (f) of this section shall be fully recorded without delay in the Oil Record Book so that all entries in the book appropriate to that operation are completed. Each completed operation shall be signed by the person or persons in charge of the operations concerned and each completed page shall be signed by the master or other person having charge of the ship. Several pages in ORB missing master signature and did not follow instructions in the front of the manual including listing the name of the tank, leaving blank spaces between entries, and not recording the unit of measurement after some entries. 33 CFR 151.25

The owner or managing operator must maintain valid proof of registration [for vessels EPIRB]. EPIRB registration found with incorrect call sign. 46 CFR 141.380(d)

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Provided first aid cabinet and wound bag contained almost all expired medicines and ointments. 46 CFR 140.435

The vessel owner or operator must ensure that they maintain one English language copy of the VRP, at a minimum the contents listed in paragraph (c)(1), (2), (3), (5), (6), (7), (9) and (10) of this section and a copy of the Coast Guard approval letter, onboard the vessel. No copy of the approved NTVRP was available onboard during the inspection. 33 CFR 155.1030(g)(1)

A response plan submitted to meet the requirements of § 155.1035 or § 155.5035 must identify the training to be provided to persons having responsibilities under the plan, including members of the vessel crew, the qualified individual, and the spill management team. A vessel owner or operator shall ensure the maintenance of records sufficient to document this training and make them available for inspection upon request by the Coast Guard. Records must be maintained for 3 years following completion of training. No training records for crew could be provided during the inspection. 33 CFR 155.1055(a)(b)

record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Required information found missing from vessel records. 46 CFR 140.820

The cognizant OCMI may require review of a vessel's watertight and weathertight integrity. Vessel found with valves in a transverse watertight bulkhead serving as part of the ventilation system. These valves were not readily operable nor was there a means of closure in the immediate vicinity. 46 CFR 144.300

Each towing vessel must be equipped with at least one fire axe that is readily accessible for use from the exterior of the vessel. All onboard fire axes were stored within the vessel accommodation spaces. 46 CFR 142.227

The emergency duties and duty stations required by this section must be posted at each operating station and in a conspicuous location in a space commonly visited by crewmembers. Vessel found missing required information for identification of duties and duty stations for each person onboard during an emergency. 46 CFR 140.405(c)

In each crew berthing area, the owner or operator of a vessel must be equipped with information in a visible location regarding the following: 1) vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage. Company policies were not posted as required. 46 USC 11101

There must be an installed or portable bilge pump for emergency dewatering. Any portable pump must have sufficient hose length and pumping capability. All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended back flooding through bilge piping. The bilge suction valve reach rod in the forepeak is inoperable. Make repairs to the satisfaction of the attending Marine Inspector. 46 CFR 143.275

All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months. The portable fire extinguishers have not been serviced within the past 12 months. Provide documentation of servicing of portable fire extinguishers to the satisfaction of the attending Marine Inspector. 46 CFR 142.240(a)

Marine employers may form or otherwise use sponsoring organizations, or may use contractors, to conduct the random chemical testing programs required by this part. Provide proof of drug and alcohol program management agreement with consortium. 46 CFR 16.230(d)

Each pyrotechnic distress signal carried to meet this section must be stowed in a portable watertight container carried at the operating station. Portable watertight containers for pyrotechnic distress signals must be of a bright color and must be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high with “DISTRESS SIGNALS”. Current container for pyrotechnic distress signals is not bright color and needs to be replaced to the satisfaction of the OCMI. 46 CFR 141.375(e)(1)

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 1 inch high "WATERTIGHT DOOR-KEEP CLOSED". MI observed 5 watertight doors that were not marked: 2 doors on main deck entrance forward and aft, 1 door to the engine room, 1 separating the engine room and aft steering, 1 between forward engine room and crew accommodations.

A towing vessel that is certificated for oceans must be equipped with both an approved 160-B semi-portable fire- extinguishing system to protect the engine room and a fixed fire-extinguishing system installed to protect the engine room. Discovered the required 160-B semi-portable extinguisher missing on board the vessel. 46 CFR 142.315(b)(1)

The owner of a vessel must designate a name for the vessel on the Redocumentation form CG-1258 submitted to the National Vessel Documentation Center when the owner elects to change the name of the vessel. Vessel did not have updated COD on board with updated name "DELTA DEANNA." 46 CFR 67.117(a)(2)

Stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel did not have updated FCC Station License on board with updated name "DELTA DEANNA." 47 CFR 80.13(a)

The bridge-to-bridge radiotelephone station will be inspected and the vessel will carry a certificate with a valid endorsement. Vessel did not have updated FCC Bridge-to-Bridge Cert with updated name "DELTA DEANNA." 47 CFR 80.1005

Vessel must complete visual inspections at least once per week or per voyage, whichever is more frequent; unless the vessel engages in multiple voyages per day, in which case a daily inspection is required. Vessel must also have a comprehensive inspection annually and record all results in the vessel's record keeping documentation. Vessel did not have updated VGP on board with updated name "DELTA DEANNA." VGP Parts 4.1.3 and 4.2

Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Discovered burned out emergency light bulb on stbd aft part of the pilothouse. 46 CFR 143.410(b)

Each carbon dioxide fire extinguishing alarm, spaces protected by carbon dioxide fire extinguishing system, and spaces into which carbon dioxide might migrate shall be conspicuously marked. Vessel's alarm, engine room space, and corresponding adjacent spaces discovered not marked with required signage alerting crew members of vacate requirements. 46 CFR 78.47-9, 11

Each towing vessel must perform the applicable tests in Table 143.245(b). Vessel was not performing required tests within the outlined frequencies (I.e.: Weekly, at least once every 3 months, etc.). 46 CFR 143.245(b)

Electrical panels containing more than one source of power must be fitted with a sign warning persons of this condition and identifying where to secure all sources. Discovered no required placard in the engine room space. 46 CFR 143.250(b)

Each towing vessel must be equipped with navigation day shapes IAW 33 CFR 83 for if aground or towing astern. Vessel did not have required day shapes in diamond and three ball configurations. 46 CFR 140.720, 33 CFR 83.24, 33 CFR 83.30(d)(ii)

Each fuel shutoff remove valve control must be marked in clearly legible letters, at least 1 inch high, indicating the purpose of the valve and the way to open it. Discovered no labeling for fuel shutoff valve point. 46 CFR 143.260(e)

The emergency duties and duty stations must be posted at each operating station and in a conspicuous location in a space commonly visited by crew members. Vessel did not have the required station bill posted. 46 CFR 140.405(c)
