The vessel's VHF DSC radios have not been programmed with the vessel's latitude and longitude input, invalidating the DSC capability of the installations. 46 CFR 140.715(a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel reported loss of steering due to slippage of port propeller while departing Bolivar Terminal. Port propeller made contact with rudder. Divers notated damage to shaft. Effect permanent repairs to affected machinery to satisfaction of Coast Guard Marine Inspector. Objective evidence in the form of a satisfactory report from TVIB will satisfy this requirement.

Closure devices must be provided for deckhouse or hull penetrations which open to the exterior of the vessel and which may allow water to enter the vessel. Port and Starboard WTD's to galley and engine room on main deck have gaskets that fit poorly with large gaps.

Resolution: Vessel rep replaced gaskets with proper width gasket material with the seam at the top of the door.

Pressure vessels over 5 cubic feet in volume and over 15psi MAWP must be equipped with one or more spring loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding MAWP. Valves on 150 psi rated air receiver had pressure relief valves set at 200 psi.

Resolution: Vessel master replaced pressure relief valves with a 150 psi setting on the air reciever and on both air compressors.
