Each liferaft must be stowed to permit manual release from its securing arrangements. Pins used to secure and release life raft launching appliance found seized; implement periodic inspections on system to ensure readiness. 46 CFR 199.130 (c)(1)

The transfer procedures required by § 155.720 must be: Permanently posted or available at a place where the procedures can be easily seen and used by members of the crew when engaged in transfer operations. Fuel transfer procedures not posted or available in the E/R or near the fuel transfer manifold. 33 CFR 155.740 (c)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced partial loss of power on their starboard engine. Starboard engine RPM's drop to idle, vessel needed to de-clutch then clutch back in to regain engine RPM's. Conduct repairs, provide tech report, and conduct sea trials to the satisfaction of the MI. 46 CFR 143.205(a)

Each survival craft must be stowed— So as not to require lifting from its stowed position in order to launch, except that— A survival craft that weights 185 kilograms (407.8 pounds) or less may be lifted not more than 300 millimeters (1 foot) in order to launch. In addition to the requirements of paragraph (a) of this section, each liferaft must be stowed as follows: Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed. A liferaft stowage arrangements meets this requirement if it— (i) Is outboard of the rail or bulwark;(ii) Is on stanchions or on a platform adjacent to the rail or bulwark; or (iii) Has a gate or other suitable opening large enough to allow the liferaft to be pushed directly overboard and, if the liferaft is intended to be available for use on either side of the vessel, such gate or opening is provided on each side of the vessel. Vessel's liferaft was located so that it must be lifted more than 1 foot to launch. In addition, vessel's liferaft was not stowed to permit it to drop into the water from the deck on which it is stowed. Modify liferaft stowage to the satisfaction of the MI. 46 CFR 141.310 46 CFR 199.130 (a)(7)(ii) & (c)(3)

Every towing vessel equipped with an inflatable survival craft must have, in conspicuous places near each inflatable survival craft, approved placards or other posted instructions for launching and inflating inflatable survival craft. The vessel's liferaft did not have launching instructions posted near the liferaft. Add instructions in conspicuous location to the satisfaction of the MI. 46 CFR 141.320

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles; or a portable pump, and hoses and nozzles. Vessel's fixed fire pump was leaking from the shaft seal at approximately 2 cups per minute. Portable fire pump placed onboard. 46 CFR 142.325

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Four rocket projectiles, each complete with bridle and leader of fire-resistant materials. Vessel only have two rocket projectiles onboard. 46 CFR 141.385 46 CFR 160.040-4(a)

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford protection against hazards to life, property, and the environment. Vessel experienced a fire in the stbd exhaust stack during operations. Determine cause of the fire and make repairs to the satisfaction of the attending CG Marine Inspector.

A vessel that is exposed to salt water more than 6 months in any 12 month period since the last examination or initial certification must undergo a drydock and internal structural examination at least twice every 5 years, with not more than 36 months between examinations. Vessel has not conducted required drydock examination. 46 CFR 137.700(a)(1)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The vessel's starboard generator was inoperable. All repairs to be conducted to the satisfaction of the OCMI. 46 CFR 143.400(a)

Each towing vessel operating in regions north of lat. 32° N. must carry at least one approropriate-size immersion suit, approved under approval series 160.171, for each person onboard. The vessel was missing one immersion suit and one was not properly serviced. The vessel is limited to carry 6 total persons onboard. 46 CFR 141.350(a)(1)

Fixed fire-extinguishing systems must be inspected and tested, as required by table 142.240 of this section, in addition to the tests required by 147.60 and 147.65 of subchapter N of this chapter. Owner/operator to provide evidence of satisfactory servicing of the fixed fire-extinguishing system to the cognizant OCMI.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the EPA, the operator must secure each Type III device in a manner which prevents discharge of sewage. Securing device was not found for the vessel's marine sanitation device. 33 CFR 159.7(c)

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under the approval series 160.040. The vessel's line throwing device has been expired, and is restricted to limited coastwise operations. 46 CFR 141.385

Verify that the vessel's crew conducted abandon ship and man overboard drills under simulated emergency conditions. Underway portion of exam was not conducted. 46 CFR 137.220(g)(9)

Emergency lighting sources must provide for sufficient illumination under emergency conditions and must be automatic, battery-operated with a duration of no less than 2 hours. Emergency lighting not operable for three locations in galley/crew accommodations and one location below deck in crew vestibule. 46 CFR 143.410(b)(1)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Vessel liferaft discovered past due for required annual servicing. 46 CFR 141.230

Each towing vessel operating north of lat. 32deg N must carry the number of immersion suits as prescribed in this paragraph. Vessel identified only 6 immersion suits on board, whereas COI requires 8 immersion suits for route. 46 CFR 141.350(a)

Propulsion machinery, including main engines must be maintained to ensure proper operation. Discovered exhaust leaks IVO turbochargers for all main engines and generators. 46 CFR 143.205(c)(1)

Pressure vessels must be equipped with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP. Pressure vessel relief valves discovered rated at 275 psi whereas MAWP is stamped at 250 psi. 46 CFR 143.300(a)

All seals must be properly maintained to prevent leaks of flammable or combustible liquid. Hydraulic leak discovered on port z-drive inboard rotation actuator. 46 CFR 143.220(a)

Each integral fuel tank must have a vent that is fitted with a 30x30 mesh corrosion-resistant flame screen. Vent pipe for 5S 6C f/o tanks missing mesh flame screen and vent pipe for 6S 7S 7C f/o tanks missing two screws to secure flame screen. 46 CFR 143.265(c)

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard for review. The following minimum information is required in the record: The dates when exams were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the exams. Vessel unable to provide towing gear maintenance records/logs. 46 CFR 140.820(b)

Machinery must be designed to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Crew unable to properly line up bilge suction line; require simple instructions posted in vicinity of system for easy access. 46 CFR 143.205(a)

Each towing vessel must carry on board and maintain the following: U.S. Coast Guard Light List, River-current tables, Tide tables, and U.S. Coast Pilot. Vessel unable to provide copies of Light List, Coast Pilot, or Tide/current tables. 46 CFR 140.705(a) and 33 CFR 164.72(b)(2)

Each lifejacket must be marked in block capital letters with the name of the vessel and have a whistle secured by a cord to the lifejacket. Discovered three lifejackets missing whistles and four lifejackets missing stenciled name. 46 CFR 141.340(f)(1) and (g)(2)

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid into the machinery space. Discovered leak on the port engine access cover for front gear case due to bad gasket. 46 CFR 143.220(a)

Verify that all hazardous situations are corrected, with all protective devices in satisfactory condition. Discovered worn, loose, and missing lagging from chillwater system in the engine room. 46 CFR 137.220(l)

Each towing vessel must perform the applicable tests in Table 143.245(b). Vessel was not performing required tests within the outlined frequencies (I.e.: Weekly, at least once every 3 months, etc.). 46 CFR 143.245(b)

On the front compartment of each work vest there shall be stenciled in waterproof ink "WORK VEST ONLY." Discovered work vests not stenciled with required lettering. 46 CFR 140.430(a) and 46 CFR 160.053-5(b)

Each EPIRB must be tested monthly. Vessel did not have records of monthly EPIRB tests. Test and record EPIRB on a monthly basis. 46 CFR 199.190

Vessels on Oceans route are required to carry 6 day and 6 night visual distress signals. Vessel had onboard 4 hand flares, 2 parachute, and 2 smoke. Provide required number of visual distress signals onboard. 46 CFR 141.370
